In the first days, you may have a dressing, temporary catheter, swelling or discomfort, and limits on activity. Sexual activity is commonly postponed for several weeks—published patient guidance ranges from four to eight weeks—but only your surgeon can clear you to resume it.
How the procedure affects recovery
Peyronie’s surgery is not one operation with one recovery plan. The European Association of Urology (EAU) says the choice depends on factors including penile length, curvature severity, erectile function, and expectations. It recommends surgery when the disease is stable and the deformity impairs intercourse. The aim is a functionally straight penis, not necessarily a return to the penis’s previous length or shape. Plication
Plication straightens the penis by shortening the side opposite the plaque. Some shortening is therefore a recognized tradeoff. The exact postoperative routine depends on your surgeon’s technique and instructions.
Plaque incision or excision with grafting
Grafting is used for selected severe or complex deformities to lengthen the affected side. It can carry risks to erectile function and sensation. It does not guarantee preservation of prior length or function.

Penile prosthesis
An implant is generally considered when erectile dysfunction does not respond to medication; a straightening maneuver may also be performed. An implant is not designed to restore the penis’s previous length. Instructions written for implant surgery should not automatically be applied to plication or grafting.
The EAU reports that no particular shortening procedure or graft has been proven superior to its counterparts. Your surgeon can explain why a specific operation fits your anatomy and goals.

What may happen in the first hours and days
Depending on the operation and the care team’s routine, you may go home the same day or stay overnight. Mayo Clinic says a catheter may be removed in the recovery room or before discharge the next morning, and that a light pressure dressing is often left on for 24 to 48 hours. Pain medicine and a short course of antibiotics may be prescribed. Expect some discomfort or swelling, but follow the discharge plan you were given rather than changing wound care or medication on your own.

If you had prosthesis surgery
The University of Washington Department of Urology describes observation in recovery, with zero or one overnight stay, and a catheter or drain that may be removed the next day. Its instructions also include daily bandage changes for five days, keeping the penis dry for two days, and intermittent ice-pack use on the first day. Wound care and activity: follow your own discharge plan
There is no universal wound-care or exercise schedule in the cited guidance. One reconstructive urology practice, for example, advises keeping its clear dressing in place for two to three days, then showering and patting the suture line dry, while avoiding soaking until the wound is fully healed. Its protocol also recommends supportive briefs or a jock strap with the penis positioned upward for two weeks, resting at home for two to three days, avoiding significant activity for two weeks, and avoiding activities that stress the penis for six weeks. Ask your surgeon what “significant activity” means for your procedure and whether your work, commute, exercise, or lifting needs special limits. The cited practice identifies running, cycling, and surfing as activities that can stress the area; its advice about lifting is specific to that protocol, not a universal rule.

Patient-facing recommendations differ: the Urology Care Foundation advises avoiding sex for at least six weeks, while Mayo Clinic and University Hospitals give a range of four to eight weeks. These are general timelines, not automatic clearance dates. Wait until your operating surgeon says it is safe, since healing and procedure type matter.
Risks and realistic expectations
Potential risks listed by Mayo Clinic include loss of feeling, erectile dysfunction, additional shortening, infection, and—rarely—recurrent curvature. NIDDK notes that plication may shorten the penis and that numbness or erectile dysfunction can occur after grafting. Which risks matter most depends on the operation and your health. The EAU describes the surgical goal as a “functionally straight” penis. Discuss what that means for your curvature, sexual function, sensation, and length expectations before surgery; a technically straight result does not necessarily mean the same length or erectile function as before.

- Which procedure am I having, and what are its expected tradeoffs for length, sensation, and erectile function?
- When should my dressing or catheter be removed, and what wound-care instructions should I follow?
- What activities, work duties, and exercise should I avoid, and when can I resume them?
- When will you reassess healing and decide whether I can resume sexual activity?
- Whom should I contact if I am concerned about a change or symptom after discharge?
Postoperative warning-sign guidance varies, and the sources cited here do not establish one universal emergency checklist. Use the contact instructions given by your own surgical team for concerns about your recovery.
