CAR-T therapy involves making a treatment from your own immune cells, an infusion, and close monitoring afterward. Your hospital stay and recovery plan depend on the CAR-T product, treatment center, and your health; your care team can give you the schedule and instructions that apply to you.
What happens before and during CAR-T infusion?
Cell collection and manufacturing
CAR-T cells are made from a patient’s own T cells. The cells are collected, altered in a laboratory to add chimeric antigen receptors, expanded, and returned to the hospital for treatment. The National Cancer Institute (NCI) says this manufacturing process takes about 3 to 5 weeks from collection to infusion. That estimate covers cell production, not the full evaluation, any bridging treatment, hospital stay, or recovery.
Infusion and observation
After the finished cells are infused, the team watches closely for side effects that can emerge after treatment. Monitoring may take place in hospital or through an outpatient pathway, depending on the product, your clinical circumstances, and the center’s protocol.

How long will I stay in the hospital after CAR-T?
There is no single hospital-stay length that applies to everyone. Center guidance illustrates the range: University College London Hospitals (UCLH) describes a pathway with at least 14 inpatient days, with a possible transition to ambulatory care or home; Memorial Sloan Kettering Cancer Center (MSK) describes its adult inpatient example as 7 to 10 days or longer. These are center-specific pathways, not a universal average, and complications can extend a stay. Clinical-trial instructions may also differ from routine-care guidance.

What to ask about inpatient and outpatient monitoring
Ask your treatment team how its pathway works for you. The practical differences are where monitoring happens, how often you must return to the center, how close you need to stay, whether a caregiver must be available, what to do if symptoms arise, and how long local monitoring is expected. The team decides whether a pathway is suitable and provides its instructions; neither option is right for everyone.
U.S. FDA monitoring instructions for certain products
In the United States, the Food and Drug Administration (FDA) says updated labeling for six listed autologous CAR-T therapies calls for monitoring for at least two weeks, including daily monitoring for at least one week; staying within proximity of a healthcare facility for at least two weeks; and avoiding driving for two weeks after administration. These are product-specific U.S. labeling instructions, not a universal rule for every CAR-T therapy worldwide. Confirm the current instructions for your product and center.

Which symptoms need urgent attention?
Two important acute risks are cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS). CRS can cause fever and flu-like symptoms and may become serious, including with low blood pressure or breathing difficulty. Neurologic effects can include confusion or difficulty speaking. NCI notes that side effects vary and that the care team cannot know in advance exactly when they will occur or how serious they will be.

What should caregivers and people traveling for treatment plan?
Caregiver support
A caregiver may help with transportation to appointments, watch for changes in symptoms or behavior, and stay with you after discharge during early recovery. Dana-Farber’s guidance describes support that may extend up to 30 days depending on the patient’s situation; this is center guidance, not a universal requirement. Ask your team what support it expects and for how long.
Accommodation and transport
If you live far from the treatment center, ask whether you should arrange short-term accommodation nearby. Dana-Farber advises patients to plan for nearby lodging, while the FDA’s U.S. labeling for the named products calls for remaining near a healthcare facility for at least two weeks. Your center may have more specific distance rules. Also ask about appointment frequency, transportation, insurance or financial support, and lodging resources.

What does recovery and follow-up look like?
The first month and beyond
Dana-Farber describes recovery as typically the 30 days after infusion, but that is not a fixed finish line. Some patients need frequent visits or further hospitalization, and caregiver support may need to continue depending on recovery. Fatigue and reduced appetite can persist after discharge.
Follow-up appointments
Follow-up schedules are individualized. MSK’s adult patient guide gives one center’s example: visits at around 30 days, 90 days, 6 months, and 1 year, followed by coordination with a local oncologist. Your own timing may differ, so ask which appointments will be at the treatment center and which can take place closer to home.

Long-term safety monitoring
The FDA says manufacturers’ required postmarketing observational safety studies include 15 years of follow-up to assess secondary malignancies and long-term safety. This study requirement does not mean every patient has the same clinic-visit schedule for 15 years.
