Ask the responsible clinician to check and correct the note before it is signed or added to your record. If you discover a mistake later, identify the exact wording, explain what was actually said or omitted, and ask the clinician or provider’s records team how it will be reviewed or amended. Treat errors involving medicines, allergies, diagnoses, treatment, or follow-up as time-sensitive; contact the treating team promptly, and use local urgent or emergency services if there is immediate danger.

What an AI medical scribe is—and why a fluent note can still be wrong

An ambient or AI medical scribe listens during an appointment and produces a draft clinical note. It may organize the conversation into sections such as symptoms, history, assessment and plan, but it does not turn the conversation into a verified medical record by itself. The clinician who used it remains responsible for checking the content.

A note can sound polished while changing the meaning of a conversation. NHS England identifies accuracy, contextual understanding and completeness as specific risks. Complex medical terminology, abbreviations, rapid speech, accents or dialects, and speech impairments can all affect transcription. The system can also mistake a hypothetical statement for a confirmed diagnosis or omit a detail that changes the clinical picture.

That is why the safe dividing line is not “AI-generated” versus “human-written.” It is unreviewed draft versus clinician-verified record.

If the clinician is still reviewing the draft

Errors are easiest to fix before the note is saved or signed. NHS England’s professional guidance says inaccuracies should be corrected before information is included in the record. Great Ormond Street Hospital describes a local workflow in which the clinician checks and edits the draft and adds relevant details manually before saving. Guidance from the American Psychological Association likewise tells clinicians to review and edit AI-generated documentation before signing.

  1. Compare the draft with the encounter. The clinician should consider what was actually said, what was observed, examination findings, test results and the agreed plan—not just whether the sentences read naturally.
  2. Check meaning-changing details first. Verify negation (“no chest pain” versus “chest pain”), medication names and doses, allergies, dates, times, laterality (left versus right), and follow-up instructions.
  3. Confirm who said what. A patient’s concern, a family member’s report and the clinician’s assessment are not interchangeable. The note should preserve that distinction.
  4. Separate possibilities from facts. Statements such as “What if this is an allergy?” or “Could it be pneumonia?” must not appear as confirmed diagnoses unless the clinician actually made that diagnosis.
  5. Fill omissions manually. If the conversation covered a relevant symptom, risk, decision or safety-net instruction that the draft missed, add it before signing.
  6. Save or sign only after review. The professional user remains accountable for the correctness of the final documentation, even when software produced the first draft.

If you notice a problem while the clinician is looking at the note, point to the specific sentence and explain the correction in plain language. A focused correction is more useful than a general statement that “the AI got it wrong.”

Common mishearing and omission patterns to check

What can go wrong Why it matters What the reviewer should verify
Negation is lost or reversed “No fever” can become “fever,” changing triage or treatment decisions. Positive and negative symptoms, denials, and safety warnings.
Medication, dose or allergy is misheard An incorrect medicine list can lead to prescribing or interaction errors. Drug name, strength, route, frequency, recent changes and allergies.
Hypothetical language becomes a diagnosis A question or possibility may be recorded as an established condition. Whether each diagnosis is confirmed, suspected, ruled out or merely discussed.
Chronology is flattened When a symptom started or a dose changed can determine the next step. Onset, sequence of events, prior treatment and response.
The speaker is misidentified A relative’s observation can be mistaken for the patient’s symptom or the clinician’s conclusion. Attribution of each important statement.
A relevant detail is omitted An incomplete history can leave the next clinician without information needed for safe care. Symptoms, examination findings, patient preferences, decisions and follow-up instructions.

These checks reflect the risks described in NHS England’s version 3 guidance for England, published 27 April 2025 and updated 29 July 2026. They are a practical review aid, not a validated scoring instrument.

If you discover an error after the note was saved

Start with the provider that created the note. Contact the treating clinician, clinic or medical-records team and identify the discrepancy precisely. Explain three things:

  • Where it appears: the date of the encounter and the exact sentence, section or medication entry.
  • What happened: what you said, what the clinician said, or what detail was left out.
  • Why it matters: for example, it changes an allergy, diagnosis, treatment instruction, symptom history or follow-up plan.

Ask the provider to have the responsible clinician review the original encounter and tell you how that organisation handles corrections or amendment requests. Keep your description factual and specific. Do not assume that deleting the original entry is the correct remedy: clinical-record rules often require an auditable correction rather than silent replacement, and the exact process depends on the provider and jurisdiction.

What to include in a request

A short written request can reduce ambiguity:

  • your name and the date and location of the appointment;
  • the note section or wording you are challenging;
  • the corrected wording or omitted fact, stated as clearly as you can;
  • the potential care consequence, if any; and
  • a request for the clinician or records team to explain the review and amendment route.

Providers may ask for additional identification or use a specific form. There is no single form, deadline or procedure that applies everywhere, so follow the instructions from the organisation holding the record.

When a documentation error is urgent

Do not wait for a routine records request if the incorrect note could affect a current decision. Contact the treating team promptly and say that the record contains a potentially care-affecting error.

Type of discrepancy Practical response
Spelling, wording or a minor historical detail with no effect on care Request ordinary clinician review or the provider’s records-correction process.
Wrong medicine, dose, allergy, diagnosis, test result or follow-up instruction Contact the treating team promptly, before relying on the note for care.
Immediate symptoms, danger or a treatment decision that cannot wait Use local urgent or emergency services. Do not wait for the record to be amended.

The Agency for Healthcare Research and Quality has discussed diagnostic-safety implications of inaccurate, inconsistent or biased AI-generated notes. The practical point is that documentation quality can influence later clinical decisions; urgency depends on the patient’s situation, not on whether the error came from software.

How formal record amendments differ by location

The core safety advice is broadly consistent—review and correct the record—but the legal and administrative route is not universal. NHS England’s patient and professional materials describe expectations for services in England. They do not establish a worldwide patient-rights rule, a common deadline or one mandatory form.

The American Psychological Association’s “Guidance for the Evaluation of AI Scribes” is professional guidance for psychologists, not a general statement of every patient’s legal rights. Great Ormond Street Hospital’s description is a local hospital process. Other providers may use a different workflow, such as an amendment request, an addendum, a patient-portal message or a formal health-information-management review.

Ask the organisation holding the record:

  • which team receives correction or amendment requests;
  • whether the request must be submitted through a portal, form or written letter;
  • how the original entry and any correction will be displayed;
  • how you will be notified of the review outcome; and
  • how to escalate if the correction is refused or the response does not address the safety issue.

If you are outside England, use your provider’s records policy and the rules that apply in your country or state. A clinician can correct a clinical note, while a formal patient-record amendment may involve a separate records or privacy process.

If you do not want ambient scribing used

NHS England’s patient guidance says a patient can tell the professional at the start of an appointment that they do not want an ambient scribe. Policies and applicable rules vary, so ask what alternatives are available if you object. You can also request clarification about when the tool is active and whether the clinician will review the draft before it enters the record.

Raising an objection does not require you to prove that the system made an error. Consent expectations, service policies and local law determine how a provider handles the request.

What clinicians and organisations should do after an error

A correction fixes the individual note; a safety review looks for a repeatable failure. Organisations using scribes should follow their local clinical-safety and records policies. NHS England advises organisations in England to assess product accuracy and safety processes and to report relevant safety concerns through appropriate channels.

  • Preserve the clinically relevant correction in the record’s audit trail.
  • Review whether the failure involved terminology, microphone quality, overlapping speech, accent, speech impairment or workflow design.
  • Check whether the clinician had time and a clear responsibility to review before signing.
  • Look for similar errors in medication, allergy, diagnosis and follow-up fields.
  • Update training, prompts or escalation procedures when a recurring pattern is found.

These controls do not make an AI scribe error-proof. They keep the accountable professional in the decision loop and make a discovered error less likely to harm the next step of care.

A quick patient checklist

  • Read the visit summary or portal note when it becomes available.
  • Mark the exact sentence or entry that is wrong or incomplete.
  • Write what was actually said or omitted, without guessing at a medical explanation.
  • State whether the discrepancy involves a medicine, allergy, diagnosis, treatment or follow-up.
  • Contact the treating team promptly when care could be affected.
  • Ask which correction or amendment process the provider uses.
  • Use urgent or emergency services for immediate danger rather than waiting for paperwork.
