Stop the agent first, then preserve evidence before attempting cleanup. Interrupt the run, revoke pending approvals, disable automations, and block further writes or external calls. Next, determine what actually changed, contain any exposed credentials, recover from the smallest verified known-good state, and validate it before restoring normal access.

An agent can affect far more than the file or chat window you can see. Connected tools may write to repositories and databases, deploy code, alter cloud resources, send messages, change tickets, expose secrets, or trigger financial and administrative actions. Stopping execution prevents additional effects; it does not undo work already committed.

1. Stop further effects immediately

Treat a destructive agent action as an active incident until you know its full scope. Do not begin by deleting the agent’s logs or forcefully “cleaning up” the workspace; those actions can destroy the evidence needed to understand what happened.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Interrupt the run. Use the agent interface’s stop or cancel control, terminate the worker process if necessary, and cancel queued jobs.
  2. Revoke pending approvals. Withdraw approvals that could still authorize commands, deployments, data exports, or external API calls.
  3. Disable automation paths. Pause schedules, webhooks, CI/CD jobs, event consumers, and integrations that could cause another invocation.
  4. Block additional writes and calls. Remove write access or place affected systems in a maintenance or read-only mode. Restrict outbound network access when an external service may still be receiving requests.

Microsoft’s Visual Studio Code guidance is explicit: “If the agent is about to run a destructive command, deploy a change, expose sensitive information, or modify an external service, stop the request before you investigate further.” Apply that principle even when the command has already started: halt what remains, then investigate.

Stopping execution is containment, not rollback. A database transaction, deployment, message, deletion, or credential change that has already been committed will remain until you reverse it through the appropriate system of record.

2. Preserve evidence before changing the scene

Make an untouched evidence copy before restoring, rotating, deleting, or overwriting anything. Preserve records in a location the agent and its credentials cannot modify. Record who collected each item and when.

Capture the agent activity

  • Start and stop timestamps, time zone, run ID, and user or service account.
  • The complete prompt and subsequent conversation, including tool-approval decisions.
  • Model identifier, agent framework, tool versions, configuration, system instructions, and enabled extensions.
  • Commands, API requests, parameters, response payloads, return codes, and error output.
  • Changed-file lists, commit IDs, deployment IDs, job IDs, object versions, and database transaction identifiers.
  • Screenshots or screen recordings of the agent state and any warning or approval dialog.

Capture affected-system state

  • Repository branches, tags, reflogs, uncommitted diffs, and artifact hashes.
  • Database schemas, audit records, transaction logs, row counts, and backup metadata.
  • Cloud-resource configurations, identity-policy changes, deployment manifests, and infrastructure state.
  • Ticket, message, email, payment, or other external-service records showing attempted and completed actions.
  • Access logs, authentication events, secret-manager audit trails, and network telemetry.

Keep a read-only or otherwise protected copy. If legal, privacy, or security review may be required, follow your organization’s retention and chain-of-custody rules before performing destructive cleanup.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Map the blast radius

Separate what the agent attempted from what a system accepted. Tool output can be incomplete, and an apparent failure may have occurred after a write succeeded. Check each connected system directly.

Area Questions to answer Useful evidence
Files and repositories Which paths changed, disappeared, or gained unexpected content? Were commits pushed or branches force-updated? Git log and reflog, commit diffs, file hashes, editor history, filesystem snapshots
Databases and data stores Were rows deleted, transformed, exported, or permissioned differently? What is the earliest known-good transaction or backup? Audit and transaction logs, point-in-time recovery markers, row counts, backup catalogs
Deployments and infrastructure Did a build reach staging or production? Were infrastructure, DNS, firewall, or identity settings changed? CI/CD records, deployment history, cloud audit logs, configuration snapshots
Credentials and secrets Could the agent read, print, copy, or transmit tokens, keys, passwords, or personal data? Secret-manager access logs, proxy logs, chat transcripts, provider activity records
External services Were messages sent, tickets edited, purchases initiated, or third-party objects deleted? Provider-side audit trails, webhook deliveries, message IDs, billing and activity logs

Mark each finding as confirmed, attempted but unconfirmed, or not observed. Identify the earliest known-good checkpoint and the latest point at which effects may have continued. This distinction defines the recovery window and prevents an apparently successful rollback from leaving hidden changes behind.

4. Contain identity and access

If an agent could see a credential, assume it may have been copied until logs prove otherwise. Containment should be scoped to the identities and systems in the blast radius, but it must be fast enough to prevent reuse.

  1. Disable the agent’s service account or remove its write and administrative roles.
  2. Revoke active sessions, OAuth grants, short-lived tokens, API keys, SSH keys, and cloud credentials that were available during the run.
  3. Rotate replacement secrets from a trusted workstation or secret-management process; do not paste new secrets into the affected conversation or workspace.
  4. Review sign-in, token-use, network, and provider audit logs for use after the incident began.
  5. Apply temporary deny rules or read-only policies while recovery is underway.

Production deployment, bulk deletion, financial activity, administrative changes, and externally visible actions are high-impact operations. They require bounded permissions, explicit human approval, and an independent policy check rather than reliance on a single confirmation prompt.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Choose the smallest safe recovery action

Pick a recovery that restores the affected function while preserving evidence and limiting collateral change. The right option depends on reversibility, dependency effects, data-loss window, authority required, and confidence that the resulting state is clean.

Recovery action Best fit Advantages Risks and checks
Restore a workspace checkpoint or known-good Git commit Wrong files, generated code, configuration, or unpushed changes Usually fast and highly reversible; scope can be limited to selected paths Does not undo database, deployment, or external-service writes; preserve uncommitted evidence first
Roll back a deployment Bad code or configuration already released Returns traffic to a previously built artifact with a known deployment record Database migrations and side effects may not roll back; verify compatibility and background jobs
Restore verified data or use point-in-time recovery Deleted or corrupted database and object data Can recover a broad, consistent state from a defined timestamp May lose legitimate changes after that point; test restoration separately before cutover
Isolate or disable the affected component Ongoing corruption, uncertain scope, or unsafe dependency Stops propagation while investigation continues Can interrupt legitimate service; document owners, duration, and safe operating mode
Fall back to a safe service or manual process Recovery cannot be trusted quickly enough for continued operation Maintains essential function without exposing the damaged path Capacity, data synchronization, and privacy controls must be checked before use

Use the narrowest restoration that returns the system to a known-good state. Do not assume that killing one process reverses downstream writes, and do not overwrite the only copy of altered data before it has been preserved.

6. Recovery by failure type

Wrong files, deleted code, or altered configuration

  1. Freeze the working tree and copy the current state, including untracked files.
  2. Compare the affected paths with the earliest known-good commit, workspace snapshot, or artifact.
  3. Restore only the required files or create a separate recovery branch; avoid rewriting unrelated legitimate work.
  4. Review build scripts, dependency manifests, environment files, and generated artifacts for changes outside the obvious target.
  5. Run focused tests, linting, configuration validation, and a clean build before merging or redeploying.

Production deployment or infrastructure change

Stop promotion and place the affected service in a controlled mode. Identify the exact artifact and infrastructure revision that reached production, then roll back to a previously validated version. Check migrations, queues, scheduled jobs, caches, DNS, firewall rules, and permissions separately; an application rollback cannot automatically reverse those dependencies. Keep enhanced monitoring in place during reactivation.

Database or customer-data deletion

Prevent further writes that could make point-in-time recovery ambiguous. Establish the deletion interval from transaction and audit logs, then restore a copy into an isolated environment. Compare schemas, row counts, checksums, referential relationships, permissions, and representative business queries with the known-good baseline. Obtain the data owner’s approval before replacing production data, and account for legitimate transactions that occurred after the recovery point.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Secret or personal-data exposure

Rotate or revoke every potentially exposed credential, not only the one visible in the transcript. Determine whether the value was logged, returned by a tool, stored in an artifact, sent to an external endpoint, or included in a deployment. Preserve relevant access records, apply the organization’s breach and privacy-notification process, and restrict copies in caches, build logs, and chat exports. A password change alone is insufficient if the agent retained a valid session or service token.

External actions such as messages, tickets, or transactions

Use the provider’s audit and reversal mechanisms where available. Contact the system owner before deleting records that may be needed for investigation. For financial, legal, customer-facing, or irreversible actions, escalate immediately and document who authorized each compensating action. Notify affected parties through the established incident process rather than through the compromised agent.

7. Validate the restored state before reopening access

Recovery is complete only when the result is both usable and trustworthy. Validate in an isolated or restricted environment first, then perform a controlled cutover.

  • Completeness: expected files, rows, objects, permissions, indexes, queues, and relationships are present.
  • Correctness: schemas, configuration values, dependencies, and business outputs match the selected baseline.
  • Integrity: hashes, signatures, checksums, and audit trails show no unexplained alteration.
  • Safety: malware, unexpected accounts, unauthorized jobs, backdoors, and leaked secrets are absent.
  • Function: focused smoke tests and critical user journeys succeed, including authentication, writes, backups, and integrations.
  • Observability: alerts, logs, rate limits, and access monitoring are active for the reactivation window.

Compare restored files, schemas, permissions, dependencies, and outputs with a known-good baseline. Reopen access in stages, beginning with the smallest audience and least privilege, and keep a named owner watching for regression.

NIST SP 800-61 Rev. 3, published in April 2025, treats preparation, detection, response, and recovery as an organizational capability. The decision record should identify the incident lead, technical recovery owner, communications owner, and person authorized to accept residual risk. Escalate even when the visible damage appears small if the agent had broad privileges or the evidence is incomplete.

9. Close the loop and prevent a repeat

Record a timeline from the first prompt through containment, recovery, validation, and reactivation. Include root cause, affected systems, data-loss window, evidence locations, decisions and approvers, user impact, residual risk, and follow-up owners.

NIST SP 1800-25 emphasizes backups, secure storage, integrity checking, audit logs, and access controls for protecting data from corruption or destruction. NIST SP 1800-26 addresses detection and response for destructive events involving databases, system files, configurations, application code, and customer data. Use those principles to improve the environment rather than merely adding another warning message.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Give agents task-specific, time-limited permissions and separate read, write, deploy, and administrative identities.
  • Require approval gates plus independent policy checks for destructive, financial, administrative, production, or externally visible actions.
  • Make high-impact operations idempotent where possible, with dry-run and scoped-preview modes.
  • Maintain immutable or access-controlled backups, tested restore copies, versioned artifacts, and known-good commits.
  • Keep tamper-resistant audit trails for prompts, tool calls, approvals, identities, and outputs.
  • Provide a reliable interrupt and rollback mechanism; OWASP’s AI Agent Security Cheat Sheet specifically recommends that users be able to “interrupt and rollback agent operations” and calls for controls beyond a simple approval prompt.
  • Run incident scenarios in which an agent performs unintended or destructive actions, including rapid credential revocation, isolation, known-good rollback, secure replacement, enhanced monitoring, dependency review, and vendor coordination where applicable.
  • Tabletop the runbook and measure tested recovery time, data-loss window, and the authority needed for each recovery path.

The goal is not to make an agent incapable of useful work. It is to ensure that every consequential action has bounded authority, an auditable decision trail, and a recovery path that has been exercised before production use.
