Feeling well does not show whether a high LDL cholesterol level is affecting your long-term heart and stroke risk. High cholesterol usually causes no symptoms, so review the full lipid panel with a healthcare professional and discuss your overall risk before deciding what to do. A high result does not, by itself, mean that everyone needs the same treatment.
Why you can feel fine with high LDL
High cholesterol commonly has no noticeable symptoms. The only way to know your cholesterol levels is through a blood test, not by how you feel. The American Heart Association emphasizes regular checks for this reason: you may not feel any symptoms of high LDL. American Heart Association: About Cholesterol
That absence of symptoms does not tell you whether your level is harmless. LDL cholesterol is one part of cardiovascular risk, and a lipid panel provides measurements—including LDL, HDL, triglycerides, and total cholesterol—that a clinician can interpret alongside your health history.
What to do next
- Review the complete result. Bring the lipid panel or make sure your clinician can see it. Confirm which value is high—LDL, non-HDL cholesterol, triglycerides, or another measure—and ask what goal or range applies to your risk.
- Share the details that shape risk. Be ready to discuss your age, blood pressure, diabetes and other health conditions, smoking status, current medicines, and personal and family history of heart attack or stroke.
- Ask how your risk will be assessed. For U.S. adults aged 30–79 being evaluated for primary prevention, the 2026 ACC/AHA dyslipidemia guideline calls for PREVENT-ASCVD risk assessment, followed by personalization and a clinician–patient discussion. - Discuss whether more information would help. The guideline highlights measuring lipoprotein(a), or Lp(a), at least once in a lifetime; selective apolipoprotein B (apoB) testing; and coronary artery calcium (CAC) scoring in selected cases when it may clarify or reclassify risk.
- Agree on a follow-up plan. Ask what changes to make, when to recheck cholesterol, and how you and your clinician will judge whether the plan is working.
How the 2026 U.S. guideline frames treatment discussions
The guideline uses risk categories to inform a conversation; they are not an individual prescription or a promise of benefit. Its summary says LDL-lowering therapy can be considered for adults at a 10-year PREVENT-ASCVD risk of 3% to under 5%, and should be considered at 5% to under 10%, after discussion between the clinician and patient. Your clinician must interpret any estimate in light of your history and circumstances.

The right decision is not based on one cholesterol number alone. It can depend on estimated short- and longer-term cardiovascular risk; whether you have already had a heart attack or stroke; LDL and non-HDL cholesterol and your risk group; diabetes, family history, other conditions, and medicines; and the expected benefits, side effects, preferences, and feasibility of available options. The guideline also restores LDL-C and non-HDL-C goals that vary by risk, rather than setting one goal for every person.
This framework is for general U.S. adult guidance, not a personalized assessment. The guideline was published in 2026 and its evidence review covers studies through late 2024. Ask a qualified clinician for advice specific to you, especially if pregnancy, childhood, familial hypercholesterolemia, very high LDL, statin intolerance, or advanced kidney or liver disease is relevant; those situations need condition-specific guidance.

When cholesterol medicine is part of the plan
Lifestyle changes may not be enough for some people, and treatment decisions depend on individual risk and characteristics. Statins are commonly used first-line LDL-lowering medicines; they reduce cholesterol production in the liver. Other options include ezetimibe, which reduces cholesterol absorption in the intestines, as well as bempedoic acid and PCSK9 monoclonal antibodies.
