If you ate fish that may have spoiled, watch for symptoms and seek medical care for warning signs—especially trouble breathing, bloody diarrhea, persistent vomiting, or dehydration. Drink fluids if you have vomiting or diarrhea, but do not let hydration delay urgent care. The fish seeming spoiled does not reveal the cause: foodborne infection and fish toxins can produce overlapping symptoms.
Get urgent help for severe or worsening symptoms
Call emergency services or seek emergency care for difficulty breathing, respiratory distress, fainting, severe weakness, or rapidly worsening symptoms after eating fish. Do not wait to see whether breathing symptoms pass.
The Centers for Disease Control and Prevention (CDC) advises medical care for bloody diarrhea; diarrhea lasting more than three days; fever above 102°F; vomiting so frequent that you cannot keep liquids down; or dehydration signs such as little urination, dry mouth or throat, or dizziness when standing. The CDC’s food poisoning signs and symptoms guidance also recommends drinking fluids to help prevent dehydration when vomiting or diarrhea occurs. Fluids do not replace medical care when warning signs are present.
Use a lower threshold to contact a healthcare professional if the person who ate the fish is pregnant, 65 or older, under 5, or immunocompromised; these groups are at greater risk of serious foodborne illness.

Recognize possible symptoms and timing
Common foodborne illness symptoms include diarrhea, stomach pain or cramps, nausea, vomiting, and fever. The CDC says symptoms can begin from a few hours to a few days after eating, depending on the germ. Fish-related illness can also cause allergic-like, neurologic, or cardiovascular symptoms. Timing and symptom patterns may offer context, but they cannot identify the cause on their own.
| Possible cause | Timing reported by public-health sources | Possible symptoms or context |
|---|---|---|
| Foodborne infection | A few hours to a few days, depending on the germ, according to the CDC. | Often diarrhea, stomach pain or cramps, nausea, vomiting, and fever. |
| Histamine fish poisoning (scombroid) | The CDC describes typical onset around 10–60 minutes; its outbreak guidance lists 1 minute to 3 hours, usually about 1 hour. The FDA says symptoms may appear within minutes to a few hours. | Flushing, rash or hives, itching, headache, dizziness, burning in the mouth or throat, gastrointestinal symptoms, palpitations, and sometimes respiratory distress. It can resemble an allergic reaction. Histamine-contaminated fish may look, smell, and taste normal. |
| Ciguatera | Often begins within hours, though the possible range is broader; neurologic symptoms can last longer than ordinary stomach upset. | Associated with certain reef fish, often from tropical or subtropical waters. Symptoms can include nausea, vomiting, diarrhea, tingling or numbness, itching, muscle weakness, altered temperature sensation, dizziness or vertigo, irregular heartbeat, and low blood pressure. |
These ranges come from separate CDC and FDA guidance, not a prediction of when symptoms will begin for any one person. The CDC Yellow Book guidance on marine toxins and the FDA seafood-associated poisoning guidance describe fish toxin symptoms and risks. Cooking does not reliably remove histamine or ciguatera toxin, according to the CDC.

Contact a clinician and share the key details
Call a healthcare professional if symptoms concern you, even if they do not match a specific pattern. Tell them:
- What kind of fish it was, if known, and how much was eaten.
- When it was eaten and when symptoms began.
- How the fish was prepared or stored, if known.
- Whether anyone else who ate it became ill.
- For suspected reef-fish toxin exposure, where the fish was caught or purchased.
The FDA advises consulting a physician and explaining the types and amount of food eaten and when symptoms began. If a clinician or public-health authority asks you to preserve a suspect fish remnant for toxin reporting, the FDA says to freeze it immediately and keep it frozen until it is retrieved. Do not handle or store it in a way that delays getting medical attention.

Why the cause cannot be confirmed at home
“Spoiled” describes how fish seemed; it is not a diagnosis. A foodborne infection, histamine poisoning, and marine toxins can overlap in their gastrointestinal symptoms, while some fish toxin illnesses also cause symptoms outside the digestive system. Appearance, smell, taste, timing, and symptoms alone cannot reliably establish which cause is responsible. A clinician can assess the symptoms and relevant exposure details.
