If you have had close contact with someone diagnosed with pneumonic plague, contact your local or state health department and a healthcare provider promptly. Tell them exactly when and where the contact occurred, your approximate distance and duration, whether the person was coughing, whether you touched respiratory fluids or tissue, and what mask or other protective equipment you wore. Ask whether you need post-exposure prophylaxis (PEP); do not wait for symptoms, use leftover antibiotics, or take someone else’s prescription.

Seek immediate medical attention if you develop fever, headache, weakness, cough, chest pain, shortness of breath, or bloody or watery mucus. Tell medical staff about the possible plague exposure before arrival when possible. Pneumonic plague can progress quickly, and treatment should not be delayed while laboratory results are pending when clinicians suspect the disease.

What to do in the next hour

  1. Call public health. Contact your local or state health department as soon as possible. CDC guidance calls for immediate health-department notification when plague is suspected, and public-health staff can coordinate exposure assessment, prophylaxis, testing, and monitoring.
  2. Call a healthcare provider. Explain that you may have been exposed to pneumonic plague. Ask how you should be evaluated and whether PEP is indicated. If you cannot reach your usual clinician, use the health department’s instructions for an urgent-care or emergency evaluation.
  3. Write down the exposure details. Record the date and time, location, how close you were, how long you were together, whether the patient was coughing, any contact with saliva, sputum, blood or tissue, and the mask, respirator, gloves, eye protection or other PPE you used. Include whether the contact involved an infected animal or a laboratory incident.
  4. Follow the instructions you receive. Public-health authorities may arrange medication, a symptom-watch plan, or an in-person assessment. Do not substitute a thermometer reading, an internet checklist, or advice from another person for that individualized plan.

When you call, say “possible pneumonic-plague exposure” immediately so staff can give you safe instructions for entering a clinic or hospital. If you already have symptoms, do not wait for a routine appointment.

What counts as a potentially significant exposure?

Exposure is assessed from the entire encounter, not from a single distance or time rule. CDC describes known exposure situations that include close, sustained contact within 6 feet of a person or animal with pneumonic plague, and direct contact with infected body fluids or tissues. Whether adequate PPE was worn is central to the assessment.

Encounter factor Why it matters What to report
Close, sustained contact within 6 feet This is the CDC scenario in which PEP should be considered, particularly when adequate PPE was not worn. Approximate distance, duration, room or vehicle setting, and whether the patient was coughing.
Direct contact with respiratory fluids, other body fluids or infected tissue Direct contamination can change the public-health recommendation even if the contact was brief. What material touched you, which body site was involved, whether skin was broken, and what protection you used.
Brief or non-close contact Some contacts may be monitored rather than automatically given PEP, but only public health can classify the encounter. Exact timing, distance and any symptoms that develop.
Adequate standard and droplet precautions CDC’s MMWR recommendation says first responders and healthcare providers who maintained these precautions while caring for a pneumonic-plague patient do not need PEP under that recommendation. Which precautions were used and whether they remained intact throughout care.
Accidental laboratory exposure or close/direct contact with an infected animal CDC says PEP may also be considered in certain laboratory incidents and animal exposures. Specimen or animal involved, work performed, protective equipment and any breach or splash.

The table is a guide to the questions officials will ask, not a self-diagnosis score. A person who was more than 6 feet away may still need assessment after a splash or other direct-fluid contact, while a close contact who used the specified precautions may receive different advice.

Do you need antibiotics after exposure?

For people with known exposure, CDC clinical-care guidance recommends 7 days of post-exposure prophylaxis. A clinician or public-health authority must decide whether you meet the definition of known exposure, select the medicine, check for allergies and interactions, and provide the prescription. The recommended duration appears on CDC’s Clinical Care of Plague page dated May 15, 2024: “Post-exposure prophylaxis should be given for 7 days.”

Do not self-prescribe. Leftover antibiotics may be the wrong drug, the wrong dose, or unsafe with your medical conditions. Stopping early, sharing medication, or taking a drug selected for someone else can leave you unprotected and complicate care. If you are pregnant, have kidney or liver disease, take regular medicines, or have a medication allergy, tell the prescriber before treatment is chosen.

PEP is not a replacement for evaluation. Even after a prescription, follow the monitoring plan and report new symptoms immediately. If you cannot obtain the medication promptly, contact the health department again rather than waiting or improvising.

What one recent investigation shows—and does not show

In a report published October 1, 2026, CDC described a fatal pneumonic-plague case in Coconino County, Arizona, in July 2025. Investigators identified 46 potentially exposed people two days after the patient’s death, and all began oral ciprofloxacin PEP. That was the medication chosen for that investigated event; it is not a universal antibiotic instruction for every exposure. Your regimen, if any, depends on the exposure assessment and your health history.

How to monitor yourself

Follow the schedule and reporting method supplied by public health. Lower-risk contacts may be asked to watch for fever or cough and notify their healthcare provider and state or county health department if symptoms occur. A digital thermometer can make temperature checks easier when monitoring is advised, but a normal reading cannot rule out plague and cannot determine whether PEP is needed.

After inhalation, the incubation period for pneumonic plague can be as short as one day. Do not use a symptom-free interval found online as proof that an encounter was harmless. Keep the health department’s phone number available, note the time of each check if officials request it, and report any change rather than waiting for several symptoms to appear.

Symptoms that require urgent attention

  • Fever
  • Headache or marked weakness
  • Cough
  • Shortness of breath
  • Chest pain
  • Bloody or watery mucus

CDC describes pneumonic plague as a rapidly developing pneumonia. Its symptoms page, dated May 15, 2024, states: “If you are experiencing symptoms like those listed here, seek immediate medical attention.” In an emergency, call emergency services. Tell dispatchers and responders about the possible exposure, and follow their directions about where and how to be evaluated.

What happens if symptoms begin?

  1. Seek care immediately. Do not wait for a scheduled public-health call, a home test, or a laboratory result.
  2. Disclose the exposure before arrival when possible. A clinic or emergency department can give you instructions that protect other patients and staff and prepare the appropriate evaluation.
  3. Bring your exposure information. Provide the timeline, distance, duration, symptoms, PPE details and any PEP already taken.
  4. Take treatment instructions seriously. CDC says plague treatment should not be delayed for laboratory results when clinicians suspect the disease. Clinicians decide which treatment and tests are appropriate; do not wait for confirmation before seeking care.

Symptoms after exposure do not prove that you have plague, but they make prompt professional assessment essential. Conversely, having no symptoms does not cancel a PEP recommendation made for a known exposure.

How public health will assess your encounter

Officials generally reconstruct the event: who was ill, whether pneumonic plague was confirmed or suspected, when the contact occurred, how close and prolonged it was, whether coughing or aerosol-generating care was involved, and whether fluids or tissue were touched. They also review respirators or masks, eye protection, gloves, gowns and any breach in protection. The same person may receive different advice for different encounters.

Tell officials about every potentially exposed person you know, including household members, coworkers, passengers, caregivers and people who handled an infected animal or specimen. Do not delay your own call while trying to identify everyone. Public-health investigators can determine who needs notification and what precautions apply.

Special situations

Healthcare workers and first responders

If you cared for a pneumonic-plague patient while maintaining standard and droplet precautions, the 2021 CDC MMWR recommendation says PEP is not needed under that specific circumstance. This is not a blanket exemption for every worker or every lapse in protection. Report the encounter, confirm which precautions were maintained, and let occupational health or public health make the determination.

Animal contact

Close or direct contact with an infected animal can qualify for consideration of PEP. Provide the animal’s species, the nature of the contact, any bite or scratch, and whether respiratory secretions or tissue were involved. Do not assume that an animal exposure follows the same pathway as household contact with a person.

Laboratory incidents

Accidental laboratory exposures require prompt occupational-health and public-health notification. Describe the organism or specimen, the procedure, the containment level, any splash, sharps injury, aerosol or equipment failure, and the PPE worn. PEP decisions are made from those details.

Common mistakes to avoid

  • Waiting for symptoms. The incubation period can be as short as one day, and PEP is intended for known exposure before illness develops.
  • Assuming “I was nearby” answers the question. Distance, duration, coughing, fluids and PPE all matter.
  • Taking someone else’s antibiotics. Medication and dosing must be selected for you by a clinician or public-health authority.
  • Treating a thermometer as a clearance test. Temperature checks support monitoring but do not diagnose or exclude plague.
  • Assuming every close contact needs the same drug. The Arizona investigation involved oral ciprofloxacin, but that event does not establish a universal regimen.
  • Failing to mention the exposure at urgent care. Staff need the information before evaluation so they can direct you safely.

Which guidance should you follow?

The CDC MMWR treatment and prophylaxis recommendations cited here date to 2021. CDC’s Clinical Care of Plague page was published May 15, 2024, and its diagnostic information was updated in April 2026. Local or state instructions may be newer or more specific to your jurisdiction, so follow those instructions if they differ. Public-health authorities can also account for details not covered by a general web page, including the confirmed status of the patient, the type of contact and current local procedures.

A practical call checklist

Have these details ready when you contact public health or a clinician:

  • Your name, location and a callback number
  • The patient’s diagnosis, if known, and your relationship to that person
  • Date, time, place, distance and duration of contact
  • Whether the person was coughing or producing respiratory secretions
  • Any direct contact with saliva, sputum, blood, other fluids or tissue
  • Mask, respirator and other PPE used, including any failure or removal
  • Current symptoms, temperature readings if monitoring was advised, allergies, pregnancy status and regular medicines
  • Any prior PEP dose or other antibiotic already taken

Prompt reporting gives officials the information needed to decide between PEP, monitoring, testing and urgent evaluation. If symptoms appear, treat the situation as urgent even if your original contact seemed brief or you were initially told only to monitor.
