If you have COVID-19 symptoms, stay home, rest, drink fluids, and test. A negative rapid antigen test does not rule out infection: if you have symptoms, repeat it 48 hours later. If you may be at higher risk of severe illness, contact a healthcare provider promptly to ask about treatment. Get emergency care for trouble breathing, persistent chest pain or pressure, new confusion, or other severe symptoms.
What to do first if you feel sick
- Stay home and limit close contact. Keep away from other people, including household members who are not sick, when possible. Local rules vary, so check your public health guidance. If you must be around others, wearing a well-fitting mask and improving ventilation can reduce exposure.
- Rest and drink fluids. Most people with mild COVID-19 can recover at home. For discomfort, CDC and NHS list acetaminophen (called paracetamol in the UK) or ibuprofen as options. Follow the product label and ask a pharmacist or clinician if you are unsure whether a medicine is suitable for you. NHS says honey may help a cough, but do not give honey to a baby under 12 months.
- Watch how symptoms change. COVID-19 can be mild or severe, and symptoms can change over the course of an illness. Symptoms overlap with flu, so you cannot reliably distinguish the two by symptoms alone.
When and how to test
If you have symptoms, take an at-home COVID-19 antigen test as soon as possible and follow the instructions for that specific test. In the United States, FDA-authorized over-the-counter tests can be used without a prescription. Check the FDA’s test list for current instructions and expiration information before relying on a product.
If your antigen test is negative
A single negative rapid test is not a definitive rule-out, particularly early in an infection. FDA advises people with symptoms to take at least two tests in total, 48 hours apart. If the first test is negative, take the second 48 hours later. For people without symptoms, FDA advises at least three tests over five days, 48 hours apart. See the FDA guidance on negative results and follow the package directions. If symptoms persist or you remain concerned after negative tests, contact a healthcare provider; molecular testing may be considered.

A positive at-home antigen result means you likely have COVID-19. Follow local public health guidance and seek follow-up care if needed. If you may be at higher risk of severe illness, contact a healthcare provider promptly about treatment, even if symptoms are mild.

When to call a healthcare provider about treatment
Do not wait for symptoms to become severe if you may be at higher risk. Risk depends on personal circumstances; CDC highlights older adults, especially those 75 and older, people who are not up to date on vaccination, and people with certain conditions such as chronic lung or heart disease or weakened immunity. WHO also identifies age 60 and over and conditions including chronic heart, lung, liver, or rheumatological disease, HIV, diabetes, cancer, dementia, obesity, and low immune function. If you are unsure whether you qualify, ask a clinician.
For eligible higher-risk people with mild to moderate illness, antiviral treatment can reduce the risk of hospitalization and death. It must be started within a short window after symptoms begin: depending on the medicine, within five to seven days. CDC lists oral nirmatrelvir with ritonavir (Paxlovid) for eligible adults and children 12 and older within five days, IV remdesivir for adults and children within seven days, and molnupiravir for adults within five days. A healthcare professional must determine eligibility and the appropriate option; drug interactions and side effects need to be considered with a provider or pharmacist. A positive test is not required to start discussing treatment. See CDC outpatient treatment guidance.

When to seek emergency care
In the United States, call 911 or go to emergency care for trouble breathing, persistent chest pain or pressure, new confusion, inability to wake or stay awake, or pale, gray, or blue skin, lips, or nail beds. Tell the dispatcher or facility that COVID-19 is possible. CDC says its list is not exhaustive: seek immediate help for any severe or concerning symptom.

Other warning signs can also require urgent assessment. CDC’s broader respiratory illness guidance includes persistent dizziness, seizures, not urinating, severe muscle pain or weakness, unsteadiness, or a fever or cough that improves and then returns or worsens. In children, warning signs include fast or difficult breathing, bluish lips or face, ribs pulling in with each breath, chest pain, dehydration, lack of alertness, seizures, and specified high fevers. Do not wait for multiple warning signs to appear.
