If you are losing weight, appetite, or strength during cancer, ask your oncology team to assess what is driving the change and help you set a plan. Cancer cachexia can involve loss of appetite, body weight, and skeletal muscle; it is more complex than simply not eating enough, so nutrition advice alone may not address every cause.
Bring the questions below to your appointment. They are prompts for a conversation, not a diagnosis or a treatment plan to start on your own.
Questions to ask at your appointment
- What might be causing my weight or muscle loss, and how are you assessing it? Ask whether your team will track weight, appetite, strength, or daily function, and whether other health or treatment-related factors need attention.
- Could symptoms be making it harder to eat? Mention nausea, pain, mouth sores, trouble swallowing, feeling full quickly, constipation, or changes in mood. Ask which symptoms could be contributing and what can be done about them.
- Can you refer me to an oncology dietitian? Ask for advice tailored to your cancer, treatment, symptoms, food preferences, and nutrition needs.
- Would a medication make sense for me? If your oncologist suggests olanzapine or another medicine, ask what outcome it is meant to improve, what benefit is realistic, what risks or interactions matter, and when you would stop or reassess it.
- Would a nutrition drink help, and which one fits my needs? Ask whether a supplement drink is appropriate for your symptoms and dietary needs. A dietitian can help you choose; a drink is individualized nutrition support, not a proven cure for cachexia.
- Could physical therapy or rehabilitation help? Ask what activity is safe for you and whether a therapist could help with strength, endurance, or everyday tasks.
- How will we know whether the plan is helping, and when will we review it? Agree on outcomes that matter to you, such as appetite, weight trend, strength, daily activities, treatment tolerance, or quality of life.
- Is a clinical trial appropriate for my cancer, treatment stage, and goals? Ask whether there are cachexia studies worth considering.
- What changes should prompt me to contact the team sooner? Get specific instructions for your situation rather than waiting for the next scheduled visit.
What current guidance says about treatment
Start by assessing nutrition and symptoms
The National Cancer Institute recommends regular nutrition screening during treatment and discussion with a doctor about malnutrition and cachexia risk. Depending on individual needs, the care team may involve a registered dietitian, physical therapist, speech therapist, or dentist. A dietitian can offer practical advice, including ways to add protein and calories when appropriate. The American Society of Clinical Oncology (ASCO) advises against fad and extreme diets.

Medication is an individualized discussion
ASCO’s 2020 adult guideline says evidence was insufficient to strongly endorse a drug as standard care for cancer cachexia; clinicians may choose not to prescribe a cachexia-specific medication. When a medication trial is considered for appetite or weight, that guideline discussed progesterone analogs or short-term corticosteroids.

ASCO’s 2023 rapid update says clinicians may offer low-dose olanzapine once daily to adults with advanced cancer to improve appetite and weight gain. Most of the evidence involved people with lung or gastrointestinal cancers, and the largest study enrolled people receiving cytotoxic chemotherapy. This is a clinician-led option, not a recommendation for everyone. Appetite or weight gain should not be assumed to mean muscle, strength, function, or survival has improved. ASCO states that no medication is FDA-approved to treat cancer cachexia in the guidance cited here.
These recommendations address different questions: the 2020 guideline covered cachexia care broadly, while the 2023 update added a specific option for appetite and weight gain in adults with advanced cancer. Neither establishes one treatment as right for every patient.

Nutrition support is not the same as a cure
Dietitian counseling can help tailor food choices and manage eating difficulties, but cachexia can involve disease-related and metabolic changes as well as reduced intake. A nutrition drink may suit some people; it is not established as a treatment that reverses cachexia for everyone.

ASCO does not recommend routine feeding tubes or IV nutrition solely to manage cachexia in advanced cancer outside a clinical trial. A short-term IV nutrition trial may be considered in selected cases, such as reversible obstruction, short bowel, or malabsorption in someone otherwise reasonably fit. That decision requires clinical judgment and is not a routine approach.

Rehabilitation may target function
Rehabilitation may help support strength, endurance, and daily activities. ASCO’s 2020 guideline did not make a cachexia-treatment recommendation for exercise because evidence was insufficient. Ask your team whether physical therapy or another form of supervised activity fits your health and treatment.

There is no universal ranking of cachexia treatments in the cited guidance. Before trying an intervention, ask what it is intended to improve and how the team will decide whether it is worth continuing.
