Immunotherapy for bowel (colorectal) cancer can cause rash, diarrhoea and fatigue, but the more important risk is immune-related inflammation in healthy organs. The bowel, lungs, liver, kidneys, heart, pancreas, hormone-producing glands and nervous system can all be affected. Symptoms may begin during treatment or months after the last dose.
Contact your oncology team or its urgent advice line promptly for a new change in bowel habit, abdominal pain, blood or mucus in stool, unusually high stoma output, or difficulty replacing fluids. Cancer Research UK gives “2 or more loose bowel movements than usual in 24 hours” as an example trigger; your own treatment card and oncology team’s instructions take priority. Severe or rapidly worsening symptoms require the emergency route supplied by your treatment centre or local emergency service.
What immunotherapy side effects are possible?
Checkpoint inhibitors work by releasing brakes on the immune system so it can attack cancer. That immune activation can also irritate normal tissues. The exact pattern depends on the drug or combination, dose, previous treatment, other illnesses and the individual patient. A symptom is not proof that immunotherapy is the cause: infection, another medicine, the cancer itself or an unrelated condition can look similar. It still needs assessment.

Not every person receiving a checkpoint inhibitor develops these problems, and the list is not a prediction for a particular medicine. Keep the exact drug name available because the risk profile and monitoring plan differ between regimens.

Bowel symptoms: why diarrhoea is not always a minor upset
Diarrhoea during or after checkpoint-inhibitor treatment can indicate colitis, an immune attack on the bowel lining. It may start as a modest increase over your usual pattern and then become frequent, painful or dehydrating. A person with a stoma should compare output with their own normal amount and consistency rather than relying on a generic stool count.
Report a change from baseline, especially when it is accompanied by cramps or abdominal pain, urgency, mucus, blood, black stool, fever, dizziness or reduced urine. Increased stoma output can cause fluid and salt losses even when the output does not look like conventional diarrhoea. Tell the team if you cannot drink enough to replace what you are losing.
How many loose bowel movements are too many?
Cancer Research UK’s patient guidance uses “2 or more loose bowel movements than usual in 24 hours” as a reason to contact the cancer team. This is an example threshold, not a universal rule for every drug, patient or hospital. Your written treatment instructions may tell you to call after one change, use a different grading system or contact a 24-hour advice line.

Count changes against your personal baseline, record when they began and note whether they are accelerating. Do not wait for a routine appointment to mention new diarrhoea. Early immune-related symptoms can progress, and prompt assessment lets clinicians distinguish colitis from infection, diet-related illness, antibiotics, laxatives or another cause.
When should you call your care team?
Call promptly through the oncology advice line
Use the number on your treatment card, clinic letter or hospital portal for any new bowel-pattern change that meets your team’s threshold, abdominal pain, blood or mucus in stool, black stool, higher-than-usual stoma output or difficulty keeping up with fluids. Call even if the symptom seems mild and you otherwise feel well. The oncology service may ask questions, arrange same-day tests or tell you to attend an assessment unit.
Seek urgent advice for symptoms outside the bowel
Contact the team urgently for a new cough or breathlessness; chest symptoms; yellowing of the skin or eyes; dark urine; marked or rapidly increasing weakness; new numbness or tingling; confusion; a severe headache; or any symptom that is worsening. These signs can have causes unrelated to immunotherapy, but delaying assessment can be dangerous.

Use emergency services for severe or rapidly worsening illness
Follow the emergency plan supplied by your treating centre or local emergency service for severe breathlessness, collapse, fainting, severe abdominal pain, heavy bleeding, confusion or inability to stay awake, signs of severe dehydration, or a symptom that is progressing rapidly. Do not wait for the next clinic session and do not rely on a general article to choose a local emergency number.

National Cancer Institute guidance notes that an oncologist may direct you to call the cancer team first or go straight to emergency care. The correct route depends on your location, treatment centre and severity.
Can side effects start after treatment ends?
Yes. Immune-related effects are not limited to the infusion day. Cancer Research UK notes that checkpoint-inhibitor diarrhoea can appear months or even years later. Other inflammatory reactions can also be delayed. Tell every clinician who assesses a new symptom that you have received immunotherapy, even if the final dose was a long time ago.

Keep the name of the medicine, dates of treatment and oncology contact details in your phone or with your medicines. A delayed symptom should not automatically be attributed to immunotherapy, but the treatment history changes which tests and specialist advice may be appropriate.

- The number and character of bowel movements compared with your normal pattern.
- Whether there is abdominal pain, fever, mucus, red blood or black stool.
- Any change in stoma output, including frequency, volume or watery consistency.
- How much you have been able to drink, whether you are urinating normally and any dizziness.
- Other new symptoms, such as cough, breathlessness, jaundice, weakness, numbness, confusion or severe headache.
- All medicines taken recently, including antibiotics, laxatives, anti-diarrhoeal products, steroids and non-prescription remedies.
NCI patient information recommends keeping the drug name and the doctor or hospital contact information handy. If someone calls for you, have them note the advice, where to go and when to call back.

What will the clinical assessment involve?
The team may ask about your baseline bowel pattern, examine you, check hydration and order blood tests. Depending on the symptom, they may test stool for infection, perform scans, check oxygen levels or arrange specialist review. A symptom that looks like colitis can instead be infectious diarrhoea, and treating the wrong cause can be harmful.
If clinicians determine that an immune reaction is responsible, they may pause immunotherapy and prescribe treatment appropriate to the organ and severity. The National Cancer Institute explains that some immune side effects are managed with medicines such as steroids that slow an overactive immune response. Do not start, stop or change steroids, anti-diarrhoeal medicine or cancer treatment unless your oncology team tells you to do so.
Which bowel cancer treatments can cause these effects?
Immunotherapy is not given to everyone with bowel cancer. National Cancer Institute information lists pembrolizumab, nivolumab and ipilimumab among immunotherapy drugs used for colon cancer. Nivolumab with ipilimumab is used for some advanced colorectal cancers with high microsatellite instability (MSI-H) or deficient mismatch repair (dMMR). Eligibility depends on tumour testing, stage, previous treatment, overall health and specialist assessment.

Do not use side-effect information from a different regimen as a personal prediction, and do not assume that a symptom is harmless because someone else on immunotherapy experienced it differently.
Practical monitoring between doses
A simple record can make a telephone assessment more accurate. Note your usual bowel frequency before treatment, each day’s change, stoma output when relevant, fluid intake, temperature if the team has asked you to check it and any new symptom outside the bowel. Bring the record to appointments or read it to the advice nurse.
Keep your treatment card accessible and make sure household members know where the urgent contact number is. If you change address, travel or move between hospitals, ask how to reach the oncology service outside office hours. Contact arrangements and emergency numbers vary by country and treatment centre.
