Mental-health prediction, coaching and therapy apps can collect far more than the mood score you enter. Depending on the service and the permissions you grant, records may include your identity and contact details, IP address and device identifiers, screening answers, symptom and medication information, journal or chat text, usage patterns, and data imported from wearables. Those inputs can reveal sensitive facts—such as that you sought counseling or may have a particular symptom pattern—even when the app never displays a diagnosis.
Before signing up, read the app’s privacy policy, restrict permissions, disable advertising and cross-app tracking where possible, secure the account, and learn how export and deletion work. HIPAA may not cover a direct-to-consumer app; privacy promises and unauthorized sharing can still fall under Federal Trade Commission (FTC) rules, including the Health Breach Notification Rule.
What a mental-health app can collect
There is no single standard data set. A journaling app, a symptom tracker, an AI coach and a service connected to a clinician may collect different information. The privacy policy and your device’s permission screen are the authoritative sources for a particular app.
Recommended Free Tools
| Data category | Examples | Why it matters |
|---|---|---|
| Identity and account | Name, email address, phone number, age or birth date, login credentials and payment details | These fields can identify you directly and link activity to a billing record. |
| Device and network | IP address, device and advertising identifiers, browser or in-app events, approximate location and crash logs | Identifiers can connect sessions, devices and advertising profiles. |
| Mental-health content | Screening answers, mood or symptom scores, conditions or diagnoses you enter, medication and treatment details, therapy goals, journal entries and chat transcripts | Free text and scores may disclose health information more precisely than a profile label. |
| Context and behavior | Session frequency, clicks, feature use, contacts or calendar data when permitted, and information imported from wearables, fitness trackers or other services | Patterns can show routines, sleep or activity changes, relationships and treatment engagement. |
An app may receive information from you, your phone, an advertising or analytics software development kit, a connected health service, or a business that processes payments or hosts data. A permission to read contacts, a microphone, photos, precise location or a health platform is not automatically necessary for a mood or coaching feature; treat every request as a separate decision.

Questionnaires and free text
Screening questions can ask about anxiety, depression, trauma, substance use, suicidal thoughts, medication or prior treatment. A journal or chat transcript can contain names, workplaces, relationship details and events that you did not intend to share outside the service. Text entered for an AI feature may be retained for safety review, support, service improvement or model-related processing if the policy allows it.
Imported and inferred information
Wearable or fitness integrations can add sleep, heart-rate, activity and time-of-day data. Even if an app does not call any entry a diagnosis, it or its partners may infer that you are seeking counseling, have a symptom pattern, take medication, may disengage from treatment or fit an advertising audience. An inference can be sensitive even when every underlying field looks ordinary.
How ordinary identifiers become health information
The FTC alleged that BetterHelp shared email addresses, IP addresses and health-questionnaire information with Facebook, Snapchat, Criteo and Pinterest for advertising. The agency said the information revealed that people had sought counseling. In 2024, the FTC’s complaint against Cerebral alleged that tracking integrations sent sensitive information about nearly 3.2 million consumers to LinkedIn, Snapchat and TikTok.

These cases illustrate the risk of combining an identifier with an app event. An email address, IP address or advertising ID may not look medical by itself. Attached to a visit to a counseling page, a depression questionnaire or a medication feature, it can identify a person as someone seeking mental-health care.
Why hashing and “de-identification” are not guarantees
Hashing converts a value into a fixed-looking string, but it does not necessarily make the person unknowable. The FTC has explained that hashed identifiers can still be matched or used to target people. Its 2024 discussion of hashing said the technique did not stop Facebook from learning who was seeking mental-health counseling. Ask what re-identification controls exist, who receives the value and whether it is used for advertising or profiling; do not treat the word “hashed” as proof that sharing is harmless.
Can an app sell or share your information?
“Sell” can have a narrow legal definition, while a policy may describe sharing, disclosure, advertising, analytics, service providers or business transfers separately. Read the policy’s definitions and purposes rather than relying on a short marketing statement.

Common recipients
- Advertising and measurement companies that receive identifiers or event data.
- Analytics, crash-reporting and software providers that process app activity.
- Cloud-hosting, customer-support and payment vendors.
- AI or machine-learning processors, if the policy permits sending prompts, journals or transcripts for that purpose.
- Corporate affiliates, investors or a buyer in a merger or asset sale.
- Clinicians or other parties you deliberately connect to the account.
A policy should identify recipients by name or describe them clearly, explain the purpose of each disclosure, and state whether data is used for targeted advertising or model training. If it does not, uncertainty itself is a reason to minimize what you enter.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Does HIPAA protect mental-health app data?
Usually, not automatically. HIPAA generally applies to health plans, most health-care providers and health-care clearinghouses, plus their business associates. A direct-to-consumer app that you download and use independently may not be one of those entities. If a provider offers an app as part of care, the provider’s relationship and the app’s contractual role may change the analysis; ask the provider whether the app is acting as a HIPAA business associate.
Protections that can still apply outside HIPAA
The FTC Act prohibits deceptive privacy promises and unfair practices. The FTC’s Health Breach Notification Rule covers many non-HIPAA health apps that maintain personal health records or draw identifiable health information from multiple sources. In April 2024, amendments clarified that a breach can include an unauthorized disclosure, not only an outside hack. Depending on the incident, notice may be required to affected people, the FTC and sometimes the media.

Think of two separate questions:
- Is the service a HIPAA covered entity or business associate for this use?
- What does its privacy policy promise about collection, sharing, advertising, retention, deletion and model training?
A “HIPAA compliant” label, by itself, does not answer the second question. Keep a copy of the policy and the consent choices you accepted; policies can change.
What enforcement actions show about privacy promises
BetterHelp
In 2023, the FTC announced a proposed order involving BetterHelp that included $7.8 million for consumers after allegations that email addresses, IP addresses and questionnaire information had been disclosed for advertising. The agency also alleged that the service made privacy representations that did not match its practices. The case demonstrates that a company can face consequences for sharing health-related information even when the disclosure occurs through advertising technology rather than a conventional data breach.
Cerebral
The FTC’s 2024 complaint concerning Cerebral alleged that tracking tools provided sensitive information about nearly 3.2 million consumers to LinkedIn, Snapchat and TikTok. A complaint is an allegation, not a final finding, but it shows why a policy’s analytics and advertising sections deserve the same attention as its security section.

Neither matter establishes what every mental-health app does. No authoritative market-wide percentage establishes how many such apps share data or train models. Evaluate each service on its own policy, permissions and controls.

- Open the full policy. Search within it for “data collected,” “sources,” “purposes,” “sharing,” “advertising,” “analytics,” “artificial intelligence,” “machine learning,” “retention,” “deletion,” “export,” “sale” and “model training.” Note whether free-text journals, audio, transcripts and inferred data are included.
- Write down every requested permission. For each one, ask which feature needs it. Deny contacts, microphone, precise location, photos or health integrations unless the feature genuinely depends on them. You can often change the choice later in the phone’s Settings app.
- Identify third parties. Look for advertising, analytics, cloud-hosting and AI-processing recipients. A generic phrase such as “trusted partners” gives you less practical visibility than a named vendor list or a precise category-and-purpose explanation.
- Check user controls. Confirm whether you can opt out of targeted advertising, export records, correct information, withdraw consent and delete the account. Ask whether deletion removes backups, support tickets, connected-service copies and data already used in model training.
- Secure the account. Use a unique password and multifactor authentication, update the app and operating system, and review active sessions. Do not reuse a password from email or another health service.
- Minimize what you enter. If a display name works, do not provide a full legal name. Avoid unnecessary employer, school, address or identifying relationship details. Do not invent clinical information that could make an emergency or treatment feature unsafe; instead, choose a service whose data practices you accept.
FTC mobile-health guidance recommends minimizing data, limiting access and permissions, using authentication, building security into the product, encrypting health information and deleting it when there is no legitimate business need to retain it.
How to compare two mental-health apps
Do not use an app-store rating as a privacy assessment. A 2024 CHI study found a disconnect between users’ privacy concerns and the criteria used by prominent app-evaluation resources. Compare the same questions for every candidate:
| Comparison axis | Questions to ask |
|---|---|
| Data minimization | What is required to use the core feature, and what is optional? |
| Categories and sources | Does the app collect free text, audio, location, contacts, wearables or data from other services? |
| Third-party sharing | Are advertisers, analytics firms, AI processors and affiliates named or clearly described? |
| Advertising and model use | Is health-related activity used for targeted advertising, profiling, personalization or model training? |
| Retention and deletion | How long are records kept, and does account deletion cover backups, support records and derived data? |
| Permission controls | Can you deny or later revoke each permission without losing unrelated features? |
| Security | Does the policy describe encryption, access controls, authentication and incident response? |
| Legal coverage | Is the service a HIPAA covered entity or business associate, and does the FTC Health Breach Notification Rule potentially apply? |
| History | Has a regulator alleged deceptive sharing, unauthorized disclosure or misleading privacy claims? |
| Clarity | Can an ordinary reader understand the policy’s purposes, recipients and controls? |
Prefer the app that needs less information for the feature you actually want and gives you clearer, reversible choices. If two apps perform similarly, a service with fewer third-party disclosures and a shorter retention period creates less exposure.
How to delete or export your data
Use the in-app controls first
- Open the account or profile menu and look for Privacy, Data, Account or Security.
- Export a copy if you may need mood history, treatment notes or records for a clinician. Check the file format and whether attachments or chat transcripts are included.
- Disconnect wearables, health platforms, calendars and other integrations before deletion so the app stops receiving new data.
- Choose account deletion rather than merely uninstalling the app. Uninstalling removes the local program; it does not necessarily remove server records.
- Save the confirmation screen or email and record the request date. Read the policy for the stated deletion period and exceptions for legal, fraud-prevention, billing or safety records.
If the control is missing or incomplete
Send a written request to the privacy or support address listed in the policy. Identify the account without adding new sensitive details, specify whether you want access, correction, export, deletion or consent withdrawal, and ask what happens to backups, vendors and model-training copies. Keep the response. If the company does not honor a clear promise, you can report the issue to the appropriate privacy regulator; in the United States, that may include the FTC or a state attorney general, depending on the facts.
What to do after a suspected disclosure
- Change the app password and any reused password immediately; enable multifactor authentication.
- Revoke unnecessary permissions and disconnect integrations.
- Review account activity, connected devices and payment records for signs of misuse.
- Save policy versions, notices, screenshots and relevant messages.
- Read any breach notice carefully. Under the FTC Health Breach Notification Rule, an unauthorized disclosure can qualify even without an external hack.
- If the exposed information could cause immediate personal danger, contact a trusted person or appropriate emergency service; do not rely on an app’s privacy team for urgent safety help.
Questions worth asking the provider
- Which fields are mandatory, and which can I leave blank?
- Do you use journal entries, chats, voice recordings or screening answers to train or evaluate an AI system?
- Which advertising, analytics, hosting and AI companies receive data, and what identifiers do they get?
- How long are raw entries, derived predictions and backups retained?
- Does deleting my account remove data held by processors and data already included in a model or aggregate report?
- Is this service a HIPAA covered entity or business associate for my use?
- How will you notify me about an unauthorized disclosure, and where can I obtain a copy of my records?
Bottom line
Treat a mental-health app as a holder of highly sensitive information, not merely a wellness gadget. The safest approach is to minimize inputs, deny unrelated permissions, disable advertising and cross-app tracking, use strong account security, inspect third-party and AI disclosures, and verify how export and deletion work. HIPAA may not apply, but misleading promises and unauthorized health-data disclosures can still trigger FTC protections and enforcement. Choose only after you understand what the app collects, what it can infer, who receives it and how you can get it back or remove it.
