TMAO (trimethylamine N-oxide) is a small molecule produced through interactions between dietary compounds, gut microbes and the body. A human study found higher TMAO in the cerebrospinal fluid of people with mild cognitive impairment (MCI) and Alzheimer’s dementia than in cognitively unimpaired participants. That finding links TMAO to Alzheimer’s-related biomarkers, but it does not show that TMAO causes the disease or that lowering it prevents or treats Alzheimer’s.
What is TMAO?
Trimethylamine N-oxide, usually shortened to TMAO, is a metabolite associated with the way the body processes dietary choline and other precursor compounds. Gut microbes can transform some of these compounds into trimethylamine; the body then converts trimethylamine to TMAO. Its production therefore involves both microbial and host metabolism—it is not simply a toxin made by one food.
TMAO has been studied in several disease contexts, but finding it in the body, or measuring a particular level, does not diagnose Alzheimer’s disease.
What did the human Alzheimer’s study find?
A 2018 study measured TMAO in cerebrospinal fluid (CSF), the fluid surrounding the brain and spinal cord, from 410 participants: 40 with an Alzheimer’s clinical syndrome, 35 with MCI and 335 who were cognitively unimpaired. After adjusting for age, sex and APOE ε4 genotype, the researchers found higher CSF TMAO in the MCI and Alzheimer’s dementia groups than in the cognitively unimpaired group. Read the study record.
Higher CSF TMAO was also associated with Alzheimer’s-related biomarkers, including phosphorylated tau and the phosphorylated tau/Aβ42 ratio, as well as total tau and neurofilament light, a marker associated with neuronal injury. These are measured relationships within a cohort, not evidence that TMAO started the disease or drove its progression.
Does TMAO cause Alzheimer’s disease?
The study does not establish cause and effect. Because it measured TMAO and biomarkers in participants rather than testing whether changing TMAO changes disease, it cannot show which came first. TMAO could contribute to disease processes, arise as a consequence of them, reflect another underlying process, or play more than one role.

A 2021 review characterized the TMAO–Alzheimer’s link as “far from definitive.” That caution is important: a biomarker association can identify a question for further study, but it is not proof of a cause. See the review.

| Evidence type | What it can show | What it cannot establish by itself |
|---|---|---|
| Human CSF biomarker study | Whether TMAO levels are associated with Alzheimer’s-related clinical groups and biomarkers in the people studied. | Whether TMAO causes disease or whether lowering it improves clinical outcomes. |
| Mechanistic and animal research | Whether proposed pathways or Alzheimer’s-like effects merit investigation. | Whether the same effects occur in people or whether a human intervention works. |
| Reviews | How researchers synthesize findings and identify unresolved questions. | A substitute for direct evidence that an intervention prevents or treats Alzheimer’s. |
Researchers have proposed possible connections involving gut–brain communication, inflammation, blood-vessel or endothelial function, amyloid processing, tau-related pathology and neuronal injury. Reviews discuss these as biologically plausible avenues, while animal studies investigate possible effects on Alzheimer’s-like features. Plausibility and animal findings justify further research; they do not demonstrate a human cause or treatment. A 2025 review reports synthesizing 92 peer-reviewed animal, human and meta-analytic studies published from 2010 through 2025, but a broad evidence review still does not turn associations into intervention evidence. Read the 2025 review. A 2024 review also surveys proposed mechanisms and findings. See the 2024 review.

The evidence described here does not show that lowering TMAO—through diet, probiotics, supplements or another approach—prevents Alzheimer’s, treats it or improves cognition. It also does not establish a TMAO-specific prevention or treatment plan. Reviews call for large, longitudinal studies and research that can better test mechanisms before microbiome-directed interventions can be validated.
