Software as a Medical Device (SaMD) is software intended to perform a medical purpose on its own, without being part of a hardware medical device. It can run on a phone, workstation, browser, server, or virtual machine. A symptom checker, image-analysis application, or algorithm that interprets physiological data may therefore be a medical device even when no dedicated medical hardware is included.

Whether a product qualifies depends mainly on its intended purpose: the claims made about the software, its users, the clinical situation, and the decision or action supported by its output. Product teams must settle that scope before development, then match risk controls, quality processes, cybersecurity, and clinical evidence to the intended use and the market in which the software will be offered.

SaMD in plain language

The International Medical Device Regulators Forum (IMDRF) defines SaMD as “software intended to be used for one or more medical purposes that perform these purposes without being part of a hardware medical device.” The U.S. Food and Drug Administration (FDA) reproduces this definition and treats SaMD as software that is itself a medical device.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The key distinction is not where the code runs. SaMD may run on a general-purpose computer, smartphone, tablet, cloud service, virtual environment, or dedicated server. The distinction is what the software is intended to do. If it independently performs a medical purpose, its regulatory status follows that purpose even when the underlying computing platform is ordinary consumer technology.

Examples of medical purposes include detecting, diagnosing, screening for, monitoring, or predicting a disease; supporting treatment decisions; or providing information used to prevent or manage a clinical condition. A program that merely stores a record is different from one that interprets the record and recommends a diagnosis or treatment.

What “standalone” means

“Standalone” does not mean the product must work without any data source or network. A cloud algorithm can receive data from a sensor, analyze it, and return a clinical result while remaining SaMD. The phrase means the medical function is not merely a component that makes a separate hardware medical device operate.

What SaMD is not

FDA distinguishes SaMD from software that is integral to a hardware medical device and from software used to manufacture or maintain a device. Firmware that controls an infusion pump, for example, is part of the hardware device rather than standalone SaMD. Production-line software that calibrates a device is also a different category. The applicable legal treatment depends on the complete product and the jurisdiction.

Does your app qualify as SaMD?

Use the product’s intended purpose, not its technical sophistication, as the starting point. The same machine-learning model could be regulated differently when marketed as a general wellness feature, as an administrative tool, or as a diagnostic aid. Claims in labeling, onboarding, sales material, app-store text, user instructions, and customer training can all help establish intended purpose.

Ask these five scope questions

  1. What medical claim is made? Does the product claim to detect a condition, calculate a clinical measurement, predict risk, guide treatment, or otherwise serve a medical purpose?
  2. Who uses the output? A licensed clinician, a patient, a caregiver, a laboratory, and an operations employee may face different risks and regulatory expectations.
  3. What does the output actually contain? A raw display or data transfer is different from an interpreted score, alert, diagnosis, or recommendation.
  4. What decision or action does it support? Identify whether the result informs triage, diagnosis, therapy selection, monitoring, discharge, referral, or another consequential action.
  5. In what healthcare situation is it used? Emergency care, critical illness, routine diagnosis, chronic-disease management, and population screening do not carry the same potential for harm.

Functions commonly outside device scope or subject to policy discretion

Administrative support, healthy-lifestyle functions, some electronic-record functions, and software limited to transferring, storing, or displaying data without interpretation may fall outside FDA device regulation. Certain clinical-decision-support functions may also be treated under enforcement discretion or another policy, depending on their exact operation and the information provided to the user. These are not blanket exemptions: a change in claims, audience, or algorithmic interpretation can alter the analysis.

Functions that commonly indicate SaMD

  • An application uses a smart-device microphone to detect interrupted breathing during sleep and issues an alert.
  • Software analyzes heart-rate signals or patterns and presents findings to a clinician as an aid in diagnosing arrhythmia.
  • A cloud service interprets medical images or physiological data and returns a result used for diagnosis, monitoring, or treatment decisions.

These examples illustrate the principle rather than create automatic legal classifications. The exact intended use, output, user, and market still have to be documented and assessed.

How regulators think about SaMD risk

IMDRF’s risk framework uses two dimensions. First is the significance of the information supplied to a healthcare decision. Second is the healthcare situation or condition in which the software is used. Product teams should record both dimensions rather than assigning a risk label based only on the algorithm or technology.

Dimension 1: significance of the information

Consider what the user does with the output:

  • Treat or diagnose: The output directly informs treatment or diagnosis and can carry substantial clinical consequences.
  • Drive clinical management: The result helps determine whether to monitor, refer, admit, discharge, or otherwise manage a patient.
  • Inform clinical management: The output contributes useful information but is one input among several.
  • Guide routine or non-clinical actions: The result has a lower direct effect on a clinical decision, although the complete context still matters.

Use the strongest realistic claim and workflow, not a narrower description that ignores how customers are expected to use the result.

Dimension 2: healthcare situation

Describe the condition and setting in which the software operates. A failure during an emergency or for a life-threatening condition can have greater consequences than a failure in routine wellness monitoring. Document whether the software is intended for critical, serious, or non-serious conditions and whether users will rely on it in urgent settings.

Why the combination matters

A product that provides highly significant information in a critical healthcare situation generally demands more rigorous controls and evidence than a product that supplies supplementary information for a non-serious condition. The framework is a way to scale engineering, quality, clinical, and review work. An IMDRF category is not automatically a legal device class in every country; each jurisdiction applies its own definitions and routes.

Risk question What to document How it affects the program
Significance of information Whether the output treats, diagnoses, drives management, informs management, or has a less direct role Depth of hazard analysis, verification, human-factors work, clinical evidence, and review
Healthcare situation Condition severity, urgency, patient population, and care setting Required reliability, fail-safe behavior, escalation, monitoring, and postmarket response
Combined risk The documented intersection of the two dimensions Proportionate controls and the market-specific regulatory pathway

Define intended use and claims before building

A controlled intended-use statement is the anchor for the entire product. It should identify the medical purpose, target users, patient population, data inputs, output, clinical context, and important limitations. Write it in plain language first; then map each phrase to the product requirements and regulatory analysis.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical intended-use template

Describe the software as: “Software that [performs function] using [specified inputs] to provide [output] for [named users] in [healthcare situation] for [medical purpose], subject to [material limitations].” The wording is a working control, not marketing copy. Every release, claim, and customer-facing instruction should remain consistent with it.

Build a claim inventory

Create a table of every external claim and classify it as medical, administrative, wellness, or technical. Include website language, demonstrations, sales scripts, app-store descriptions, user-interface labels, alert text, and customer configuration options. A feature presented as “wellness tracking” can become a medical function if the interface tells users that a result detects disease or determines treatment.

Separate mixed-product functions

Many products contain both regulated and non-regulated functions. Separate the SaMD function from hardware control, administrative workflow, wellness content, and display-only components. Define interfaces between them, identify which component generates the clinical output, and prevent a non-medical module from silently expanding the intended use.

Build a lifecycle quality system

IMDRF quality-system guidance covers the full lifecycle: requirements management, design, development, verification and validation, deployment, maintenance, and decommissioning. The system should be scaled to complexity and risk, then updated as new information, incidents, or milestones change the product.

Core records and controls

  • Intended-use and claims record: The approved purpose, users, population, outputs, limitations, and market assumptions.
  • Requirements specification: Functional, performance, safety, usability, interoperability, privacy, cybersecurity, and regulatory requirements.
  • Traceability: Links from intended use to requirements, architecture, hazards, tests, defects, and release decisions.
  • Architecture and risk files: Data flows, dependencies, interfaces, failure modes, hazard controls, residual risk, and assumptions about users and environments.
  • Verification and validation plans and results: Objective evidence that requirements are met and the software works for its intended use.
  • Release and change control: Approval criteria, version identification, regression testing, rollback, and assessment of whether a change alters intended use or regulatory status.
  • Cybersecurity and maintenance plans: Threat management, vulnerability intake, patching, dependency updates, access control, backup, monitoring, and end-of-support decisions.
  • Complaint and incident handling: Intake, triage, investigation, reportability assessment, corrective action, and trend review.
  • Decommissioning plan: Notice, data retention or export, service shutdown, replacement, account closure, and safe handling of devices or integrations that depend on the software.

Why updates need regulatory discipline

SaMD changes after launch: models are retrained, operating systems change, libraries are patched, interfaces are redesigned, and data sources evolve. A change-control process should classify each change, identify affected hazards and claims, run appropriate regression and validation tests, and determine whether a new submission, notification, or other market action is required. Do not treat a software release as purely technical when it can change a clinical output or user decision.

Plan the three parts of clinical evaluation

IMDRF clinical-evaluation guidance describes three connected evidence questions. They are harmonized principles for jurisdictions to adopt, not regulations by themselves. The evidence should be appropriate to the intended use, target users, healthcare situation, and risk.

1. Clinical association

Show that there is a valid relationship between the software’s output and the targeted clinical condition or clinical concept. For example, if an algorithm claims to identify a pattern associated with arrhythmia, the team must establish why that pattern is clinically meaningful and supported by appropriate medical knowledge and data.

2. Analytical validation

Show that the software reliably produces its stated output from its inputs. This includes accuracy, repeatability, reproducibility, handling of missing or corrupted data, performance across relevant devices or data sources, and behavior at boundaries. Test data and acceptance criteria should be defined before results are reviewed, with defects and deviations controlled.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Clinical validation

Show that the output achieves the intended purpose in the target users, population, and context. A technically accurate classifier may still fail to improve or safely support a clinical decision if the workflow, user interpretation, or patient population differs from the development setting. Clinical validation should therefore reflect the real use environment and foreseeable use errors.

Evidence question Core question Typical evidence focus
Clinical association Is the output meaningfully related to the targeted condition or clinical concept? Medical rationale, literature, reference standards, and justification of the claimed relationship
Analytical validation Does the software reliably generate the specified output? Accuracy, repeatability, robustness, data quality, interfaces, and performance across relevant inputs
Clinical validation Does the output achieve the intended purpose for target users and patients? Representative users, population, workflow, context, endpoints, and clinically meaningful performance

Plan these evidence streams early. Retrofitting clinical validation after architecture, claims, and user workflows are fixed can force expensive redesign.

Cybersecurity, data, and human factors are clinical controls

For SaMD, cybersecurity and usability are not separate from safety. A compromised dependency, unavailable cloud service, corrupted input, misleading alert, or poorly understood result can create the same patient harm as a coding defect.

Cybersecurity planning

  • Map every data flow, interface, third-party service, model endpoint, and administrative privilege.
  • Define authentication, authorization, encryption, logging, backup, recovery, and availability requirements.
  • Maintain an inventory of software components and a process for vulnerability intake, assessment, patching, and customer communication.
  • Test failure and degraded modes, including loss of connectivity, stale data, invalid input, service outage, and partial deployment.
  • Specify how security updates are verified, released, rolled back, and evaluated for effects on clinical performance.

Human-factors questions

Identify who sees the output, what they are expected to do next, and what happens when they disagree with it. Design alerts with clear meaning, priority, timing, and acknowledgment behavior. State when the result is unavailable or outside validated conditions. Evaluate foreseeable misuse, over-reliance, alert fatigue, and interpretation errors in the target workflow.

Choose a market-specific pathway

IMDRF documents help harmonize concepts, but they do not replace national or regional law. Before launch, confirm the applicable definition, classification, premarket route, quality-system obligations, clinical evidence expectations, labeling rules, registration requirements, and postmarket duties for every market.

Questions to resolve with the relevant regulator or qualified counsel

  1. Is the product a medical device under the local legal definition?
  2. Which function, claim, and version determine the classification?
  3. Does the market recognize an equivalent device, and what evidence is required for that route?
  4. What quality-system standard, supplier controls, software documentation, and cybersecurity materials are expected?
  5. What clinical evidence, performance testing, usability work, and labeling must accompany the submission?
  6. What reporting, monitoring, correction, recall, and change-notification duties apply after launch?
  7. Do cloud hosting, data location, cross-border transfers, or professional licensing rules create additional obligations?

Do not assume that a product’s IMDRF risk category, FDA treatment, or approval in one country automatically determines its status elsewhere. The same software may need different evidence or a different route because legal definitions, enforcement policies, and postmarket rules differ.

A practical product-team decision path

  1. Write the intended purpose and claims. Include users, patients, inputs, outputs, clinical setting, limitations, and every customer-facing claim.
  2. Map each function to the legal definitions. Distinguish standalone medical functions from hardware control, administration, wellness, storage, transfer, and display-only functions.
  3. Identify the decision supported. State whether the output treats, diagnoses, drives management, informs management, or has a less direct role.
  4. Characterize the healthcare situation. Record the condition severity, urgency, population, and setting, then document the resulting risk rationale.
  5. Establish the lifecycle quality plan. Put requirements, traceability, risk management, verification, validation, release control, cybersecurity, complaints, maintenance, and decommissioning under controlled processes before development scales.
  6. Plan the evidence. Define clinical association, analytical validation, and clinical validation questions, datasets, users, endpoints, acceptance criteria, and documentation.
  7. Confirm the market route. Obtain a jurisdiction-specific determination from the relevant regulator or qualified regulatory counsel before making launch commitments.
  8. Operate the postmarket loop. Monitor complaints, incidents, performance drift, cybersecurity events, model changes, and new clinical information; feed the results into risk and release decisions.

Common mistakes and how to correct them

Calling a medical feature “wellness” without changing the claim

Problem: The interface or marketing tells users that a result detects, rules out, or manages a condition while the regulatory file describes general wellness.

Correction: Reconcile every claim with the intended-use statement, or redesign the feature and language so the non-medical purpose is genuine and consistent.

Correction: Start with the medical purpose, user, output, decision, and healthcare situation; then apply the jurisdiction’s definitions and policies.

Testing the algorithm but not the workflow

Problem: Bench accuracy is treated as proof that clinicians or patients can safely use the result.

Correction: Separate analytical validation from clinical validation and test representative users, populations, workflows, and foreseeable errors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Treating launch as the end of quality work

Problem: Updates, vulnerabilities, complaints, and model drift are handled informally.

Correction: Maintain controlled release, change, cybersecurity, incident, and postmarket processes through maintenance and decommissioning.

Assuming one country’s decision travels everywhere

Problem: A classification or enforcement policy from one market is used as a global answer.

Correction: Maintain a market matrix covering legal scope, pathway, evidence, QMS, labeling, registration, and postmarket obligations in each target jurisdiction.

What product leaders should retain

  • SaMD is software with a medical purpose that performs that purpose without being part of a hardware medical device.
  • Intended purpose and claims determine scope; identical technical capabilities can receive different treatment when the claims or users change.
  • Risk depends on both the healthcare situation and the significance of the information supplied to the decision.
  • A credible quality system runs from requirements through development, verification, validation, deployment, maintenance, and decommissioning.
  • Clinical evaluation connects clinical association, analytical validation, and clinical validation; none of these alone proves the full intended use.
  • IMDRF principles support harmonization, but each jurisdiction decides the legal classification and required pathway.
