Alpha-gal syndrome (AGS) is a potentially serious allergic condition in which the immune system reacts to galactose-α-1,3-galactose, a sugar found in most mammals. In the United States, it is most often associated with lone star tick bites. Reactions to foods such as mammalian meat commonly begin hours after eating, and diagnosis depends on symptoms and exposure history as well as a blood test—not the test result alone.
What is alpha-gal syndrome?
Alpha-gal is short for galactose-α-1,3-galactose, a carbohydrate naturally present in most mammals but not in people. AGS is an allergic reaction to alpha-gal. It is sometimes called “red meat allergy,” but that name can be misleading: some people may also react to dairy, gelatin, or other mammal-derived ingredients, while others tolerate some of these products.
The syndrome is not simply an intolerance to meat. It can cause allergic symptoms affecting the skin, digestive system, breathing, or circulation, and in some cases it can lead to anaphylaxis. See the CDC overview of alpha-gal syndrome for additional background.

Some ticks have alpha-gal in their saliva. When an implicated tick bites a person, alpha-gal may enter the bloodstream and prompt the immune system to develop antibodies. Later exposure to alpha-gal in certain products can trigger an allergic reaction. In the United States, the lone star tick is most often associated with AGS; the CDC has also reported a few U.S. cases after blacklegged and western blacklegged tick bites. Tick species linked with AGS differ in other parts of the world.

Not everyone bitten by a tick associated with AGS develops the condition, and the reason for that difference is not known. Symptoms may take weeks to months to begin after a tick bite, so the bite and a later reaction may not seem connected at first.
What are the symptoms, and when do they start?
AGS reactions usually begin 2–6 hours after exposure to products such as red meat or dairy, although timing and symptoms can vary. A person may not react to every potential exposure, and reactions to the same product may differ from one occasion to another. This delay can make it harder to identify the trigger than with allergies that cause symptoms soon after eating.

Possible symptoms include:
- Hives or an itchy rash
- Nausea, vomiting, diarrhea, severe stomach pain, heartburn, or indigestion
- Cough, shortness of breath, or difficulty breathing
- Swelling of the lips, throat, tongue, or eyelids
- Low blood pressure, dizziness, or faintness
- Anaphylaxis, a severe, potentially life-threatening allergic reaction
Symptoms range from mild to life-threatening and can involve more than one body system. If you are experiencing a severe allergic reaction or possible anaphylaxis, seek emergency care immediately. If you have been prescribed epinephrine, follow your clinician’s plan; the CDC advises seeking emergency care even after using it. The CDC symptoms guidance describes the range and timing of reactions.

Clinicians assess the pattern of symptoms and exposures, perform a physical examination, and may order a blood test for alpha-gal-specific IgE (sIgE). The history can include what was eaten or otherwise encountered, how long it took for symptoms to start, what the symptoms were, and whether there were tick bites or outdoor exposure. Allergy skin testing to mammalian meat allergens may also help.

A positive blood test does not by itself establish AGS. As the CDC states, “However, a positive alpha-gal sIgE test does not mean a person has AGS.” The result needs to be interpreted alongside a compatible clinical history; sensitization on a test is not the same as having symptomatic syndrome. The CDC’s clinical diagnosis and testing guidance, updated January 5, 2026, explains this distinction.
The CDC/Council of State and Territorial Epidemiologists surveillance case definition lists alpha-gal sIgE of at least 0.1 IU/mL or 0.1 kU/L as confirmatory laboratory evidence within its surveillance criteria. That threshold is not a stand-alone patient diagnosis or a universal interpretation rule; the CDC says the surveillance definition does not replace clinical diagnosis. A CDC MMWR report published July 2, 2026 also highlights the difference between testing positive for alpha-gal IgE and having clinical AGS, and advises testing people with compatible symptoms rather than relying on tick exposure alone.

What foods and products may trigger reactions?
Potential sources include products from mammals. The CDC describes red meat in this context as including beef, pork, lamb, venison, and rabbit. Some people may also need to consider dairy, gelatin, or other mammal-derived ingredients. The relevant triggers differ between individuals; a diagnosis does not mean every person will react to every product containing alpha-gal.

Work with an allergist or other healthcare provider to identify which products matter in your case rather than adopting a universal avoidance list. Read food labels and discuss medicines or vaccines with a healthcare provider if you are concerned about their ingredients. Reactions to ingredients in some products are very rare and individualized; do not stop a prescribed medicine or vaccine on your own. How is alpha-gal syndrome managed and prevented?
Management is individualized. A clinician can help identify relevant triggers, plan avoidance, and prepare for emergencies. People diagnosed with AGS should also take steps to prevent future tick bites, since new bites may reactivate allergic reactions.
- Use an EPA-registered tick repellent with an active ingredient such as DEET, picaridin, IR3535, oil of lemon eucalyptus, para-menthane-diol, or 2-undecanone, following its label.
- Consider clothing and gear treated with 0.5% permethrin, following product directions.
- Have an emergency plan from your healthcare provider, including how and when to use any prescribed epinephrine.
Tick-prevention measures can reduce exposure risk; they do not diagnose, treat, or reverse AGS.
How common is alpha-gal syndrome?
The true number of people with AGS is unknown. The CDC reported more than 110,000 suspected cases identified in the United States from 2010 through 2022 on a page updated in 2025; it cautions that AGS is not nationally notifiable and that this count is not the true burden. The CDC has also cited a 2023 estimate that as many as 450,000 people in the United States may be affected. That figure is an estimate, not a confirmed case count.
