The most defensible way to support brain health with age is to combine ordinary health measures: move regularly, manage blood pressure and other medical conditions with a clinician, eat a balanced diet, address hearing loss, stay mentally and socially engaged, and seek care for sleep or mood problems. These habits support cardiovascular, sensory and emotional health and may support cognition.

They are not guarantees against dementia. The National Institute on Aging (NIA) describes evidence for several preventive approaches as “encouraging but inconclusive.” The useful question is therefore not which single product or hack prevents dementia, but which habits have reasonable benefits, what the studies actually measured, and how to apply them safely.

What “supporting brain health” actually means

Brain health includes the ability to think, learn, remember, communicate, regulate mood and carry out everyday activities. It is affected by blood flow, hearing, sleep, mood, medications, illness, social connection and physical activity. Improving one of those areas can be worthwhile even when a study has not proved that it prevents dementia.

That distinction matters because studies use different outcomes. An intervention might improve general cardiovascular health, test performance or the risk of mild cognitive impairment (MCI) without proving a reduction in dementia diagnoses. Some findings are observational associations, while others come from randomized interventions. Age, baseline health and dementia risk also change how much a result can be generalized.

The NIA’s overview of cognitive health and older adults and its review of what is known about preventing Alzheimer’s disease are useful starting points because they separate plausible healthy-aging advice from claims that remain unproven.

Move regularly, starting at a level you can sustain

The practical target

Federal guidance cited by the NIA recommends at least 150 minutes (2.5 hours) of physical activity each week for adults. That can be divided across the week rather than completed in one session. Walking is a reasonable starting point if you are inactive; pace and duration can increase gradually as your fitness and confidence improve.

Activity can also include forms that protect independence: strengthening exercises for major muscle groups, balance work and activities that raise your heart rate. Choose options that fit your joints, mobility, medications and available space. If you have been inactive, have heart or lung disease, or are worried about falling, ask a health professional how to begin safely.

What the evidence supports—and what it does not

Regular movement supports strength, balance, mood and cardiovascular health. Those benefits are independently valuable and reduce some of the conditions that can interfere with daily functioning. Research connecting activity with cognitive outcomes is encouraging, but the NIA says more research is needed to determine how much exercise prevents cognitive decline. No exercise schedule can promise that an individual will avoid dementia.

Turn the recommendation into a routine

  1. Pick a repeatable cue. Attach a short walk to a meal, a regular appointment or another daily event.
  2. Build time before intensity. Add minutes or another day only when the current amount feels manageable.
  3. Cover more than endurance. Include strength and balance work when your clinician says it is appropriate, because the goal is functional health as well as aerobic fitness.
  4. Track symptoms, not just minutes. New chest pain, unusual breathlessness, dizziness or repeated falls are reasons to stop and seek medical advice.

Manage blood pressure and other vascular risks with a clinician

Why blood pressure matters

High blood pressure, particularly in midlife, is a risk factor for later cognitive decline. Controlling it also protects the heart, kidneys and blood vessels. Take prescribed medicines as directed, attend follow-up visits and discuss home readings rather than changing a dose on your own.

What SPRINT-MIND found

The NIA reports that SPRINT-MIND followed more than 8,000 adults aged 50 and older who had elevated cardiovascular risk. In that trial, intensive systolic blood-pressure lowering to below 120 mmHg reduced the risk of MCI over five years compared with the standard target used in the study. The finding is important evidence that vascular treatment can affect a cognitive outcome, but it is not a universal blood-pressure prescription.

Lower targets can create risks such as dizziness, falls or medication-related complications for some people. The appropriate target depends on age, frailty, other conditions, current medicines and how readings were taken. Use the trial as a reason to have a careful treatment conversation, not as a number to pursue without supervision. The NIA’s prevention summary explains this context at Preventing Alzheimer’s Disease: What Do We Know?

A broader vascular checklist

  • Know your blood-pressure readings and bring a log to appointments if your clinician recommends home monitoring.
  • Review diabetes, cholesterol, smoking, weight, sleep and physical activity as part of overall cardiovascular care.
  • Report medication side effects, faintness or falls promptly; an apparently “better” reading is not useful if treatment is unsafe.
  • Ask for an individualized plan after a stroke or other vascular event, when cognitive symptoms and rehabilitation needs can change.

Eat a balanced diet without treating food as a cure

Mediterranean and MIND patterns

The Mediterranean and MIND diet patterns are associated with better cognitive outcomes in some studies, but the NIA describes the evidence as promising and inconclusive. These patterns generally emphasize vegetables, fruits, beans, whole grains, nuts and other minimally processed foods, with fish and unsaturated fats included in Mediterranean-style eating and limits on foods high in saturated fat or added sugar. The useful takeaway is to improve overall dietary quality, not to treat a named diet as a guaranteed dementia-prevention program.

Diet changes should fit allergies, cultural foods, budget, kidney or digestive disease and any medicines that interact with food. A registered dietitian or clinician can adapt the pattern when a medical condition requires a specific amount of protein, potassium, sodium or another nutrient.

Why supplements deserve skepticism

NIA does not currently recommend a vitamin or supplement to prevent Alzheimer’s disease or other cognitive decline. Clinical trials have reported some cognitive benefits from daily multivitamins, but an improvement on cognitive testing is not proof that a supplement prevents dementia. Products marketed as “brain boosters” can also interact with medicines, vary in quality or delay evaluation of a real medical problem.

Correct a documented deficiency when a qualified professional identifies one. Do not infer that a high dose is better, and do not replace blood-pressure treatment, hearing care or an evidence-based diet with a supplement routine.

Check hearing and treat hearing loss

Why hearing belongs in a brain-health plan

Untreated hearing difficulty can make conversation, exercise classes and other mentally stimulating activities harder to follow. It can also contribute to withdrawal or misunderstanding. Arrange a hearing assessment if speech is difficult to follow, the television needs progressively higher volume, or family members notice that you miss sounds.

What the ACHIEVE report showed

In a 2025 progress report, the NIA summarized the ACHIEVE study as finding that hearing aids were associated with an almost 50% slower rate of cognitive decline in older adults at higher dementia risk. The result was a subgroup finding and was not established for every participant group. It should therefore be read as evidence for evaluating and treating hearing loss—not as proof that hearing aids function as a universal cognitive supplement.

Start with an appropriate assessment, then discuss hearing aids or other treatment recommended for your type and degree of loss. Proper fitting, regular use, maintenance and communication strategies all affect whether treatment helps you participate in daily life.

Stay mentally and socially engaged

Choose activities that are meaningful and varied

The NIA says cognitive training appears to help maintain cognitive health in older adults. Training can include learning, practicing a skill, solving problems or following structured exercises. Ordinary activities—reading, music, writing, languages, crafts, volunteering or learning technology—can also provide challenge and enjoyment. No particular game, app or puzzle has been shown to prevent dementia for everyone, so select activities you will continue rather than chasing a “brain-training” score.

Social engagement adds conversation, planning and emotional support. Schedule regular contact in a way that suits your hearing, mobility and energy: a club, class, family call, shared meal, faith community or volunteer role can all count. If transportation or cost is a barrier, libraries, community centers and remote groups may offer alternatives.

Do not overlook treatable health problems

Stroke, depression and delirium can affect cognitive function. Sudden confusion is different from the gradual changes associated with aging and needs urgent medical assessment. Persistent low mood, loss of interest, major sleep disruption or noticeable changes in memory and daily functioning warrant a conversation with a clinician. Treating the underlying problem may improve thinking and quality of life even when it does not alter long-term dementia risk.

Consider a combined approach rather than a single “brain” habit

What U.S. POINTER tested

The NIA’s 2026 Alzheimer’s disease and related dementias progress report describes the U.S. POINTER multidomain intervention. It combined exercise, diet, cognitive and social engagement, and health monitoring. In high-risk older adults, the intervention improved global cognition over two years.

This result supports organizing care around several modifiable areas at once. It does not identify one magic ingredient, establish that every older adult will experience the same change, or prove prevention of dementia. The participants were high-risk older adults and the reported outcome was global cognition over two years; those boundaries belong with the headline result. The progress report is available from the NIA at 2026 NIH Alzheimer’s Disease and Related Dementias Research Progress Report.

A manageable combined plan

  1. Measure your starting point. Note current activity, blood-pressure follow-up, hearing concerns, sleep, mood and social routines.
  2. Add one change at a time. A short walk, a scheduled appointment or one additional social activity is easier to maintain than a complete overhaul.
  3. Use professional care for medical risks. Review blood pressure, medicines, hearing and mood with the appropriate clinician.
  4. Reassess function. Look for easier participation, steadier movement, better communication or improved daily routines—not only a memory score.

How to interpret brain-health claims

When a headline promises that a habit or product “prevents dementia,” check five details before accepting it:

Question Why it changes the meaning
What type of evidence is it? An observational association can identify a pattern, while a randomized intervention provides a different level of evidence.
What outcome changed? General health, a cognitive test, MCI and dementia are not interchangeable outcomes.
Who was studied? Age, cardiovascular risk, hearing status and baseline cognitive risk determine how broadly a result can be applied.
How long was follow-up? A two-year cognition result or a five-year MCI result does not establish lifelong dementia prevention.
Is it a clinical recommendation? An intervention may justify further research or a discussion with a clinician without being suitable for everyone.

For example, the SPRINT-MIND result concerns MCI in adults at elevated cardiovascular risk, the ACHIEVE result concerns slower decline in a higher-risk hearing subgroup, and U.S. POINTER concerns global cognition after two years in high-risk older adults. Keeping those populations and outcomes attached to each claim prevents a narrowly supported finding from becoming an exaggerated promise.

When to seek help

  • Ask a clinician about blood pressure, diabetes, cholesterol, medicines or a prior stroke rather than trying to manage these risks alone.
  • Arrange hearing evaluation when communication becomes difficult or others notice missed sounds.
  • Seek prompt medical attention for sudden confusion, a sudden neurological change or a rapid decline in function.
  • Discuss persistent depression, anxiety, severe sleep problems or loss of interest in usual activities.
  • Bring a trusted person to an appointment if memory or communication problems make it difficult to describe changes accurately.

Supporting brain health is a long-term health practice: move, protect vascular health, eat well, hear well, stay engaged and address problems early. Those steps have solid reasons beyond dementia prevention, while the dementia-specific evidence remains limited and dependent on the person, intervention and outcome studied.
