If an NFL team’s backup quarterback is injured, play may stop so he can receive medical attention. The team may turn to a designated emergency third quarterback only if its first two quarterbacks on the game-day Active List cannot participate because of injury or disqualification. The coach cannot use that emergency player simply to replace a healthy quarterback for performance or conduct reasons.
What happens next depends on which quarterbacks are available
An injury does not automatically activate a third quarterback. The NFL’s emergency-third-quarterback rule, as reproduced in an NFL.com report from 2023, applies only when both quarterbacks on the game-day Active List are unable to participate because of injury or disqualification.

Who can serve as the emergency third quarterback?
The 2023 NFL.com report says a club may designate one emergency quarterback from its 53-player Active/Inactive List; elevated players are not eligible for that designation. The report also says clubs must establish their game-day Active List 90 minutes before kickoff. These are details from the bylaw text reproduced in that 2023 report, not a newly issued 2026 bylaw text.

The rule is conditional, not a way to expand the quarterback depth chart whenever a coach wants another option. The coach cannot activate the emergency quarterback because the starter or backup is playing poorly or has been removed for conduct.
What if one of the first two quarterbacks returns?
If medical staff clear either of the first two game-day quarterbacks to return, the emergency quarterback must leave the game. He cannot continue at quarterback or move to another position. The 2023 report says he may re-enter at quarterback if another emergency-third-quarterback situation arises.
How the injury stoppage and medical evaluation work
An official can call an injury timeout to allow a player to receive medical attention. NFL ATC spotters also monitor game action and broadcast video and can prompt a stoppage in specified circumstances—for example, when a player appears obviously disoriented or unstable and is trying to remain in the game without medical attention. Suspected concussion has additional safeguards: NFL guidance requires removal for assessment when signs or symptoms are present or an authorized person initiates the protocol. A player diagnosed with a concussion must complete the league’s return-to-participation process and receive the prescribed clearances before returning to practice or a game. The NFL Football Operations site presents a 2026 NFL Rulebook. Its listed current-season rule changes concern kickoff adjustments and consultation on potential disqualifications; it does not list an emergency-third-quarterback change. That absence is not the same as a newly published 2026 version of the bylaw passage. The detailed emergency-quarterback language cited here is reproduced in NFL.com’s 2023 report.

Does a team automatically forfeit if it runs out of quarterbacks?
The cited emergency-quarterback provision explains when a designated third quarterback may be used; it does not say that losing the first two quarterbacks automatically results in a forfeit. The available sources therefore support neither an automatic-forfeit claim nor a broader guarantee about what would happen in every unusual game circumstance.
