Seeking mental health care is not, by itself, an automatic FAA disqualification. The FAA encourages pilots to seek appropriate counseling, but medical certification depends on the condition, symptoms, treatment, medication, records, and applicable FAA standards. Applicants must answer the medical application accurately and disclose the conditions and medications the FAA asks about. An aviation medical examiner (AME) may issue a certificate when requirements are met, or defer the decision to the FAA for further review.

What happens when a pilot seeks mental health care?

The FAA’s guidance says counseling or therapy is encouraged when medically appropriate. Its Pilot Mental Fitness page also says that most mental-health conditions, if treated, do not disqualify a pilot, while identifying some conditions as disqualifying under the FAA’s stated policy. That general guidance is not a prediction about an individual case: the result depends on the person’s circumstances and the applicable certification rules.

Seeking care does not automatically mean a special issuance, denial, or loss of a certificate. It does mean that an applicant should understand what the FAA asks them to report and be ready for questions or requests for records. The FAA’s therapy, psychotherapy, and counseling handout, updated May 27, 2026, advises pilots to work with a therapist, counselor, physician, or other provider when they are considering counseling.

What must you disclose to the FAA?

On the medical application, FAA guidance directs applicants to report health-professional visits during the previous three years, medications they are taking, and other medical history requested by the application. During the exam, applicants must disclose existing physical and psychological conditions and medications. The AME may ask follow-up questions based on the application and examination.

The FAA describes the process in How do I get a Medical Certificate and what to expect during the AME examination: submit the application through MedXPress, attend an exam with an FAA-designated AME, and provide relevant records. The AME may issue a certificate when the standards are met and required documentation is available. In some cases the AME must defer the application to the FAA, which may request additional information before deciding.

How does counseling compare with antidepressant treatment?

The key practical difference is that medication can bring additional FAA restrictions and a specific review pathway. Counseling without medication is not automatically certification-neutral: the condition, symptoms, and application answers still matter.

Situation What the FAA guidance establishes What to expect
Counseling or therapy without medication The FAA encourages counseling when medically appropriate. A visit or diagnosis is not, by itself, proof of disqualification. Report information requested by the application and disclose existing conditions during the exam. The AME may ask questions or request records.
Antidepressant treatment that is continuing Psychotropic medication is generally disqualifying for routine aeromedical certification, but the FAA has a conditional, case-by-case antidepressant pathway. Applicants seeking to continue medication may need evaluation by a HIMS AME and FAA authorization; an AME cannot treat the pathway as an automatic approval.
Antidepressant stopped The FAA describes a route to reapply for regular issuance after a 60-day period off medication and a favorable report from the treating physician. Do not stop or change medication to pursue certification without the treating clinician’s direction. The FAA’s current requirements and the applicant’s case determine the next steps.

The FAA’s AME Guide: Item 47, Psychiatric Conditions and its Use of Antidepressant Medications guidance describe the medication rules and exceptions. Requirements can change, so consult the FAA’s current guidance rather than relying on an old medication list or a general online summary.

What is the FAA antidepressant pathway?

Some applicants taking an antidepressant may be considered for special issuance or special consideration. This is a conditional review, not a guaranteed clearance. FAA guidance describes possible consideration for certain diagnoses when the applicant has been clinically stable for at least three continuous months on a stable dose, without aeromedically significant side effects or increased symptoms, and uses a conditionally acceptable single medication.

The FAA lists exclusions, including psychosis, suicidal ideation, electroconvulsive therapy, concurrent use of multiple SSRIs, and a multi-agent psychiatric drug protocol. Applicants who meet the stated conditions and wish to remain on medication are directed to further evaluation by a HIMS AME. The FAA’s list of conditionally acceptable medications and formulations is subject to conditions; it was updated August 27, 2025, and should be checked at the current FAA antidepressant guidance page.

For initial antidepressant certification, the FAA’s applicant handout, updated August 26, 2026, tells airmen to seek care, contact a HIMS AME, gather relevant treatment records, and document three months of stability on the same dose without symptoms or side effects before the HIMS AME determines whether a submission is appropriate. The handout also instructs applicants in this pathway not to fly under 14 CFR 61.53 until they have FAA authorization. That instruction applies to this pathway; do not assume it describes every pilot’s circumstances. See the FAA initial certification handout and its FAQ for applicants treated with antidepressants.

What records should you prepare?

Follow the documentation instructions that apply to your case; a routine portal printout may not be enough. The FAA says that some conditions require a detailed clinical progress note from a visit no more than 90 days before the AME exam. It cautions that an After Visit Summary or portal notes may omit required details and says that, in most cases, it requests the actual clinical progress note rather than a new physician letter. Check the FAA’s medical certification guidance for the process and any case-specific requests.

  • For an antidepressant pathway, the FAA’s initial applicant instructions call for treatment records from periods involving psychiatric medication or symptoms. Use the FAA certification aid and coordinate required provider reports through the HIMS AME.
  • For certain FAA-requested psychiatric or psychological reviews, the FAA specifies records review, detailed clinical interviews, a mental status examination, and integrated findings with an explicit diagnosis and recommendations.
  • When both are required, separate psychiatrist and clinical psychologist reports may be needed. The FAA recommends evaluators familiar with aerospace medicine or psychology because a lack of that background may limit a report’s usefulness.

The FAA’s Specifications for Psychiatric and Psychological Evaluations describe these evaluations. The exact packet depends on the FAA’s request and the certification pathway; do not assume every applicant needs the same reports.

What does an FAA deferral mean?

A deferral means the AME is sending the case to the FAA for a decision rather than issuing the certificate at the exam. It is not the same as a denial. The FAA says an applicant who is deferred generally receives a letter listing the information to provide. The FAA then reviews the case; the outcome depends on whether the applicant meets the relevant standards and supplies the required documentation.

Because missing records can delay review, ask the AME what documents are needed for your specific history and whether the FAA has issued instructions for your case. A HIMS AME is the relevant point of contact for applicants pursuing the FAA’s continued-antidepressant pathway.

Does FAA certification determine airline employment?

No. FAA medical certification and an airline’s own employment screening are separate matters. The FAA notes that commercial airlines may have their own mental-health screening and requirements. A decision about an FAA medical certificate does not establish what a particular employer will require.

How should pilots interpret the FAA’s denial statistic?

The FAA reports that about 0.1% of medical-certificate applicants who disclose health issues are denied, according to its page checked in 2026 and last updated in 2024. That broad figure is not broken down by mental-health condition, medication, or applicant subgroup, so it is not a mental-health-specific denial rate or an estimate of an individual pilot’s chances. See the FAA’s Pilot Mental Fitness page for its stated context.
