Short answer: Google’s AMIE performed better than primary-care physicians on many scored dimensions in a controlled, text-chat simulation with trained patient actors. That result does not show that AMIE is a better doctor in ordinary practice, replaces a clinician, or is ready for unsupervised care. The later clinic study found that supervised pre-visit chats were feasible, while physicians still scored higher on the practicality and cost-effectiveness of management plans.
What AMIE is, and what the headline comparison actually measured
AMIE stands for Articulate Medical Intelligence Explorer. Google describes it as a research AI system built on a large language model and optimized for diagnostic reasoning and medical dialogue. It is a system under study, not a generally available primary-care service or a licensed clinician.
The often-repeated claim that “Google’s AI beat primary-care doctors” refers to a specific experiment. It does not mean AMIE saw patients in normal clinics and outperformed their doctors. The principal comparison used synchronous text conversations between AMIE or a board-certified primary-care physician (PCP) and a trained patient actor working through a standardized case. The format borrowed from an objective structured clinical examination (OSCE), but Google says it was not intended to reproduce a traditional face-to-face OSCE or normal clinical messaging, email, chat, telemedicine, or office workflows.

The study tested how two conversational agents handled selected diagnostic scenarios in the same text-only setting. How the main AMIE evaluation was designed
Randomized, double-blind crossover format
The peer-reviewed publication in Nature describes a randomized, double-blind crossover study. In a crossover design, the comparison is made across the same broad set of cases, with the identity of the conversational partner concealed from evaluators during assessment. The participants were not ordinary walk-in patients: they were trained actors using validated case scenarios.
The indexed abstract reports 159 case scenarios supplied by providers in Canada, the United Kingdom, and India, and 20 primary-care physicians. Specialist physicians and the patient actors rated the interactions. The use of multiple countries and independent evaluator groups broadens the assessment, but it remains a controlled sample rather than a survey of everyday primary care.
Text chat was the common interface
Both AMIE and the physicians communicated through synchronous text chat. The physicians therefore worked through an interface the study authors say was unfamiliar to them. A doctor who is highly effective in a room—using tone, timing, physical cues, a hands-on examination, and an electronic health record—was assessed without those tools. Conversely, AMIE was evaluated in a setting that favors a system designed to generate and organize text.

The interface choice is not a minor technical detail. Communication channel changes what information is available, how quickly uncertainty can be resolved, and how a participant can notice distress or deterioration. A text-only score is evidence about text-only performance on the assigned cases.
What the published results say
| Measure | Published figure | What it represents |
|---|---|---|
| Case scenarios | 159 | Standardized scenarios in the peer-reviewed Nature abstract, involving providers in Canada, the United Kingdom, and India |
| Primary-care physicians | 20 | Board-certified PCP comparators in the simulated evaluation |
| Specialist-rated axes | AMIE rated higher on 30 of 32 | Performance dimensions scored by specialist physicians |
| Patient-actor-rated axes | AMIE rated higher on 25 of 26 | Performance dimensions scored by the trained actors who played patients |
On the published figures, specialists rated AMIE higher than the PCPs on 30 of 32 performance axes, while patient actors rated it higher on 25 of 26. The abstract also reports greater diagnostic accuracy for AMIE in the study setting. “Higher” describes the ratings and accuracy observed in these simulated conversations; it is not a finding that AMIE is more accurate across all diagnoses or patient populations.
Why two sets of figures appear online
Google’s January 2024 Research post described an earlier reporting snapshot: 149 scenarios, with AMIE higher on 28 of 32 specialist-rated axes and 24 of 26 patient-actor-rated axes. The peer-reviewed abstract later reported 159 scenarios and the 30-of-32 and 25-of-26 results. These are different versions of the reported analysis, not numbers to blend into one result. For the headline account, the peer-reviewed figures are the appropriate ones; the earlier post is useful for understanding the study design and chronology.

What those ratings can—and cannot—tell you
What the experiment supports
- AMIE can conduct coherent diagnostic dialogue in a synchronous text interface.
- On the standardized cases used, independent specialist and patient-actor raters often preferred its performance to that of the participating PCPs.
- The system showed promising diagnostic reasoning and communication under the information and time structure supplied by the protocol.
- A controlled comparison can identify strengths worth testing in more realistic workflows, such as asking clarifying questions or explaining a differential diagnosis.
What it does not establish
- It does not establish that AMIE is generally more accurate than PCPs in routine care.
- It does not show that AMIE can replace a clinician, make unsupervised decisions, or safely manage emergencies.
- It does not measure performance when a physical examination, vital signs, longitudinal history, medication list, laboratory data, imaging, or an electronic health record is required.
- It does not show how the system performs with unselected patients, language barriers, low health literacy, disabilities, rare presentations, or incomplete and contradictory information.
- It does not settle questions about fairness, equity, privacy, security, robustness to adversarial or misleading input, liability, or integration into local clinical governance.
The Nature abstract explicitly says the text-chat interface was unfamiliar to the physicians and that further work is needed before translating the findings to real-world care. The authors write: “Our research has several limitations and should be interpreted with caution.” Google Research authors Alan Karthikesalingam and Vivek Natarajan used the same caution in their January 12, 2024 post, updated October 6, 2026. In both cases, the statement is an author warning about a study, not an endorsement from a regulator.
What the later clinic feasibility study adds
A subsequent study moved one step closer to practice but answered a different question. Google and Beth Israel Deaconess Medical Center (BIDMC) described a prospective, single-center feasibility study in an academic urgent-care workflow. Adults with new, non-emergency episodic complaints completed an AMIE interaction before their visit. A physician supervised the session, could intervene under preset criteria, and received a transcript and summary for the appointment.
| Feature | Main simulated evaluation | Later feasibility workflow |
|---|---|---|
| People interacting with the system | Trained patient actors | Adults with new, non-emergency episodic complaints |
| Clinical setting | Standardized OSCE-style cases in text chat | Single academic urgent-care center, before a clinician visit |
| Oversight | Study evaluators rated the conversations | Physician monitored sessions and could stop or intervene |
| Sample reported by Google | 159 scenarios and 20 PCPs | 100 adults completed the interaction; 98 attended their appointments |
| Primary outcomes | Diagnostic accuracy and ratings across specialist and patient-actor axes | Feasibility, safety monitoring, differential diagnosis, and management-plan quality |
| System context | Text only | No EHR access or physical examination; transcript and summary sent to the PCP |
What happened in the supervised workflow
Google’s account says all 100 enrolled adults completed the AMIE interaction and 98 attended the subsequent visit. No safety stops were required during the monitored sessions. That is a useful feasibility signal: the protocol could be run with this sample and did not trigger its predefined stop criteria.

Blinded evaluators rated AMIE and PCPs similarly for overall differential-diagnosis quality and management-plan quality. PCPs scored higher, however, on the practicality and cost-effectiveness of the plans. A plan can be medically plausible yet difficult to carry out, too resource-intensive, poorly matched to a patient’s circumstances, or inefficient for a clinic. The feasibility study therefore points to a gap between generating a reasonable plan and delivering one that works in a real service.
Why this is not a head-to-head victory for either side
The two studies should not be combined as if they were one trial. The first compared AMIE with PCPs in standardized actor cases. The second was a supervised, single-arm workflow study in one center; its purpose was to see whether the process could operate around a visit, not to provide a definitive randomized comparison of patient outcomes.

The later study was also narrow in scope. It covered new, non-emergency episodic complaints and did not represent chronic disease management, preventive care, mental-health crises, pediatrics, pregnancy, complex multimorbidity, or emergency presentations. Its authors identify the absence of EHR access and physical examination as important constraints. Those omissions limit what AMIE could know before the clinician saw the patient and make the results unsuitable for claims about comprehensive primary care.

Why physical examination and clinical context matter
Primary care is not only a conversation. A clinician may detect an abnormal heart rhythm, weakness, respiratory distress, a changing skin lesion, dehydration, or a sign that changes urgency. The clinician can reconcile a medication list, inspect prior results, recognize a patient’s baseline, and account for social circumstances that affect whether a recommendation is realistic.
AMIE’s text interaction can collect information that a patient types, but it cannot independently verify those observations through touch, auscultation, measurement, or direct visual assessment in the studies described here. It also lacked EHR access in the later feasibility workflow. A transcript and summary may help a PCP prepare, yet they are not a substitute for the clinician’s examination and record review.
How to read the numbers without overclaiming
- Name the setting. Say that the result came from synchronous text chats and standardized cases with trained actors before citing the 30-of-32 or 25-of-26 figures.
- Identify the raters. Specialist physicians and patient actors scored different dimensions; the figures are not a single universal accuracy percentage.
- Keep the versions separate. Use the Nature abstract’s 159-case results for the published headline and label the 2024 Google post’s 149-case numbers as an earlier report.
- Separate feasibility from effectiveness. The 100-person supervised workflow shows that the process was feasible in one center; it does not prove better health outcomes, lower costs, or safe deployment at scale.
- Ask what information was available. Text-only interaction, no EHR, and no physical examination are material boundaries on generalization.
- Look for the missing populations and tasks. A system tested on selected non-emergency complaints has not thereby been tested on the full range of primary-care work.
What would be needed before clinical adoption
A convincing case for routine use would require evidence beyond favorable ratings in simulated dialogue. Future evaluations would need representative patients and settings, clinically meaningful outcomes, and comparisons that account for the tools available to physicians rather than disadvantaging them with an unfamiliar interface.
- Prospective, multi-site studies across primary-care practices and health systems.
- Clear inclusion of chronic disease, prevention, pediatrics, older adults, mental health, urgent symptoms, and patients with multiple conditions.
- Access to the same relevant information for the AI and clinician, including records, medications, laboratory results, and appropriate measurements.
- Independent monitoring for missed diagnoses, unsafe recommendations, inequitable performance, privacy incidents, and automation bias.
- Protocols defining when a human must review, override, or take over, with auditable logs of those decisions.
- Measures that include patient outcomes, time to appropriate care, workload, practicality, cost, patient trust, and clinician acceptance—not only conversational ratings.
Until those questions are answered, the strongest defensible description is that AMIE is a promising conversational diagnostic system demonstrated in controlled and supervised settings. Its performance merits further testing; it does not justify treating an experimental model as an autonomous primary-care clinician.
Bottom line for patients, clinicians, and technology readers
Google’s AMIE study showed impressive performance on the task it actually tested: text-based diagnostic conversations with trained actors in standardized cases. The peer-reviewed report involved 159 scenarios and 20 PCPs, with AMIE rated higher on 30 of 32 specialist axes and 25 of 26 patient-actor axes. A later supervised study showed that 100 adults could complete a pre-visit interaction and that 98 attended their visits, with no safety stops in that monitored sample. It also found physicians’ plans were judged more practical and cost-effective.
Those findings support continued evaluation of AI as a supervised aid around primary care. They do not demonstrate replacement of doctors, general superiority in ordinary practice, or readiness for unsupervised clinical use.
