For adults with generalized anxiety disorder (GAD), the main alternatives to cannabis are structured psychological treatments—especially cognitive behavioral therapy (CBT)—and, when clinically appropriate, prescription medicines such as selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs). Acceptance and commitment therapy (ACT), applied relaxation, CBT-based self-help, mindfulness, exercise, sleep support and reducing caffeine can also have a role.

“Anxiety” is not one diagnosis. The clearest treatment pathway below comes from guidance for adults with GAD, so it should not be treated as a universal plan for panic disorder, social anxiety, trauma-related symptoms, obsessive-compulsive disorder, a medical condition or substance-related anxiety. A clinician can assess the pattern, severity, functional impact, other conditions and your preferences before recommending a route.

What to consider instead of cannabis

Cannabis is not established here as a first-line treatment for anxiety, and no direct cannabis-versus-treatment comparison is provided. Instead, it describes options used in clinical care and supportive habits that may reduce symptoms. The practical choice depends on diagnosis, how much anxiety disrupts life, access to a trained practitioner, medical history, other medicines, likely side effects, time commitment and personal preference.

Option Typical role in adult GAD guidance What to know before choosing
CBT Leading, well-studied psychotherapy; high-intensity treatment is delivered by a trained practitioner. Requires regular participation and practice; individualized therapy is different from a workbook or app.
ACT Psychotherapy using acceptance, meaningful activity, mindfulness strategies and goal setting. Research is growing; it has not been established as superior to CBT.
Applied relaxation High-intensity psychological option listed by NICE alongside CBT. Structured skills training may suit someone who prefers relaxation-based work.
CBT-based self-help or psychoeducation Lower-intensity step after education and active monitoring when GAD persists. NICE says materials should be worked through systematically for at least six weeks; this is not equivalent to individualized therapy for everyone.
SSRIs or SNRIs Clinician-guided medicines that can reduce GAD symptoms. Benefits can take several weeks; side effects, interactions and discontinuation effects require planning.
Mindfulness or meditation Supportive option with small-to-moderate benefits in some reviews. Evidence for diagnosed anxiety disorders has limitations; one trial does not prove universal equivalence to medication.
Exercise, sleep and less caffeine Supportive habits that can accompany standard care. They may help symptoms but are not guaranteed stand-alone treatments or reasons to delay needed care.

Start with an assessment, not a product

NICE recommends assessing more than a symptom count. A useful evaluation considers the level and duration of anxiety, distress and functional impairment, and whether depression, another anxiety disorder, substance misuse, a medical condition, a relevant mental-health history or previous treatment response changes the plan. These details can alter both the diagnosis and the safest treatment.

Bring a concise record to an appointment: when symptoms occur, situations that trigger them, sleep, caffeine and other substances, physical symptoms, work or study effects, current medicines and what you have already tried. Ask which diagnosis is being considered, what improvement would look like, how progress will be reviewed and what to do if the first option is not tolerable or effective.

NIMH emphasizes that treatment should reflect your needs, preferences and medical situation in consultation with a mental-health or health-care professional. Finding a workable plan can involve trial and error, and both psychotherapy and medication can take time.

How NICE’s stepped-care pathway works for adult GAD

NICE describes a progression so that treatment intensity matches impairment and response. The steps below concern adults with diagnosed GAD; they are not a universal sequence for every anxiety condition.

1. Education and active monitoring

At the beginning, a professional explains GAD, available treatments and how symptoms affect functioning. Active monitoring means arranging follow-up rather than simply being told to wait. This can be appropriate when symptoms are less impairing or when diagnosis and preferences are still being clarified.

2. CBT-principle self-help or psychoeducation for persistent symptoms

If diagnosed GAD does not improve, NICE includes individual non-facilitated or guided self-help and psychoeducational groups. Materials should be based on CBT principles and used systematically for at least six weeks. A workbook or electronic program is a structured practice tool, not proof of diagnosis and not a substitute for assessment when symptoms are severe, complex or disabling.

3. High-intensity psychological treatment or medication

For marked impairment or an inadequate response to lower-intensity care, NICE offers either high-intensity CBT or applied relaxation, or drug treatment. High-intensity psychological interventions are delivered by trained practitioners, usually in a weekly course. The recommendation states: “Base the choice of treatment on the person’s preference as there is no evidence that either mode of treatment (individual high-intensity psychological intervention or drug treatment) is better.”

Psychotherapy alternatives

Cognitive behavioral therapy (CBT)

NIMH describes CBT as a research-supported, commonly used psychotherapy for GAD and notes that it is well studied. CBT generally links anxious thoughts, physical sensations, behaviors and avoidance, then uses planned exercises to change unhelpful patterns and build coping skills. In NICE’s high-intensity pathway, treatment is delivered by a trained practitioner in a regular course, often weekly.

CBT can be a good fit if you want a structured treatment with between-session practice and measurable goals. Discuss the format—individual sessions, group work, guided self-help or another delivery method—because the amount of practitioner support differs. Do not assume that a retail book, online program or brief article provides the same assessment and tailoring as therapist-delivered CBT.

Acceptance and commitment therapy (ACT)

NIMH lists ACT as another option for GAD. It uses acceptance, mindfulness strategies, meaningful activity and goal setting to help a person respond differently to difficult thoughts and feelings while moving toward valued activities. NIMH notes a growing body of research, but the evidence does not establish ACT as better than CBT. It may appeal if your goals include reducing avoidance and reconnecting with important activities rather than trying to eliminate every anxious thought.

Applied relaxation

NICE lists applied relaxation alongside high-intensity CBT for adult GAD. It is a structured psychological intervention centered on learning and applying relaxation skills. Ask a qualified provider how the method is taught, how often practice is expected and how progress will be assessed. It is not simply an occasional relaxation exercise chosen at random.

CBT-based self-help and psychoeducation

Self-help can be a reasonable lower-intensity starting point when symptoms persist but do not yet require high-intensity treatment, or while you are arranging professional support. NICE specifies CBT-principle written or electronic materials and at least six weeks of systematic use. Guided self-help adds some professional support; non-facilitated self-help does not.

Choose material that clearly explains its CBT basis, gives a sequence of exercises and states how to obtain help if symptoms worsen. Treat a workbook as a tool for structured practice, not as a diagnosis, a promise of recovery or a replacement for individualized care. If you have substantial impairment, multiple conditions or difficulty functioning, ask whether a higher level of support is more appropriate.

Medication options: SSRIs and SNRIs

NIMH identifies SSRIs and SNRIs as commonly used antidepressant classes that can also help GAD symptoms. NICE includes medication as an option when treatment intensity is warranted. The choice belongs with the prescribing clinician, who can consider your medical history, other medicines, previous response, side-effect concerns and local labeling.

What to expect about timing

These medicines do not necessarily relieve anxiety immediately. NIMH notes that medication can take time to work; NIMH notes that the onset can potentially take several weeks. Your prescriber should explain what early effects to watch for, when a review is planned and what improvement or lack of improvement would prompt a change.

Side effects, interactions and stopping

Medication decisions involve possible adverse effects, interactions and withdrawal or discontinuation effects. Do not start, stop, combine or change a dose without guidance from the prescriber. Ask how a medicine interacts with every prescription, over-the-counter product and supplement you use, and what tapering or follow-up would be needed if it is discontinued.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Choosing medication or high-intensity therapy

NICE does not identify either high-intensity psychological treatment or drug treatment as generally better for GAD. Preference therefore matters alongside clinical factors. Consider whether you prefer regular skills practice with a therapist, a medicine routine, or a discussion of both; whether you can attend sessions; how quickly you need support; and which risks or burdens you are most concerned about.

Mindfulness and meditation: useful, but not a universal substitute

The National Center for Complementary and Integrative Health (NCCIH) reports small-to-moderate benefits in some reviews of mindfulness and meditation, while also noting limits in the evidence for diagnosed anxiety disorders. Results vary by intervention, population and comparison treatment.

NCCIH summarizes a 2023 randomized trial involving 208 people with anxiety disorders. In that study, mindfulness-based stress reduction (MBSR) was noninferior to escitalopram. “Noninferior” means the trial did not find MBSR worse than escitalopram within its design and margin; it does not establish that every meditation program works as well as medication for every person, diagnosis or severity level. The result also does not remove the need for assessment or follow-up.

If you want to try mindfulness, discuss it as supportive care or as one treatment option. Ask what program is being offered, how long it lasts, how practice is supervised and how progress will be judged. Continue appropriate clinical care rather than using one study to justify replacing a treatment that is helping.

Lifestyle supports that can accompany treatment

Exercise

NHS guidance says regular exercise can reduce anxiety and stress and help sleep. Choose an activity that is realistic for your health and schedule, and build consistency rather than treating one workout as a cure. If a medical condition limits exercise, ask a health professional what level is appropriate.

Sleep

NIMH says getting enough sleep may reduce symptoms when paired with standard care. Focus on a sustainable sleep routine and mention persistent insomnia to a clinician, especially if it worsens daytime functioning or anxiety.

Reducing caffeine

NIMH identifies reducing caffeine as a lifestyle practice that may reduce symptoms alongside standard treatment. Track whether coffee, tea, energy drinks or other caffeinated products coincide with physical sensations such as restlessness or a racing heart. Change consumption cautiously and discuss it with a professional if you use caffeine to manage work, medication effects or another health issue.

Why habits are supportive rather than curative

NIMH’s official guidance states: “Practicing a healthy lifestyle also can help combat anxiety, although this alone cannot replace treatment.” Exercise, sleep and caffeine changes can make therapy or medication easier to use, but they should not delay care when anxiety causes marked impairment or does not improve.

Over-the-counter remedies and supplements

“Natural” does not mean proven or interaction-free. NICE advises discussing over-the-counter medicines and preparations because evidence supporting their safe use is lacking and interactions are possible. Before taking a supplement, herbal product, cannabinoid preparation or nonprescription medicine, show the complete ingredient list to a health professional and ask how it fits with prescribed treatment.

This guidance does not establish a particular supplement, dose or cannabinoid product as an anxiety treatment. Local labeling and clinical guidance differ by country, so a product sold without a prescription still deserves a medication-safety check.

How to choose among the options

Use these questions with a qualified professional rather than trying to rank treatments in the abstract:

  • What is the likely diagnosis? GAD guidance may not apply to panic attacks, social anxiety, trauma-related symptoms, obsessive-compulsive symptoms or anxiety caused by a medical or substance-related problem.
  • How much is functioning affected? Distress, duration and impact on work, study, relationships and daily tasks help determine treatment intensity.
  • What can you realistically access? Consider trained-therapist availability, session schedules, group versus individual care, guided versus non-facilitated self-help and follow-up arrangements.
  • What do you prefer? NICE specifically recommends using the person’s preference when choosing between high-intensity psychological treatment and medication because neither is proven better overall for GAD.
  • What medical factors matter? Review other diagnoses, current medicines, substance use, previous treatment response, side-effect concerns and potential interactions.
  • How will success be measured? Agree on functional goals and a review date, not just a vague intention to feel better.

When the first plan does not help

Do not conclude that all treatment is ineffective after one disappointing attempt. NIMH notes that finding the best plan may take trial and error. Tell the clinician what happened: whether symptoms changed, whether side effects were manageable, whether exercises were practical, whether access was a barrier and whether another condition may be contributing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A review may lead to more time for an intervention to work, a different intensity, a different therapy approach, a medication change or assessment of depression, another anxiety disorder, substance misuse or a medical condition. Do not make abrupt medication changes or add supplements to compensate without professional advice.

A practical first-week plan

  1. Write a symptom snapshot. Record the main worries, physical symptoms, sleep, caffeine, triggers and the tasks anxiety is disrupting.
  2. List everything you take. Include prescriptions, over-the-counter products, supplements and cannabis or other substances so interactions and patterns can be discussed accurately.
  3. Book an assessment. Ask specifically whether the presentation fits GAD or another condition and what level of care is appropriate.
  4. Choose one structured next step. Depending on the assessment, that could be active monitoring, a CBT-based self-help course, psychoeducation, high-intensity CBT, applied relaxation, ACT, medication or a discussion of mindfulness.
  5. Set a review point. Agree how and when to evaluate symptoms, functioning, side effects and adherence. Bring your notes rather than relying on memory.
  6. Add supportive habits. Use realistic exercise, adequate sleep and reduced caffeine alongside—not instead of—the selected treatment.

Bottom line for readers considering an alternative to cannabis

For adult GAD, CBT is a leading, well-studied option; ACT and applied relaxation offer other psychotherapy routes; CBT-based self-help is a lower-intensity pathway; and SSRIs or SNRIs are clinician-guided medication choices. Mindfulness, exercise, sleep and less caffeine may support recovery, while over-the-counter products require interaction and evidence checks. Because anxiety has multiple causes and diagnoses, the safest alternative is the one selected through an assessment that weighs impairment, medical factors, preferences, access and follow-up.
