Vagus nerve stimulation (VNS) and spaced repetition are not established alternatives for learning a general skill. The FDA-authorized VNS system is used with rehabilitation for a specific group of people recovering from chronic ischemic stroke; the available sources do not establish how spaced repetition works across skills or compare it directly with VNS. They target different questions, so this evidence cannot identify a winner.

Why these approaches are not a like-for-like comparison

VNS is a form of neuromodulation: in the authorized stroke-rehabilitation system, stimulation of the vagus nerve is paired with prescribed physical therapy. The target is upper-limb motor impairment after chronic ischemic stroke, not learning in healthy people or acquiring an arbitrary skill. The FDA’s approval announcement and PMA record describe that indication.

Spaced repetition means revisiting material or practicing a skill across multiple sessions separated in time, rather than concentrating all practice into one session. That description explains the learning strategy; it is not evidence here about an optimal interval, how much it improves retention, or whether it transfers across different cognitive and motor skills. The cited sources do not provide a sufficient evidence base for those claims or a direct VNS-versus-spacing trial.

So the useful distinction is one of scope: paired VNS is a medically supervised rehabilitation intervention for a defined clinical population, while spaced repetition is a practice-scheduling concept. Someone choosing a study routine is not choosing between two proven treatments.

What the FDA-described VNS study tested

The FDA approved the MicroTransponder Vivistim Paired VNS System on August 27, 2021, for use during rehabilitation therapy to reduce upper-extremity motor deficits and improve motor function in people with chronic ischemic stroke and moderate-to-severe arm impairment. The approval announcement summarizes a study of 108 patients at 19 U.S. and U.K. sites.

Both groups completed substantial rehabilitation: 300–400 physical therapy exercises in each 90-minute session, three times weekly for six weeks. The treatment group received VNS during rehabilitation; the control group received very low-level stimulation for the first five exercises and none for the remainder. Average FMA-UE scores rose by 5 points in the VNS group and 2.4 points in the control group. At 90 days, 47.2% of the VNS group and 23.6% of the control group had improved by at least six FMA-UE points, according to the FDA’s 2021 announcement.

These results describe VNS added to intensive, repeated stroke rehabilitation. They do not isolate stimulation from practice, show that VNS teaches skills in general, or compare it with spaced repetition. FMA-UE is a measure of upper-extremity motor function, not a general measure of learning.

How later reviews qualify the benefit

Reviews have not produced one interchangeable estimate: they include different sets of studies and participants, and assess outcomes and time points differently.

Review Scope and reported finding How to interpret it
Gao et al., 2023 Seven randomized controlled trials and 263 analyzed participants. The review reported a medium post-intervention motor-function effect (g=0.432; 95% CI 0.186–0.678) and a larger effect at follow-up (g=0.840; 95% CI 0.288–1.392). The review found no advantage over rehabilitation for activities of daily living or mental health. See the Journal of Neurology, Neurosurgery & Psychiatry record.
Cochrane, 2026 Ten studies with 547 participants, covering invasive and non-invasive VNS paired with motor rehabilitation. For short-term upper-limb motor function, the estimate was SMD 1.22 (95% CI 0.68–1.77; 10 studies, 499 participants). Cochrane judged the evidence for short-term motor function, activity and quality of life very uncertain. It cited high risk of bias in most studies, small samples and limited long-term follow-up. The review record is available at PubMed.
Implanted-VNS review, 2025 Eight studies and 498 patients; pooled FMA-UE mean difference was 2.73 points (95% CI 1.32–4.13). This review focused on implanted VNS paired with rehabilitation and noted that only one included study reported a serious adverse event. Its scope differs from Cochrane’s, so the estimates should not be treated as interchangeable. See the PubMed record.

The 2026 Cochrane review also reported low-certainty evidence that VNS may result in little to no increased risk of serious adverse events (RR 2.38, 95% CI 0.77–7.30; 8 studies, 416 participants). That wide interval and the review’s stated certainty matter: the estimate is not a guarantee of safety or proof that harms are absent.

What is known about VNS safety and use

The FDA lists adverse events including dysphonia, bruising, falls, hoarseness, pain, headache, dizziness, throat irritation and fatigue. The Vivistim system requires implantation and physician-directed programming; it is not a consumer learning gadget or a device for self-stimulation.

The FDA says the system is not approved for uses beyond its stated chronic ischemic stroke rehabilitation indication. It should not be used in patients with vagotomy, and the agency advises discussion of relevant medical history, including cardiac abnormalities, respiratory disorders and pre-existing hoarseness. The FDA’s announcement and PMA record contain the device and indication details. FDA approval records can be supplemented after the original decision, so clinicians and patients should check current labeling rather than assume the 2021 record captures every later update.

How to choose what is relevant to your goal

  • If you are recovering arm function after ischemic stroke: discuss rehabilitation options with your stroke-care or rehabilitation team. Paired VNS is a clinical intervention with a defined indication and device-specific risks; eligibility and suitability require medical assessment.
  • If you are learning a language, instrument, procedure or other skill: VNS evidence summarized here does not support using stimulation as a learning aid. Spaced practice may be a way to organize practice sessions, but these sources do not establish an optimal schedule or its comparative effectiveness for your skill.
  • If you are comparing outcomes: distinguish motor impairment measures such as FMA-UE from retention or proficiency in a specific skill. A valid comparison would need the same population, target outcome, practice conditions and follow-up period; the cited evidence does not supply that comparison.
