Utah has authorized Nolla Health to use an AI system to issue initial prescriptions and refills for certain topical acne treatments to eligible adults in the state. It is a limited, time-bound pilot—not a general approval of AI doctors—and every prescription must receive physician review before it reaches a pharmacy in the initial stage.
What Utah authorized
Utah’s Office of Artificial Intelligence Policy and Division of Professional Licensing signed a regulatory agreement with Magic Health, Inc. and Magic Health Services, LLC, which do business as Nolla Health. It covers the company’s Nolla Skin App and permits it to issue first-time topical acne prescriptions and renewals for eligible Utah adults, then transmit prescriptions to Utah-licensed pharmacists.
The state’s Authorized AI Pilots listing gives the pilot an active term of October 5, 2026, through October 5, 2027. The agreement says its term begins when the technology is made available to customers and Nolla gives the state written notice. The authorization applies only to the named service, participants, and requirements in the agreement. It is neither a nationwide permission nor a state endorsement of Nolla’s use of AI.
Who can use it, and what can it prescribe?
Eligibility and assessment
The pilot is for Utah residents aged 18 or older with mild-to-moderate acne. Nolla says the app uses a medical intake and a five-angle facial scan, then applies constrained treatment protocols. The agreement’s attached proposal describes an assessment based on structured intake information and standardized facial images. Nolla also says users can message a licensed physician; these are company-described service features, not independent validation of the system.

Topical-only treatment scope
The agreement’s formulary includes topical tretinoin, adapalene, clindamycin with benzoyl peroxide, benzoyl peroxide, and azelaic acid options. The AI cannot prescribe outside that formulary. Depending on what is offered, a patient may choose a non-compounded option through a local pharmacy or a compounded product for delivery. Some options have retail prescription or over-the-counter equivalents, while some combinations are available only through compounding.
Severe or nodulocystic acne—and cases that may need oral, systemic, or isotretinoin treatment—are outside the AI prescribing scope. The proposal also identifies pregnancy, trying to become pregnant, breastfeeding, certain medical conditions, prior adverse reactions to formulary drugs, and delivery outside Utah as exclusions or hard stops. Depending on the situation, the system must route the patient to a physician or direct them to in-person care.
How much physician oversight is required?
The agreement sets out three stages. Moving to a less intensive review model is not automatic: each transition requires written state approval and specified performance gates, and the state can pause or reverse progression.

| Stage | Prescription review | What must happen to move beyond it |
|---|---|---|
| Initial | Two Utah-licensed physicians review every prescription before it reaches a pharmacy. | Written state approval and the agreement’s performance gates are required before a transition. |
| Later | Prescriptions can be submitted directly, with a physician reviewing all cases retrospectively at least weekly. | Written state approval and performance gates are required before moving to monthly sampling. |
| Further | Physicians review a monthly retrospective sample of at least 10% of prescriptions; all escalations and adverse-event cases are reviewed. | The agreement does not make this stage automatic or remove the state’s power to pause or reverse progression. |
At launch, therefore, the AI does not send prescriptions to pharmacies without a clinician first reviewing them. Nolla’s overseeing physician, Dr. Zaid Fadul, described the system in the company’s October 5, 2026, launch announcement this way: “The AI isn’t making freeform decisions. It’s selecting from pre-approved protocols reviewed by physicians, and clinicians are built into every stage.” That is the company’s characterization of its system.
What safeguards and reporting does the agreement require?
- Users must be told that they are interacting with generative AI, and pharmacies must be told that a prescription was generated by AI.
- Adverse events must be reported within 24 hours. Monthly reporting must include clinician agreement rates.
- Nolla must maintain professional liability insurance and is restricted from selling or transferring patient data.
These are agreement requirements; their existence does not establish that the system is clinically effective or safe for every patient.

Nolla reported roughly 96% clinician agreement in prior cases handled through its app, with the remaining differences described as minor strength adjustments. That is a company-reported figure, not an independently validated outcome for Utah pilot patients.

The agreement’s stage gate sets a target of at least 95% clinician agreement, along with zero missed hard stops after quality-assurance review and zero serious adverse events. Those are thresholds for progression, not results already demonstrated by the Utah pilot. The sources available for the pilot do not provide independently validated patient outcome statistics.
What it may cost
Nolla advertised a $4.99-per-month pilot subscription in its 2026 launch announcement. Medication may cost extra, and the price varies by fulfillment route; the subscription figure alone does not establish a patient’s total cost.
Why the “AI doctor” headline needs a qualifier
This is a specific state regulatory mitigation agreement for one company’s topical acne service, with defined patient and medication limits and physician oversight that can change only after state approval. It should not be read as Utah approving AI doctors generally, as permission for the system to treat every form of acne, or as proof of federal approval or endorsement. Independent coverage has also distinguished the initial-prescription authorization from AI-assisted refill services; Nolla’s claim to be “first” is the company’s claim.
