If a system cannot be patched promptly, treat it as a managed security exception—not as a problem solved by leaving it alone or putting it behind a firewall. Identify what it does, assess how it can be reached and what failure would mean, restrict unnecessary access, monitor it, and set a review date for support or replacement. Isolation can reduce exposure; it does not remove the vulnerability.
What makes a system “unpatchable”?
The term can describe different situations: a vendor no longer supports a component, a patch is not yet available, testing or approval will take time, or applying an update could disrupt an uptime-sensitive or safety-critical process. These are not interchangeable. A temporary delay may call for a short-term mitigation and a firm patch deadline; an unsupported system may require a replacement plan or an alternative source of support.
Record why patching cannot happen, who owns the exception, and what event will trigger a change in the decision. That makes the risk visible to the people responsible for the system and prevents “we cannot patch it” from becoming an indefinite status.

Build an inventory that supports action
For each affected asset, track its hardware, software and firmware; owner; vendor support status; business function; dependencies; network exposure; known vulnerabilities; and operational or safety impact if it is changed or unavailable. NIST’s SP 1800-31 patch-management guidance emphasizes identifying assets, prioritizing remediation and tracking implementation. An inventory that omits ownership, dependencies or exposure is unlikely to help responders make a safe decision.

Prioritize risk, not just a score or a count
Consider whether exploitation is known, whether the vulnerable function is reachable, the potential impact on the organization, and what controls already limit access or consequences. A severe flaw on an exposed system may need urgent action; a flaw that is not reachable through enabled paths may have a different immediate priority, while still requiring tracking. Neither a vulnerability score nor a growing CVE count, on its own, tells you which asset creates the greatest risk to your organization.
Google Threat Intelligence Group (GTIG), in an analysis covering January 2025 through August 31, 2026, reported that monthly disclosed vulnerabilities rose from 5,045 in January 2026 to 10,740 in August 2026. GTIG also reported that the average number of vulnerabilities exploited per month rose from 10.5 in 2025 to 18 between January and August 2026, while exploited vulnerabilities remained a small share of all disclosures. The group cautioned that disclosure totals can be affected by vulnerability-numbering policies and concentrated vendor release cycles. These are GTIG’s measurements and interpretation, not a forecast of the volume or risk any one organization will face.

What can you do while a fix is unavailable or delayed?
Reduce reachable paths
Separate assets that cannot be patched promptly from the routine patch queue so their exceptions remain visible. Limit inbound and outbound communication to what the system’s business function requires. Where appropriate, isolate it from less trusted networks, restrict which users and systems can connect, and disable vulnerable functionality if doing so is feasible without creating a greater operational or safety risk.

Its guide states, “Isolation is a form of mitigation that can be highly effective at stopping threats against vulnerable devices,” while also advising that isolation be undone when appropriate. Isolation blocks or narrows paths an attacker could use; it does not fix the flaw or prove that the asset is safe.

Keep essential workflows available
Before changing network access or disabling a feature, document which approved workflows must continue and which dependencies they require. Define how authorized access will work during isolation, who can approve a change, and how to restore normal access in a controlled way. A hardware firewall appliance may be one way to enforce a network boundary, depending on the architecture, throughput needs, access paths and available management skills. The device alone is not a security plan: rules must reflect the actual workflows, and someone must maintain and verify them.
Monitor the remaining exposure
Choose monitoring that fits the asset and the controls in place. Watch for unexpected connections, use of restricted functions, configuration changes and other activity relevant to the threat and environment. Define who reviews alerts and what actions follow. If you disable a vulnerable function as an emergency mitigation, record the change and the recovery procedure; NIST’s patching guidance calls for being able to revert such mitigation after an approved patch is released.

Which response is appropriate for the situation?

The NIST publications have different scopes and applicability. SP 1800-31 is guidance for general IT patching and does not cover the special patching challenges of legacy IT, industrial control systems, IoT or other operational technology. SP 800-171 Revision 3 applies where the organization’s obligations and the relevant version make it applicable; it is not a universal requirement for every system.
What does AI-driven vulnerability activity change?
AI-assisted research may increase the capacity to find vulnerabilities or develop exploits, which can add pressure to verification, coordinated disclosure and remediation. But the available figures do not establish a universal increase in exploitation risk for every organization, and disclosure volume is not the same as active exploitation.

Anthropic’s May 22, 2026 Project Glasswing update reported more than 10,000 high- or critical-severity findings from partner work using Mythos Preview. That is a vendor’s report about a particular initiative, not an independent census of AI-discovered vulnerabilities. In the update, Anthropic said, “Now it’s limited by how quickly we can verify, disclose, and patch the large numbers of vulnerabilities found by AI.” That is the initiative’s own description of its challenge, not a general measurement of how quickly organizations can remediate flaws.
GTIG’s May 12, 2026 AI threat-tracker report described one case in which it assessed that a threat actor used a zero-day exploit it believed was AI-developed. That specific intelligence assessment does not establish how common AI-developed exploits are. For defenders, the practical implication is to keep vulnerability intake, verification, escalation and mitigation processes ready; it is not to treat every new disclosure as proof of imminent exploitation.

How should you govern the exception and plan its end?
- Assign accountability. Name the technical owner and the business owner who accept and review the exception, and document why a prompt patch is not viable.
- Record the risk and controls. Capture the affected asset and vulnerability, its exposure and business impact, known exploitation evidence, compensating controls, and any operational constraints.
- Set a review trigger and date. Reassess when exploitation evidence changes, the vendor releases a fix, the system’s exposure changes, or a planned replacement milestone arrives. Set a regular review interval appropriate to the risk rather than letting the exception renew automatically.
- Track patch or mitigation effectiveness. Record what was deployed or changed, verify its status, and keep the evidence and recovery steps available to the people responsible for the system.
- Plan replacement or continued support. When vendor support ends, NIST SP 800-171 Revision 3 says to replace the component; where replacement cannot be done, it calls for risk mitigation or alternative support. Turn that into an owned plan with dependencies and milestones, rather than assuming an isolation rule is a permanent substitute.
- Use coordinated disclosure when a flaw is being reported. NIST SP 800-216 recommends formal processes for federal systems to receive, assess, manage and communicate vulnerability reports. Organizations handling a report should establish who receives it, who assesses impact, how remediation is coordinated, and how communications are managed.
