Choose Frost if you want an icy, weapon-focused combat identity built around runic power and strikes. Choose Unholy if infection, death magic, and undead minions sound more appealing. Both are melee damage specializations; neither is a universal performance winner. The better choice depends on the activity, current build, and patch.
How Frost and Unholy feel different
Death Knights fight at close range and use runes and Runic Power. Blood is the class’s tank specialization; Frost and Unholy are its damage specializations. Blizzard’s class descriptions distinguish the two damage specs by their combat themes.
Frost: icy weapon strikes
Frost channels runic power into icy attacks and weapon strikes. It is the natural fit if you want the fantasy of striking enemies with frost-charged weapons.
Unholy: infection and minions
Unholy focuses on infection and death magic, with undead minions as part of its identity. Choose it if spreading disease and commanding the undead are central to what you want from a Death Knight.

For raids, Mythic+, PvP, or solo play, consider the specific build and current balance rather than assuming one specialization wins everywhere. The sources cited here do not settle which performs best in any of those activities.
Why performance advice needs a patch date
Balance changes can alter a damage comparison. Blizzard’s October 1, 2026 hotfix record includes Frost damage adjustments and Unholy-specific changes. That is evidence of recent tuning, not proof that either specialization is currently stronger: the official hotfix page’s surfaced locale versions differed in the abilities listed, so exact percentages and a definitive ranking cannot be established here. Season bonuses and builds can change the comparison
Season-specific systems add another layer. Blizzard’s Midnight Season 1 class-set announcement lists different set bonuses for Frost and Unholy, while the pre-expansion update notes include specialization and hero-talent adjustments. Those details apply to particular game versions, not to the permanent identity of either specialization.
