AI mental-health advice can be convenient and sound empathetic, but fluent conversation is not proof of clinical competence. A consumer chatbot or wellness app may offer general information or a prompt to reflect; it should not be treated as a diagnostic authority, a replacement for a licensed clinician, or the only support in a crisis. Whether a particular tool is useful or risky depends on what it claims to do, how it was evaluated, how it handles emergencies and personal data, and whether it supports or displaces human care.

Why AI mental-health advice can feel helpful—and still be unsafe

A conversational tool is available on demand and can respond without the social friction some people feel when raising a concern with another person. It may help someone organize questions or find general information. Those conveniences do not establish that the system understands a person’s history, can assess their needs, or can respond reliably when the stakes are high.

The American Psychological Association (APA) cautions that many consumer-facing AI systems are not clinically validated and may lack expert input and ongoing monitoring. A model can use reassuring, therapy-like language while producing inaccurate or biased information. That is a reason to verify important claims—not proof that every answer or every product is wrong.

What kinds of tools are being discussed?

“AI mental-health app” is not one clinical or regulatory category. A general-purpose chatbot, a wellness app with an AI feature, a clinician-led service, and software intended to diagnose or treat a condition may have different purposes and safeguards. A clinical service is not automatically a regulated medical device, and a wellness label alone does not establish whether a product is safe or effective.

Tool type What to establish What not to assume
General-purpose conversational AI Whether it is intended only for general information, what evidence and expert involvement support its mental-health responses, and how it handles sensitive data and crisis situations. That a natural conversation means clinical assessment, diagnosis, or reliable crisis response.
Consumer wellness app with AI features Its specific intended use, evidence for the feature, privacy controls, and whether it offers human oversight or emergency escalation. That “wellness” means the app has been reviewed for medical use, or that all wellness apps have the same oversight.
Clinical service or medical-device software Who provides care, what role the software plays, what evidence supports its intended use, and what regulatory status applies in the relevant country. That a clinician’s involvement makes every software feature a medical device, or that device status alone answers every question about quality.

In the United States, the Food and Drug Administration (FDA) says oversight depends on a product’s intended use and whether it meets the definition of a medical device. Certain healthy-lifestyle software functions are excluded from device oversight. In its November 6, 2025 executive summary, the FDA reported that it had authorized more than 1,200 AI-enabled medical devices across categories, but none was then authorized for mental-health uses. That is a dated US figure, not a current count or a claim that every consumer wellness app requires FDA authorization.

A 2024 perspective in Nature Medicine describes a US regulatory “gray area”: many generative-AI wellness apps are not reviewed by health regulators even though users may bring serious mental-health concerns or crisis needs to them. The authors note that regulators elsewhere face similar challenges; this is not a complete account of other countries’ laws. Check the rules and product status that apply where you live.

Key risks to understand before relying on a chatbot

Confident answers can be inaccurate or biased

Systems trained on broad, unvetted internet material can repeat misinformation or bias, the APA warns. The FDA’s 2025 executive summary also identifies possible confabulation, biased or inappropriate content, failure to relay important medical information, and declining accuracy over time as challenges for generative-AI mental-health medical devices. These are recognized ways a system may fail; they do not establish that every product or response has those problems.

Pay particular attention when an answer makes a strong claim about a diagnosis, medication, risk, or what care to seek. A polished explanation is not evidence that the claim is correct. Verify consequential information with a health-care practitioner rather than acting on the chatbot’s confidence.

Questions and therapy language do not equal a clinical assessment

A clinical assessment can depend on a person’s history and context, including information that may not be conveyed in text or voice. A chatbot that asks follow-up questions has not necessarily gathered enough information to evaluate someone’s situation. Do not infer that it has conducted a clinical evaluation simply because it uses therapeutic language, reflects feelings, or suggests a label.

Crisis support is limited and unpredictable

The APA describes crisis management by consumer chatbots as limited and unpredictable. A chatbot is not a dependable emergency service, and relying on an app alone during an emergency can be dangerous. If someone may be in immediate danger, seek live help instead. In the United States, the APA’s guidance says to call or text 988, call 911, or go to the nearest emergency department. These numbers are US-specific; outside the United States, contact the local emergency service or crisis line.

Private-feeling conversations may involve sensitive data

People may disclose personal details because a chat feels private. The APA advises caution and recommends checking the particular service’s privacy policy and settings. Before sharing sensitive information, review what the service says about data handling and what controls it provides. Do not assume a consumer chatbot or wellness app has the same legal protections as clinical care.

Human support can be crowded out

A tool that becomes the sole place someone turns for support may displace conversations with trusted people or interfere with existing care. The APA advises consumers not to let chatbot use disrupt human relationships or care. The World Health Organization (WHO), in a March 20, 2026 update summarizing a January 2026 expert workshop, recommends treating generative-AI use as a public mental-health concern and including possible emotional dependence in impact assessments and monitoring. These are governance recommendations, not findings that every chatbot has caused harm.

How to judge a tool before using it

No single label settles whether a mental-health AI tool is appropriate. Look for clear answers to these questions; if important details are missing, do not fill the gaps with assumptions.

  • Purpose: Is the tool for general wellness or information, or does it claim to diagnose, assess, or treat a condition?
  • Evidence and expertise: Is there independent clinical evidence for the specific feature, and were qualified mental-health experts involved?
  • Human oversight: Does the service explain what happens when a user appears to need urgent help, and is a human available where appropriate?
  • Privacy: What does the service’s own policy say about personal information, and what settings or controls can you use?
  • Limits and status: Does it state its limitations plainly and explain any relevant regulatory status in your jurisdiction?
  • Role in care: Does it help you access reliable information or human support, or encourage you to rely on it instead?

Safer ways to use AI for mental-health information

  1. Keep the task general. You might use a chatbot to help organize questions for an appointment or explain a general term, but not to decide whether you have a condition or what treatment you need.
  2. Verify consequential claims. Check mental-health information with a health-care practitioner, as the APA advises. Do not use a chatbot to diagnose a condition or interpret psychological tests.
  3. Protect sensitive details. Read the specific service’s privacy policy and review its settings before sharing personal information.
  4. Keep people and care involved. Do not let the tool replace a clinician or interfere with existing care and human relationships.
  5. Use live help for urgent danger. Do not depend on a chatbot as your only crisis resource; use local emergency or crisis services.

What public-health guidance says about AI and mental health

WHO’s March 2025 guidance addresses large multimodal models in health—systems that can accept different kinds of input and generate varied outputs. A March 20, 2026 WHO update, summarizing recommendations from an expert workshop held in January 2026, calls for mental health to be included in AI impact assessments and monitoring, and for support tools to be co-designed with experts and people with lived experience, including young people. The update also identifies a need for agreement on crisis-referral frameworks and accountability. These recommendations describe governance priorities; they are not binding rules for every service.
