Indigenous health perspectives show that health is more than an individual’s physical, mental and social condition. In many Indigenous communities, wellbeing is relational and collective, grounded in land, culture, language, spirituality, family, community, environmental conditions and self-determination. That broader definition exposes important limits in health systems built mainly around clinical outcomes and individually delivered care.

Why the definition of health matters

Definitions determine what health systems measure, fund and treat as a priority. A clinic may record blood pressure, diagnoses, hospital admissions and mental-health symptoms while missing whether people can practise their culture, speak their language, access traditional foods, maintain relationships, feel safe in services or exercise control over decisions affecting their community.

Laura Gemmell-Sinnott, lead author of a study from the Kōhatu Centre for Hauora Māori, argues that the widely used definition of health has not changed in 80 years even as evidence about disease burden, structural determinants and self-determination has expanded. She says it also fails to reflect how Indigenous peoples define and view health, with serious consequences for equity.

This is not an argument that clinical medicine is unimportant. It is an argument that clinical care is only one part of a complete health system, and that the meaning of wellbeing must not be imposed without the participation and authority of the people being served.

What holistic Indigenous health means

The featured study examined perspectives from Aotearoa, Australia, Canada, the mainland United States and Hawai‘i. It identified five connected themes. They overlap, but each points to a different responsibility for health policy and practice.

Holistic wellbeing

Physical and mental health are interwoven with emotional, social, spiritual, cultural and environmental wellbeing. A person’s symptoms cannot always be separated from family circumstances, community safety, housing, food systems, cultural continuity or connection to place.

Cultural knowledge

Knowledge held by families, Elders and communities can shape understandings of illness, healing, prevention and responsibility. Treating Western clinical knowledge as the only legitimate authority can exclude useful practices and weaken trust.

Connection to land

Land is not merely a location where services are delivered. It can be a source of identity, nourishment, medicine, teaching, belonging and spiritual relationship. Damage to land or restricted access to Country can therefore affect health directly, not just indirectly through income or housing.

Relationships

Wellbeing is often understood through obligations and connections among people, family, community, ancestors, future generations and the natural world. A service designed only for an isolated individual may fail when decisions are properly understood as family or community matters.

Self-determination

Communities need meaningful authority over priorities, data, service design and evaluation. Self-determination is a health issue because control over decisions, resources and cultural life affects safety, dignity and the ability to respond to local needs.

How these perspectives differ from conventional Western frameworks

“Western health framework” describes a dominant pattern, not every clinician or institution. Many Western services already recognise social determinants and patient-centred care. The structural difference is what counts as the unit of wellbeing, who defines priorities and which outcomes are treated as valid.

Dimension Common conventional emphasis Indigenous-informed emphasis
Unit of wellbeing Individual patient Individual together with family, community and collective rights
Determinants Clinical factors and conventional social variables Land, culture, language, spirituality, environment, sovereignty and relationships alongside clinical factors
Authority Provider, institution or state-defined priorities Indigenous self-determination, community governance and shared decision-making
Measurement Standardised clinical outcomes Culturally defined indicators selected by the community, alongside clinical outcomes
Service model Conventional services delivered by professionals Culturally safe, intercultural care that may include traditional practices and land-based activity

A system can meet technical standards yet remain unacceptable if people encounter racism, dismissive treatment, language barriers or pressure to abandon culturally important practices. Conversely, adding a cultural ceremony to an otherwise unchanged service does not create cultural safety if the community still lacks influence over decisions.

Why land, culture and language are health determinants

The Australian Government identifies connection to Country, kinship, knowledge and beliefs, language, self-determination and cultural expression as cultural determinants of health. These factors influence identity, resilience, social support, access to food and medicines, and the ability to transmit knowledge between generations.

Canadian engagement work similarly links wellbeing with land, water, sky, food, culture, Elders, traditional medicines and community environments. When environmental contamination, displacement, service restrictions or policy decisions disrupt those relationships, the resulting harm can appear in physical and mental-health statistics even though its causes are social, cultural and political.

Language is particularly important. A patient who cannot describe pain, fear, family responsibilities or treatment preferences in a trusted language may receive clinically inappropriate care. Interpretation is necessary, but language revitalisation and culturally competent communication also require institutions to respect the concepts carried by the language, not simply translate individual words.

What the global evidence says

WHO’s definition

The World Health Organization describes Indigenous Peoples’ concept of health and wellbeing as holistic, encompassing spiritual, environmental, cultural and social dimensions in addition to physical and mental health. It says health is both individual and collective and is strongly determined by community, land and the natural environment.

WHO also frames the right to health through four interrelated requirements: availability, accessibility, acceptability and quality. Discrimination, remoteness, missing or poor-quality data and culturally inappropriate care can undermine all four. Colonial legacies and the absence of intercultural approaches may cause Indigenous concepts of health to be disregarded in non-Indigenous systems, creating barriers to access and wellbeing.

Unequal disease burden

Indigenous Peoples number 476.6 million globally, according to the WHO’s current Global Plan of Action page. The scale and diversity of this population make a single worldwide service model inappropriate, while the persistent inequities make reform urgent.

In Australia, Aboriginal and Torres Strait Islander people experienced a burden of disease 2.3 times that of non-Indigenous Australians, according to the Australian Government Department of Health and Aged Care’s 2021 reporting. That figure describes an outcome gap; it does not prove that Indigenous culture causes poor health. It points toward the effects of colonisation, social and economic conditions, environmental exposure, unequal access and culturally unsafe services.

What culturally safe healthcare should look like

Cultural safety is more than awareness training. It asks whether the person receiving care experiences the service as respectful, understandable and free from racism, and whether the community can influence how care is organised.

Build services with communities

Indigenous organisations, Elders, patients and local knowledge holders should participate from priority-setting through evaluation. Consultation after a program has already been designed is not the same as shared governance.

Use intercultural care

Services should make room, where communities choose, for traditional practitioners, medicines, cultural protocols, family participation and land-based activity. Clinical safeguards and informed consent still apply; cultural practice should not be treated as an optional decoration.

Make access genuinely possible

That includes transport and outreach for remote communities, interpreters, flexible appointment arrangements, continuity of care, privacy that respects family structures and facilities where people are not exposed to stereotyping or humiliation.

Measure what communities value

Health systems should retain necessary clinical measures but add culturally driven indicators. These might include cultural participation, language continuity, connection to Country, family and community wellbeing, trust in services, experiences of racism and local control over care. The indicators must be selected and interpreted by the relevant community rather than assumed to apply everywhere.

Share data authority

Data collection should answer community-defined questions, protect collective as well as individual privacy and give Indigenous authorities a role in access, interpretation and use. Counting disparities without returning information or power to communities can reproduce the same imbalance the data is meant to address.

How governments and health organisations can redesign policy

  1. Recognise multiple knowledge systems. Treat Indigenous knowledge, clinical science and community experience as sources that can inform one another, while respecting the authority of each community.
  2. Fund Indigenous-led systems. Support organisations governed by Indigenous communities to set priorities, employ local workers and design culturally grounded services.
  3. Address structural determinants. Health policy must connect with housing, food security, education, environmental protection, language, land rights, transport and action against racism.
  4. Set accountability measures. Track whether services are acceptable and trusted, not only whether they are available or whether patients attend appointments.
  5. Evaluate locally. Report results by nation, community or region where appropriate. Aggregating distinct peoples into one category can conceal different histories, rights and priorities.

Why a single Indigenous health model would also be a mistake

Indigenous peoples are not one homogeneous population. Nations and communities differ in history, language, governance, law, geography and preferred healing practices. The study’s coverage of Aotearoa, Australia, Canada, the mainland United States and Hawai‘i demonstrates both the reach of shared concerns and the danger of treating those regions as interchangeable.

WHO and Canadian policy materials likewise emphasise local context, distinct rights and community determination. The practical lesson is not to replace one universal template with another. It is to create systems that are accountable to the specific Indigenous peoples they serve.

Bottom line

Indigenous views reveal a structural gap in health frameworks that measure individuals while overlooking relationships, land, culture, spirituality, collective rights and self-determination. Closing that gap requires more than adding cultural symbols to existing services. It requires Indigenous-led priorities, culturally defined measures, intercultural care and action on the colonial and social conditions that shape health before a patient reaches a clinic.
