A healthcare time clock records when each employee starts and stops work so those hours can flow into timesheets and payroll. For a clinic, hospital, home-care agency, or senior-care organization, the right system depends less on brand than on where staff actually work: at a fixed site, moving between client homes, or from an office or remote location. Map those locations and shift patterns first, then check payroll integration, correction workflows, and how any biometric or location data will be handled.

What a healthcare time clock records

At its core, a time clock captures punch times and turns them into hours for timesheets and payroll. Healthcare-oriented platforms often add more: scheduling, approvals, department tracking, location capture, and workforce management. The exact feature set varies by vendor. Vendor pages such as ClockIt’s healthcare time tracking page and Gusto’s healthcare solutions page describe these capabilities, but those descriptions are product claims. They are not independent testing and do not show that a system meets your payroll rules or legal obligations.

A usable record for healthcare work normally includes:

  • the employee’s identity and the method used to verify it (badge, PIN, biometric, login, or device location);
  • the clock-in and clock-out time, including any unpaid meal or rest breaks;
  • the site, unit, or department where the time was worked;
  • any approval or edit, with the name of the person who made the change and when;
  • exceptions such as missed punches, late punches, and time spent traveling between client visits.

Fixed-site and mobile clocking capture different things

The main decision is how the clock recognizes the worker. A fixed clock at a facility answers “was this person physically at this location?” A mobile clock answers “where was the device when the punch was made?” A web clock answers “who logged in?” Each answer carries different trade-offs for accuracy, privacy, and exception handling.

Many healthcare employers end up with a mix. A hospital may use wall clocks for in-house nurses and a mobile app for field staff, and then needs both to feed the same payroll export.

Fixed kiosks and badge, PIN, or biometric clocks

Fixed clocks suit staff who report to one building. They are easy to supervise and do not depend on each worker’s personal phone. Their weak points are shared-use problems: a queue at shift change, a badge that is lent to a colleague, and a device placed where it cannot be reached during a power or network failure. Ask how the device behaves if it loses connection and how punches are stored until it reconnects.

Mobile clocking and location capture

Mobile clocking is built for staff who never reach a fixed terminal. The practical questions are narrower than the marketing: whether the location is captured only at clock-in and clock-out or continuously; whether a worker can clock in when the app cannot confirm a location; and what a manager sees when a punch falls outside the expected area. Home-care operations often need the record to show travel time between visits, so confirm whether that time is captured as work, as a separate entry, or not at all.

Web clocks and apps

Web clocks are simple to deploy for office and remote roles, but they rely heavily on account security. Shared logins, weak password rules, or unrestricted edit rights make the timesheet less trustworthy. Require individual accounts, role-based permissions, and an audit trail for every change to a punch.

Map staff and shift types before comparing products

Most selection mistakes come from describing the business too loosely. Before you open a vendor demo, write down the following for each role:

  1. Where each person clocks in: a fixed facility, client homes, several sites, or a remote office.
  2. Which shift patterns apply: overnight shifts that cross midnight, rotating schedules, split shifts, on-call periods, and callback work.
  3. How breaks are taken and whether they are paid or unpaid, and whether break compliance must be recorded.
  4. Whether hours need to be reported by department, unit, or site for labor tracking or cost allocation.
  5. Who approves hours (a charge nurse, supervisor, or scheduler), how quickly corrections must be made, and which payroll system receives the data.

Compare systems on the points that affect payroll and operations

Once the staffing map is written down, compare each candidate on the same criteria:

  • Clocking methods and whether they match your workplaces, including the fallback for a worker who cannot use the primary method.
  • Scheduling and timekeeping rules for overnight shifts, rounding, breaks, and overtime.
  • Payroll and HR integrations, including whether the export is automatic or manual and who maintains it.
  • Auditability and access controls, including who can edit punches and whether original punches are preserved.
  • Offline behavior for fixed and mobile devices.
  • Employee-data handling, covered in the policy section below.
  • Total cost, including hardware, subscriptions, setup, training, and support.

Current like-for-like prices were not established in the vendor pages or official documents reviewed. Request itemized quotes that list each cost separately so you can compare them directly.

Test before you commit

Vendor feature lists cannot show how a system handles your schedules. A short, structured test can:

  1. Ask the vendor to configure one real unit or team, including its overnight and on-call shifts, and to show how a punch appears in the timesheet.
  2. Create a missed punch and a late correction, then check whether the original entry, the change, and the approver are all recorded.
  3. Run one pay period in parallel with your existing timesheets and compare the hours line by line.
  4. Disconnect the device or app from the network in your own environment, make punches, reconnect, and confirm that every punch arrives once and with the correct time.

Record the results. These are your own pilot results for your staff and workflows, not an independent benchmark.

Federal hours-worked baseline in the United States

The U.S. Department of Labor’s Wage and Hour Division publishes Fact Sheet #53 on hours worked in healthcare. The sheet, revised July 2009, states: “Nonexempt employees must be paid for all hours worked in a workweek.” It describes hours worked generally as time an employee must be on duty, on the employer’s premises, or at another prescribed place of work, and says that hospitals and certain other institutions primarily engaged in care are covered employers. Because the sheet dates from 2009 and state or local rules can add requirements, treat it as a federal starting point and confirm the current rules for your location and workforce. For clock design, the practical question is whether the system captures the work the sheet describes, including short tasks before or after a scheduled shift, rather than recording only the scheduled shift times.

Biometric and location data need a policy before go-live

Biometric clocks can verify identity at the device, but they collect sensitive employee information. Whether a method is appropriate is a policy decision for each employer, not a universal recommendation. Before choosing one, document the following:

  • what is collected, and whether the device stores an image or a mathematical template;
  • where the data is stored and which vendor or system holds it;
  • who can access it, both inside your organization and at the vendor;
  • how long it is retained and how it is deleted, including when an employee leaves;
  • how employees receive notice, and whether consent is needed where you operate;
  • what alternative method is offered to staff who object or cannot use the device.

Two published examples show how these questions are answered in practice. A Thorek Memorial Hospital policy, effective and last revised in March 2021, says its ADP time clocks generate mathematical representations of employee finger features used for timekeeping and payroll. It describes the hospital’s data protections and deletion process and states that it protects this data under the Biometric Information Privacy Act (BIPA) even where that law may not apply. The Thorek timekeeping policy is one employer’s policy, not a legal standard. UKG’s September 26, 2025 sample device-scan data statement describes notice, consent, and deletion provisions and advises each customer to consult its own legal adviser. See the UKG sample statement. Neither document settles what applies to your organization, which depends on your jurisdiction and the system you choose.

Location data for mobile clocking

Location data is personal data even when it is used only for timekeeping. Limit capture to the moments needed to verify a clock-in or clock-out, set a retention period, and restrict who can view raw location history as opposed to summarized exceptions. Tell staff in writing what is captured and when.

What the available evidence does and does not establish

  • Established: the general function of healthcare time clocks, the main clocking categories, and the federal hours-worked baseline described in the 2009 DOL sheet.
  • Established as examples only: the Thorek policy (March 2021) and the UKG sample statement (September 26, 2025) on biometric data handling.
  • Not established: independent accuracy measurements, adoption rates, savings figures, or clinical outcomes for any healthcare time clock. Vendor savings claims should be treated as marketing until your own pilot data supports them.
  • Not established: how each state or country applies its wage, privacy, or biometric rules to these systems, and current comparable pricing.
