“Therapeutic screening for Alzheimer’s disease” can mean evaluating potential treatments in laboratory research and clinical development, or screening people to see whether they qualify for a research study. These are different activities: neither a biomarker test nor a trial-eligibility assessment predicts an individual’s outcome or provides a consumer route to experimental medicines.
What therapeutic screening means
There is no single standardized meaning for “therapeutic screening” in the cited guidance. In drug discovery, researchers screen potential compounds or approaches for useful biological effects. In clinical development, they test a candidate treatment in people to assess safety and whether it helps. Separately, a research team may screen prospective participants against a study’s eligibility requirements, which can include biomarker testing.
Keep the purpose clear: screening a compound is not the same as testing a medicine in a clinical trial, and neither is the same as testing a person for a biomarker or diagnosing Alzheimer’s disease.

Define the disease stage and intended population
Alzheimer’s studies need to specify whom a candidate treatment is intended to help and at what stage. The FDA’s March 2024 revised draft guidance, Early Alzheimer’s Disease: Developing Drugs for Treatment, focuses on drug development for sporadic Alzheimer’s before overt dementia. It discusses diagnostic criteria, clinical staging, outcome measures, and surrogate endpoints. The document is labeled “Draft Level 1 Guidance” and “Not for implementation”; its recommendations are nonbinding.

Choose measures that answer the right question
Researchers distinguish measures of a biological process from measures of how a person functions or feels. A biomarker may show that a treatment affected a target or disease-related process; a clinical outcome measure is needed to assess effects that matter to participants. The FDA cautions that a surrogate endpoint is not itself proof of clinical benefit, even if it may be reasonably likely to predict benefit.
The European Medicines Agency’s guidance on clinical investigation of medicines for Alzheimer’s disease addresses development across disease stages, including diagnostic criteria, biomarkers, trial outcomes, and efficacy and safety study design. Its page identifies Revision 2 as legally effective from 2018-09-01 and records a Revision 3 concept paper published 2025-08-06, whose consultation closed 2026-02-28. Those are distinct status milestones: the concept paper is not itself the effective Revision 2 guideline.

Assess safety and clinical relevance
Clinical studies evaluate safety as well as potential benefit. Results must be interpreted in light of the study population, disease stage, treatment duration, chosen outcomes, and limitations of the measures. A change in a laboratory or biomarker measure alone does not establish that a candidate improves a participant’s condition.

Trial teams may use biomarkers to help determine whether someone meets a study’s requirements and to track the effects of an intervention. A biomarker result is one part of an assessment, not a stand-alone diagnosis. The National Institute on Aging explains that no single test diagnoses dementia; some biomarker tests are limited to specialty clinics or research facilities, and insurance coverage varies by test. Eligibility rules depend on each study’s design and intended population. A screening assessment may include clinical evaluation and other tests; satisfying one biomarker criterion does not by itself mean a person is eligible, will benefit, or should receive an experimental treatment. People considering testing or research participation should discuss the purpose, implications, and costs of testing with a qualified clinician or the study team.
What Alzheimer’s research includes
Therapeutic research is broader than screening drug compounds. The NIH inventory counted 466 active Alzheimer’s and related dementias clinical trials in March 2025. That dated inventory included drug development, nondrug studies, dementia care and caregiving, disease-process studies, diagnostic tools and imaging, and neuropsychiatric symptom treatment. Approved treatments are different from experimental candidates
In the United States, the FDA has approved lecanemab and donanemab for early Alzheimer’s. These prescription medicines target amyloid plaques; clinical trials showed slower cognitive decline in some people, but neither is a cure. Both require careful medical monitoring because of potential side effects. The NIA says research continues on safety, use at other disease stages, more diverse populations, and combinations with other therapies. How to interpret a screening or evaluation claim
- Ask what is being screened. Is it a laboratory compound, a candidate medicine in clinical development, or a person being assessed for trial eligibility?
- Check the intended population and stage. Findings from one stage or group do not automatically apply to others.
- Find out what the measure represents. A biomarker or surrogate endpoint may indicate a biological change, not a demonstrated clinical benefit.
- Look for safety evidence and limitations. Trial results need to be understood alongside monitoring requirements and the study’s design.
- Do not treat screening as a personal forecast. Screening does not establish an individual’s future course or guarantee access to an experimental treatment.
