The best-documented technology failure of 2024 is the July 19 outage caused by a faulty content update to CrowdStrike’s Windows sensor. The update made Windows computers crash, and the disruption reached commercial flights and hospital care. This article covers that event in depth, then two other 2024 disruptions: a Microsoft Azure access error on July 18 and an AT&T U.S. cellphone network outage in February.

It does not present a numbered list of eight. “Worst” is an editorial judgment, and the published accounts we can rely on do not describe the other candidate incidents on comparable terms. An eight-entry ranking would imply a precision the reporting does not support.

What happened on July 19, 2024

CrowdStrike released a Windows sensor content configuration update on July 19, 2024. Its preliminary report, titled “Falcon Content Update Preliminary Post Incident Report,” gives the release time as 04:09 UTC. The U.S. Government Accountability Office (GAO) says the faulty update caused Windows systems to crash. CrowdStrike later published a root-cause analysis titled “Channel File 291 Incident RCA is Available,” which centers the investigation on that channel file.

Because the outage hit Windows machines rather than a CrowdStrike-hosted service, its effects depended on what each organization ran on those machines. That is why the impact accounts below focus on specific services rather than a single headline number.

Scale and consequences

GAO describes the event as potentially one of the largest IT outages in history. Its report states that the crashes disrupted critical infrastructure, including commercial flights and hospital care. GAO’s characterization is qualitative and does not give a measured total for losses or downtime.

The most specific count comes from an Associated Press report dated September 25, 2024, which puts the number of affected Windows computers at about 8.5 million. That figure is an estimate attributed to CrowdStrike’s account of the outage. It counts machines, not money lost, hours of downtime, or people affected, and it should not be compared with figures from other incidents.

The same AP coverage of CrowdStrike threat intelligence division leader Adam Meyers’s congressional testimony in September 2024 quotes him saying, “We let our customers down.” House Homeland Security Committee Chairman Mark Green said, “Everywhere Americans turned, basic societal functions were unavailable,” according to AP reporting from 2024.

The Azure access error on July 18

The Congressional Research Service (CRS) describes a separate Azure incident on July 18, the day before the CrowdStrike update. CRS attributes it to an access error involving virtual machines and infrastructure in the Central US region.

The two events are easy to conflate because they happened one day apart and both affected Microsoft products. They had different causes. Microsoft’s customer post “Helping our customers through the CrowdStrike outage,” dated July 20, 2024, addresses the CrowdStrike incident, not the Azure access error.

The AT&T cellphone network outage on February 22

The Associated Press reported on February 22, 2024 that an outage on AT&T’s U.S. cellphone network was not caused by a cyberattack, according to AT&T. The AP report does not establish the technical root cause of the outage. Treat the no-cyberattack statement as AT&T’s account rather than an independently verified finding. The report also does not state how many customers were affected or how long service was disrupted.

How the three incidents compare

Comparison works only when the same criteria apply to each incident. The table below uses four editorial criteria: documented reach, effect on essential services, duration and recovery burden, and failure mechanism. Where published accounts do not state a value, the cell says so and names the source.

Incident Failure mechanism (as reported) Documented reach Effect on essential services Duration and recovery
CrowdStrike Windows sensor content update, July 19, 2024 Faulty content configuration update; CrowdStrike’s root-cause analysis centers on Channel File 291 About 8.5 million Windows computers (AP, September 25, 2024; estimate attributed to CrowdStrike’s account) Commercial flights and hospital care disrupted (GAO) Not stated in the cited reporting
Azure access error, July 18, 2024 Access error involving virtual machines and infrastructure in Central US (CRS) Not stated (CRS) Not stated (CRS) Not stated (CRS)
AT&T U.S. cellphone network outage, February 22, 2024 Not established in the AP report; AT&T said it was not caused by a cyberattack Not stated (AP) Not stated (AP) Not stated (AP)

On these criteria, the CrowdStrike outage is the only one of the three with documented reach and a documented effect on essential services. That is the basis for leading with it.

Why the list stops at three

An eight-entry ranking would need each incident measured on the same criteria and supported by an authoritative account. For the CrowdStrike outage, that exists in the form of company reports, a GAO report, and congressional testimony. For Azure and AT&T, the published accounts are much thinner, and the public record does not yet supply that level of detail for the other candidates. Until each one can be placed on the same scale, a numbered list would look more rigorous than the evidence allows.

Reading the 2024 record carefully

  • Check the time zone and date for every event. Two incidents in this article fell on consecutive days, and the CrowdStrike release time is given in UTC.
  • Treat counts attributed to a company as estimates until an independent or audited figure is published.
  • Distinguish a company’s statement about cause from a published root-cause analysis. A statement that an outage was not a cyberattack does not explain what did cause it.
  • Read primary documents first: CrowdStrike’s preliminary report and root-cause analysis, the GAO report, and the CRS account.
