Spoiler warning: This guide reveals the late-game outcomes of The Witcher 3: Wild Hunt — Blood and Wine. The commonly recommended “best” ending keeps Anna Henrietta and Syanna alive, allows the sisters to reconcile, and ends with Dettlaff dead. It is a recommendation based on those character outcomes, not an objective game score.
How to get the ending where both sisters survive
Do not go directly to the Unseen Elder if you want the reconciliation route. Follow Syanna’s story into the Land of a Thousand Fables, where one key decision determines whether she survives her meeting with Dettlaff.
- Follow the Syanna route. Continue through the fairy-tale world rather than bypassing it for the Unseen Elder.
- Get the magical ribbon from the flint girl. The ribbon protects Syanna during her encounter with Dettlaff. Without it, she dies before the ceremony.
- Learn Syanna’s story and respond with empathy. Hear her account of her past and choose supportive responses when speaking with her before the ceremony. These conversations matter to the reconciliation outcome; exact dialogue wording can vary by the game’s dialogue context, so focus on listening and showing understanding rather than relying on an unverified verbatim checklist.
- Resolve the final confrontation. Keeping both sisters alive still requires Geralt to deal with Dettlaff. In this ending, Dettlaff dies.
The resulting outcome is that Anna Henrietta and Syanna both live and reconcile.
How the main endings differ
| Route or outcome | Anna Henrietta | Syanna | Sisters reconcile? | Dettlaff | Geralt’s consequence |
|---|---|---|---|---|---|
| Ribbon and reconciliation route | Lives | Lives | Yes | Dies | Not stated in the cited quest summaries |
| Fairy-tale route without the ribbon | Lives | Dies; Dettlaff kills her before the ceremony | No | Not stated in the cited quest summaries | Imprisonment is not established for this specific case by the cited summaries |
| Unseen Elder route and failed reconciliation | Dies; Syanna kills her | Dies; guards kill her | No | Not stated in the cited quest summaries | Not stated in the cited quest summaries |
| Syanna dies and Dettlaff is spared | Not stated in the cited quest summaries | Dies | No | Lives | Geralt is imprisoned |
The last row describes a consequence documented for the specific case where Syanna dies and Dettlaff is spared; it should not be treated as a complete account of every branch. Likewise, the Unseen Elder route is distinct from losing Syanna by failing to obtain the ribbon: in that route, the ceremony ends with Syanna killing Anna Henrietta, followed by guards killing Syanna.

Which ending should you choose?
Choose the ribbon-and-reconciliation route if your priority is keeping both sisters alive and resolving their story together. “Best” is a common guide label for that result, not a moral verdict built into the game. If you prefer the consequences of another branch for role-playing reasons, choose according to your priorities instead.
