A human approval click does not, by itself, prove that an AI agent performed only the operation the reviewer saw. Prompts can encourage rubber-stamping, summaries can conceal cumulative effects, and execution can diverge from the approved request. Safer systems make review risk-based, bind approval to the effective action, enforce limits outside the model, and log what actually happened.
What can an AI agent do after I approve it?
That depends on the permissions and controls around the agent, not just on the approval screen. Approval may authorize a particular operation, but a weak design can fail to show its full consequences or to ensure that the operation executed is the one the person approved.
For example, a reviewer might see a short description while the agent has assembled a multi-step operation. Or an approval may be recorded without binding it to the exact tool, arguments, target, and scope used later. These are different problems: one concerns what the reviewer understood; the other concerns whether the system enforced the decision against the action that ran.

Why isn’t human approval enough to secure an AI agent?
Repeated prompts can weaken attention
Anthropic reports that its Claude Code telemetry showed users approved roughly 93% of permission prompts. That is an organization-reported observation about one product’s users, not a general approval rate for agent systems, and it does not establish that every approval was careless. Still, AWS warns that sending every action to a person can create fatigue and rubber-stamping. A prompt that appears constantly is less likely to receive careful scrutiny.

The prompt may not faithfully describe the action
A reviewer can only judge the information presented. A brief agent-written summary may omit a consequential detail, while a series of individually modest steps can combine into a high-impact outcome. Microsoft’s June 2026 red-team taxonomy highlights both action decomposition and agent-written summaries that launder meaning as patterns approval controls should resist. These are findings from red-team engagements, not a representative estimate of how often systems fail in production.

The approval decision may not be enforced against execution
A September 2026 preprint by Yang Wang calls this approval-to-execution binding and identifies six ways the eventual operation may diverge from what was authorized:
- Scope: the executed action reaches beyond the approved scope.
- Argument: the arguments or parameters differ from those presented for approval.
- Temporal: the authorization is reused after circumstances or its validity have changed.
- Tool: a different tool or execution path is used.
- Delegation: another agent or actor carries out work not covered by the original decision.
- Semantic laundering: the description presented for approval obscures the practical meaning of the action.
The paper reports controlled repeated-measures tests of one coding-agent harness, with 19–20 runs per failure class. It is a useful taxonomy, not proof of industry-wide prevalence; the authors identify cross-harness measurement as future work.
The risk classifier can be influenced by the same untrusted request
If an LLM sees untrusted content both when planning an action and when deciding whether that action deserves review, that content may influence the risk judgment too. AWS advises against relying on such a model alone to classify risk. Define high-risk categories and review triggers through deterministic policy where possible.

Monitoring is useful, but incomplete
OpenAI describes internal monitoring of coding agents and says some misbehavior is easier to identify from reasoning and tool traces than other kinds. It also says it cannot confidently quantify false-negative rates on open-ended real-world traffic without dedicated control evaluations and red teaming. Its report of approximately 1,000 conversations triggering moderate-severity alerts is monitoring context: many were deliberate internal red-team conversations, and the examples were among the most severe observed from tens of millions monitored. It is not a measure of approval-gate failures.

Can an agent execute something different from what I approved?
It can if the system records a human decision but does not bind that decision to the exact action at execution time. A robust design treats approval as authorization for a defined operation, not as a general permission for the agent to continue pursuing a goal.

At execution time, the system should verify that the action still matches the approved values. If any material field has changed, it should require a new decision or deny execution. These checks reduce mismatch risk, but they do not automatically capture every downstream effect; the policy must also constrain what the operation can cause.

OpenAI’s Agents SDK documents a human-in-the-loop mechanism for pausing an agent run for approval. A pause or approval step is a workflow feature; its presence alone does not establish that every application has bound a decision to all relevant execution details. Route review by consequence
Do not ask for the same level of scrutiny for every action. Consider whether an operation changes external state, exposes data or money, can be reversed, and how wide its potential impact is. Let routine, low-risk actions proceed only within defined controls; require human review before critical or difficult-to-reverse operations. Microsoft recommends accounting for reversibility and blast radius, while AWS recommends routing critical decisions to review.
Show the effective operation, not just a reassuring summary
Give reviewers enough context to understand what will happen: the action, target, scope, material parameters, relevant identity, and consequential downstream effects. Make compound operations visible as a whole rather than letting a string of small prompts hide their combined result. Set clear timeout and escalation behavior so that an unanswered prompt does not silently become approval.

Use policy rules to invoke review for defined high-risk operations rather than asking an LLM influenced by the same untrusted request to decide whether review is needed. Record approval against a canonical description of the operation, then check the relevant identity, tool, arguments, scope, target, and validity at execution time. A preview or token can help only if the executing system verifies that it still represents the authorized operation.

Enforce boundaries outside model instructions
Prompt instructions express intended behavior; they do not reliably restrict what an agent can access. AWS recommends layered controls, including scoped identity and access management, input-schema validation, and policy enforcement. Anthropic describes sandboxes, virtual machines, and egress controls as ways to contain potential damage if behavioral controls fail. Prompt injection can try to steer an agent toward costly actions, and a capable model may find an unexpected route around restrictions that were not enforced.

Keep an audit trail and test the controls
Log the proposed action, the reviewer’s decision, the execution identity and parameters, the policy version, and the actual result. Use controlled evaluations and red-team exercises to test whether the reviewer saw the meaningful action and whether execution enforced the decision. A lack of observed incidents is not evidence of a low miss rate when monitoring coverage and false negatives are uncertain.
What should a sound approval design check?

| What exactly did the person authorize? | Identify the principal, agent, session, tool, arguments, scope, target, expiration, and material downstream effects; check these at execution. |
| Where is the restriction enforced? | Prefer deterministic policy, scoped permissions, schema checks, and environment containment over prompt judgment alone. |
| Can the reviewer make an informed decision? | Show the complete meaningful action, account for compound steps, and define timeout and escalation handling. |
| How strong is the evidence that the control works? | Use controlled tests and red-team coverage; do not treat anecdotes or incomplete monitoring as a measured failure rate. |
No single approval pattern is established as universally best. The safeguards need to match the operation’s consequences and the limits of what the system can observe.
