Targeted therapy attacks a defined molecular feature that helps cancer grow, divide, spread or survive. Chemotherapy works mainly by damaging or stopping rapidly dividing cells, whether they are cancerous or healthy. That difference explains why targeted drugs often require biomarker testing, while chemotherapy can be selected without a matching tumor mutation. It does not make targeted therapy universally safer or chemotherapy universally inferior: the right treatment depends on the cancer type, stage, molecular findings, previous treatment, overall health, goals and access.
Both approaches can be combined with surgery, radiation, hormone treatment, immunotherapy or one another. The sections below explain how each works, what testing is needed, what side effects and monitoring to expect, and what doctors do when a targeted drug loses effectiveness.
What targeted therapy and chemotherapy each act on
Targeted therapy follows a molecular target
Targeted therapy is designed around a protein, gene change or signaling pathway that is unusually important to a cancer cell. The National Cancer Institute (NCI) describes it as a foundation of precision medicine because treatment is selected using information about the tumor’s biology. A target may control cell growth, division, survival, blood-vessel formation or the spread of cancer.

- Small-molecule drugs are small enough to enter cells and interfere with proteins inside them. Many block an enzyme or a signal that the cell needs to keep growing.
- Monoclonal antibodies are laboratory-made proteins that attach to targets on the outside of a cell or in the surrounding tissue. They may block a growth signal, mark a cell for destruction, prevent new blood-vessel formation or carry a toxic payload.
“Targeted” describes the intended molecular action, not a guarantee that only cancer cells are affected. The target can exist in some healthy tissues, and a drug’s downstream effects can injure organs or other normal cells.
Chemotherapy follows cell behavior
Chemotherapy uses drugs that kill cells or stop them from multiplying. Many cancer cells divide rapidly, so they are vulnerable to medicines that damage DNA, interfere with cell division or otherwise prevent reproduction. Healthy tissues that also renew quickly can be affected, including bone marrow, the lining of the mouth and digestive tract, hair follicles and, with some regimens, reproductive cells.
This is a tendency rather than an absolute boundary. Some targeted medicines directly kill cells or carry a chemotherapy payload, and chemotherapy can be selected for cancers with particular molecular features. Treatment plans frequently combine drugs with different mechanisms to improve cancer control or prevent resistance.
How disabling cancer proteins can stop a tumor
A targeted drug can interfere with cancer biology in several ways. The exact mechanism depends on the drug and the cancer, so an example from one disease should not be assumed to apply to another.
Blocking growth signals
Some cancers depend on an overactive receptor or signaling enzyme. A matching inhibitor can prevent the signal from reaching the cell’s growth machinery. For example, a tumor with a treatable EGFR change may be considered for an EGFR inhibitor when the drug is appropriate for that cancer and setting. The presence of a change alone does not establish that every EGFR-directed drug is approved or effective for every tumor.

Attaching to a surface protein
An antibody can bind a protein on the cancer-cell surface, block its growth message or make the cell more visible to immune defenses. HER2-directed antibodies are one familiar example, but the drug, cancer type, stage and treatment line determine whether a particular product is suitable.
Delivering a toxic payload
Some antibody-based medicines use a target as an address label. After binding to a cell, they deliver a toxin or chemotherapy payload near or inside that cell. This is still targeted therapy in design, but the payload can produce toxicities associated with the attached drug and with the targeted tissue.
Triggering cell death or starving a tumor
Other drugs push a cancer cell toward programmed death or block the formation of blood vessels that supply a tumor. Anti-angiogenic treatment can affect normal blood-vessel function, which is why blood pressure, bleeding, clotting and wound healing may require attention.
Removing a hormone signal
Hormone-dependent cancers may require estrogen, androgen or another hormone signal. Drugs can reduce hormone production, block a hormone receptor or make the receptor unavailable. These medicines are targeted at a biological dependency, but their effects can extend to normal tissues that use the same hormone pathway.
Why biomarker testing matters
Biomarker testing examines genes, proteins and other substances in a tumor to find information that may guide treatment. NCI distinguishes this testing from inherited genetic testing: the relevant finding is usually a feature acquired by the cancer, not a mutation present in every cell of the body.

What a test can examine
- Tumor tissue: A pathologist analyzes a sample obtained during a biopsy or surgery. The procedure has risks that depend on the organ and technique.
- Blood, or a liquid biopsy: A validated blood test may look for tumor DNA or other cancer signals when a tissue sample is unavailable or when monitoring is appropriate. A negative blood result does not always prove that the tumor lacks a target.
- Protein or gene tests: Tests may measure the amount of a protein, a specific DNA alteration, gene fusions, copy-number changes or a broader panel of alterations.
A result can point to an FDA-approved treatment, an off-label option or a clinical trial. It can also identify a change for which no validated treatment match exists. A biomarker result by itself does not determine treatment; clinicians interpret it alongside pathology, stage, prior therapy, other test results, overall health and the person’s goals.

- Ask which test is being ordered and whether it examines one marker or a broad panel.
- Confirm whether the sample is adequate and whether a new biopsy is needed.
- Ask how long results are expected to take and whether treatment decisions can safely wait.
- Find out what an actionable, uncertain or negative result would change.
- Ask whether the test can identify clinical-trial options as well as approved treatments.
A test should be ordered because its result could affect care, not simply because a panel is available. Your oncology team can explain procedure risks, privacy considerations and how findings may affect relatives; inherited-risk counseling is a separate question from choosing a drug for the tumor.
Is targeted therapy safer than chemotherapy?
There is no universal safety ranking. Targeted therapy can spare some rapidly dividing healthy cells, but it can also cause serious, drug-specific injury. Chemotherapy side effects vary with the regimen, dose, schedule and the person receiving it. A fair comparison considers the expected benefit for the particular cancer as well as immediate and long-term toxicity.
| Issue | Targeted therapy | Chemotherapy |
|---|---|---|
| Main vulnerability exploited | A defined protein, pathway, receptor, gene alteration or hormone dependence | Rapid cell growth and division |
| Biomarker requirement | Often required or strongly preferred, depending on the drug and indication | Usually selected by cancer type, stage and regimen; a matching biomarker is not always required |
| Common examples of toxicity | Diarrhea, liver problems, high blood pressure, fatigue, mouth sores, rash, dry skin, nail changes, altered hair color, blood-clotting or wound-healing problems | Low blood counts, hair loss, nausea or bowel symptoms, mouth injury and fertility effects with some regimens |
| Monitoring focus | Drug-specific laboratory tests, blood pressure, skin, liver function, heart or other organs, depending on the target | Blood counts, kidney or liver function, infection risk, symptoms and organ-specific effects |
| Resistance pattern | A changed target or an alternative growth route can make the drug ineffective | Residual cells can also become resistant through multiple biological mechanisms |
| Route and schedule | May be a daily or intermittent pill, an infusion, an injection or another schedule | May be intravenous, oral, injected or regional, with schedules ranging from daily dosing to cycles |
| Cost and access | Varies by drug, indication, insurance, administration and assistance programs | Varies by regimen, administration, supportive medicines and coverage |
NCI lists diarrhea and liver problems among common targeted-therapy effects and also notes possible high blood pressure, fatigue, mouth sores, nail changes, loss of hair color, skin problems, blood-clotting problems and impaired wound healing. Rare severe injury is possible. Some effects improve after treatment ends, while others need long-term follow-up.
Do not wait for a routine appointment if you develop severe symptoms. Your treatment team should give drug-specific instructions for fever, breathing trouble, chest pain, uncontrolled diarrhea or vomiting, jaundice, unusual bleeding, a rapidly spreading rash, confusion or other urgent changes.

How doctors choose between the approaches
Oncology teams generally weigh several questions rather than asking whether one category is “better.”
- What cancer is present? Pathology identifies the disease and relevant subtype.
- How extensive is it? Stage, spread, tumor burden and symptoms affect urgency and the role of local treatment.
- Does the tumor have an actionable biomarker? The result must have a credible treatment match for that cancer and situation.
- What has already been tried? Previous drugs can affect resistance, organ tolerance and sequencing.
- What is the person’s health and treatment goal? Organ function, other illnesses, preferences and whether the aim is cure, long-term control or symptom relief all matter.
- What monitoring and access are feasible? Oral treatment can still require frequent laboratory visits; an infusion may require travel and observation. Coverage, approval status and supply can influence realistic choices.
The plan may include surgery or radiation for local control and a systemic drug for cancer cells elsewhere. It may use chemotherapy alone, a targeted drug alone, sequential therapy or a combination. The expected benefit and risks should be discussed for the exact cancer and regimen, not inferred from another disease.

Before the first dose
Clinicians review pathology and biomarker results, medications, allergies, organ function, pregnancy potential when relevant, and prior treatments. Baseline blood tests and sometimes heart, lung, kidney, liver or eye evaluations are chosen according to the drug. Ask which symptoms require a call before treatment begins.
During treatment
Monitoring may include blood counts, chemistry panels, liver tests, blood pressure, weight, heart tests or imaging. Some targeted drugs are pills taken at home; others are infusions or injections administered in a clinic. Chemotherapy is commonly delivered in cycles, but schedules vary widely. Do not change a dose, skip a treatment or crush a pill without explicit instructions.
Managing side effects
Early reporting allows the team to pause treatment, change the dose, prescribe supportive medicines or switch therapy when appropriate. Keep a record of doses, symptoms, temperature readings when instructed and other medicines or supplements. A side effect that seems mild can become dangerous if it signals liver injury, infection, bleeding, dehydration or another complication.

What happens if targeted therapy stops working?
Resistance can arise when the drug’s target changes, when the cancer activates a backup growth route or when different tumor cells survive and expand. Progression may also reflect inadequate drug exposure, an interaction, a new cancer process or a scan finding that needs confirmation. Your oncologist interprets imaging, symptoms and laboratory results together rather than assuming every change is resistance.
Typical next steps
- Confirm the pattern. The team may repeat imaging, review pathology and compare the new findings with the original disease.
- Look for a new explanation. Repeat tissue or blood testing may identify a resistance alteration or a newly actionable feature when the test is validated and safe.
- Change the strategy. Options can include another targeted drug, a combination, chemotherapy, radiation, surgery for a specific site, supportive care or a clinical trial.
- Reassess goals and tolerance. The best next treatment balances disease control, side effects, logistics and the person’s priorities.
Resistance is one reason targeted therapy is sometimes paired with another targeted drug, chemotherapy or radiation. Combination treatment can improve control in selected cancers but can also add toxicity and monitoring, so it is not automatically preferable.
Targeted therapy is not the same as immunotherapy
Immunotherapy changes or harnesses immune activity to help the body recognize or attack cancer. Targeted therapy directly interferes with a cancer-associated protein or pathway, although some targeted antibodies can recruit immune cells as part of their action. A treatment should be classified by its mechanism and the specific product, not by the fact that it is an antibody or that it affects the immune system.

“Targeted means harmless.”
Targeted drugs can cause severe diarrhea, liver injury, high blood pressure, clotting or wound-healing problems and other serious effects. They require informed monitoring just as chemotherapy does.
“Chemotherapy always harms more than targeted therapy.”
Some chemotherapy regimens are highly effective and manageable, while some targeted drugs have substantial organ-specific toxicity. The comparison must use the actual regimen and the person’s risk factors.
“A positive biomarker guarantees a response.”
A marker indicates a possible treatment opportunity, not certainty. Tumor biology, coexisting alterations, drug exposure, resistance and disease burden all influence response.
“A negative test rules out every targeted option.”
A test may not detect every alteration, the sample may be insufficient, and another validated test may be relevant. Ask what the assay covered and whether retesting is clinically justified.

The practical takeaway
Targeted therapy is target-dependent; chemotherapy is primarily growth-rate-dependent. A biomarker can open the door to a targeted treatment, but it does not make that treatment suitable by itself. Targeted drugs can be seriously toxic, chemotherapy can be highly effective, and either approach can stop working. The oncology team uses the cancer’s pathology, stage, molecular findings, previous treatment, health, goals and access to build and revise the plan.
