If several capable people rely on the same workaround to finish a task, the SOP may no longer describe the work—even if it looks polished and has a recent review date. The practical fix is a repeatable way to compare the instruction with current practice, understand any mismatch, and correct the controlled document when needed.

What it means when an SOP is “wrong”

An SOP is out of step when its sequence, prerequisites, tools, roles, or decision points no longer match how the task is performed or what the process requires. A difference between the document and the work is a signal to investigate, not proof that an employee has failed to follow instructions. The cause might be a stale procedure, unclear wording, a tool constraint, an approved deviation, a training gap, or a useful process improvement.

The U.S. Environmental Protection Agency’s Guidance for Preparing Standard Operating Procedures says, “SOPs need to remain current to be useful.” The goal is not to make review burdensome: the same guidance says, “The review process should not be overly cumbersome to encourage timely review.”

Build a practical way to notice discrepancies

Make the routine light enough to repeat, but specific enough to produce a decision and a controlled record.

  1. Name the owner and users. Identify who is accountable for the SOP’s accuracy and who actually performs the task. Include people with suitable training and experience, as well as process users who can explain where the written sequence does or does not fit.
  2. Walk through the current work. Compare the SOP with a real task walkthrough. Another useful check is to ask someone other than the writer to follow the instruction and note where they need clarification. EPA recommends testing drafts with people other than the original author; U.S. Department of Energy facility guidance also recommends validating procedures through walkthroughs or similar methods.
  3. Record observable differences. Note unclear steps, missing prerequisites, unofficial workarounds, confusing handoffs, tool differences, and decision points absent from the document. Capture what happened and where; do not assign blame before the team understands the reason.
  4. Look for review triggers between scheduled checks. A process or source-document change, an error, an unclear instruction, or an unusual incident can justify an earlier review. DOE guidance identifies unusual incidents and procedure inadequacies or errors as reasons to review relevant procedures; EPA guidance says to update an SOP when procedures change.
  5. Decide what the discrepancy means. Determine whether the SOP is stale or incomplete, the difference is an approved one-off deviation, users need training, a tool imposes a constraint, or the process itself should change. These are practical investigation categories, not a formal taxonomy supplied by the cited guidance.
  6. Revise through document control. Use your organization’s defined review and approval route. Update version identifiers and dates, notify affected users, replace active copies, and archive the superseded version so people can tell which instruction is current.
  7. Check that the revised instruction works. Have a user follow it and confirm that the intended result can be achieved. A walkthrough can reveal ambiguity that a writer may not notice.

Use event triggers as well as calendar reviews

A calendar review can catch gradual drift; an event-triggered review can catch a consequential change before the next scheduled date. A workable operating design uses both: assign each SOP a review interval, then reopen it sooner when the process changes, a source document changes, a walkthrough exposes a mismatch, or an unusual incident raises questions.

Review intervals vary by context; there is no universal schedule established by these examples. EPA QA/G-6 (April 2007) recommends systematic periodic review and gives every one to two years as an example. A particular EPA laboratory procedure, revised May 8, 2019, calls for review at least every three years and earlier revision when a change is required. DOE facility guidance says the subsequent review frequency should be specified and may vary with activity type, complexity, and operational maturity. These are differing examples, not a single rule.

Keep a register that makes stale instructions visible

EPA also notes that an electronic database can send automatic review notices. A paper procedure may still be useful at the point of work; the important control is ensuring the copy is current and that obsolete copies are withdrawn. A maintained register and accessible point-of-use instructions solve different problems, and both need an accountable owner.

A specific EPA laboratory procedure revised in 2019 describes one approach: official copies are tracked, revisions reviewed and approved, and obsolete copies withdrawn while the controlled original is archived. It also describes destroying other controlled copies. Treat those mechanics as an example, not a universal requirement; tailor retention and withdrawal practices to your organization’s recordkeeping rules.

Make the review useful, not ceremonial

A review date alone cannot show whether an instruction still works. A practical check asks whether the written sequence matches the work, whether users can understand and complete it, and whether changes to tools, roles, source documents, or decisions have been reflected. The result should be a clear decision: retain the SOP as written, clarify or revise it, address a training or tool issue, or document an approved deviation through the organization’s process.

Keep the change process proportionate. The EPA guidance’s emphasis on timely, manageable review supports a routine people can actually maintain: a named owner, a visible review date, event triggers, and user validation when the instruction changes.
