Some human insulin preparations have retained measured potency for months under warmer-than-recommended conditions in laboratory tests and manufacturer data. That does not mean insulin is temperature-proof: the findings cover specified products and conditions, and they do not replace the storage limits on your insulin’s label. If insulin has been exposed to heat, freezing, or an uncertain temperature, follow its product instructions and ask a pharmacist, manufacturer, or clinician before relying on it.
What the temperature-resilience evidence found
A 2023 Cochrane review examined human insulin stored outside manufacturers’ recommended temperature ranges or use periods. Its findings suggest that some preparations are more stable in warm conditions than conventional storage advice might imply, but results varied by product and experiment.
Laboratory studies
In some laboratory studies, unopened short-acting, intermediate-acting, and mixed insulin showed no clinically relevant activity loss under tested conditions reaching 28.9°C–37°C for up to four months. Other studies found activity losses of up to 18% after one to four weeks at 37°C. These results are not a single safe-temperature rule: they involved different preparations and study conditions.
Manufacturer data summarized by Cochrane
For specified unopened short-acting and intermediate-acting human insulin vials or cartridges, company data summarized in the review indicated storage at up to 25°C for up to six months, or up to 37°C for up to two months, without clinically relevant potency loss. In Novo Nordisk data cited by the review, measured activity loss at 37°C was:

These are attributed company measurements for tested products and conditions, not instructions for patients to store insulin outside its approved limits.
Opened containers and temperature cycling
Some studies found no clinically relevant activity reduction in opened vials or cartridges stored at up to 37°C for up to 12 weeks, or cycled between 25°C and 37°C for up to 12 weeks. The review could not formally assess evidence quality using validated methods, and much of the evidence was laboratory-based. It called for clinical studies and noted gaps on sterility, premixed insulin, pumps, and cold conditions.

A separate 2024 laboratory study measured Humulin-R and glargine U-100 concentrations over 12 months under steady 4°C, 22°C, and 42°C conditions and daily temperature cycling. It concerns those formulations in a laboratory protocol; it does not establish general storage guidance or clinical suitability after a real-world temperature excursion.

How to store insulin according to current U.S. guidance
Use the storage, expiry, and in-use period on the exact product label. Insulin products and presentations differ, and the FDA guidance below is U.S.-specific; labels and recommendations can differ in other countries.

Unopened insulin and supplied vials or cartridges
The FDA says unopened insulin stored in the refrigerator as directed remains effective until the package expiration date. Its guidance summarizes manufacturer labeling that supplied vials or cartridges may be kept at 59°F–86°F (15°C–30°C) for up to 28 days, subject to the exact product instructions. Diluted insulin or insulin removed from its original vial has a shorter stated period.
Heat, freezing, and insulin pumps
The FDA warns that effectiveness declines with longer exposure to extreme heat or cold, advises keeping insulin as cool as possible without freezing it, and says not to use insulin that has been frozen. Pump insulin has separate handling instructions: the FDA says to discard it if exposed above 98.6°F (37°C). Check the pump and insulin instructions for any additional limits.

If insulin was exposed to an uncertain temperature
Do not use the Cochrane findings as permission to exceed your product’s storage instructions. The FDA advises replacing vials exposed to extreme temperatures as soon as properly stored insulin is available. For a specific product or exposure, check the package insert and contact the manufacturer, pharmacist, or clinician. If an emergency requires a decision before replacement insulin is available, closely monitor blood glucose and seek medical follow-up.
How to think about “How long can insulin be out of the fridge?”
There is no universal answer that applies to every insulin. The FDA’s 28-day room-temperature figure applies to supplied vials or cartridges according to manufacturer labeling, within the stated 59°F–86°F (15°C–30°C) range; it is not a blanket allowance for every pen, pump reservoir, formulation, or exposure. Check the product’s in-use period, which may begin when it is opened or first used.

When assessing an exposure, the relevant details are the insulin brand and formulation, container type, whether it was opened, the temperature reached, how long it was exposed, and the product’s in-use limit. A brief moderate deviation is not equivalent to prolonged extreme heat or freezing, but the available evidence does not establish a universal threshold for deciding whether a particular supply remains suitable.
Travelling with insulin
Keep insulin away from direct sunlight and hot cars, protect it from freezing, and follow the product’s temperature limits while travelling. The ADA advises avoiding extreme heat and cold and says insulin kept at room temperature lasts approximately one month; the exact product label controls. MedlinePlus also emphasizes label-specific storage and limited in-use periods, including additional considerations for pump use.
A travel cooler can help keep insulin cool, but it must not freeze the medicine. No source cited here establishes how long a particular commercial cooler maintains a suitable temperature or that a cooler can restore insulin after an excursion. If you use one, check its actual temperature performance rather than assuming it is suitable.

The evidence challenges the assumption that every tested human insulin rapidly loses potency in warm conditions. It does not establish that all insulin can tolerate the same temperatures, that potency measurements guarantee clinical outcomes, or that products remain sterile and suitable after every excursion. Product-specific directions remain the practical standard while further clinical evidence is needed.
