Short answer: Let Sarah stay if you want the Haunted House follow-up and a shared-home outcome with Corinne. Make her leave if you want to see Sarah befriend Johnny later. Either choice frees Corinne and advances Novigrad Dreaming; the decision changes optional story content, not the main quest’s outcome.
Spoilers: The sections below describe both branches and later appearances.
What does the Sarah choice change?
In the main quest Novigrad Dreaming, Geralt visits a house in Novigrad while searching for the dreamer Corinne Tilly, whose visions may help him find Ciri. He discovers that Sarah, a godling, has been disturbing the house and keeping Corinne in a nightmare. At the oven, Geralt can negotiate with Sarah or threaten to drive her out.

It determines whether Sarah stays in the house and unlocks a follow-up with Corinne, or leaves and later befriends Johnny.

Both choices let Corinne wake and move the main quest forward. The outcomes above are described in the Gamer Guides walkthrough, the Prima Games walkthrough, and The Witcher 3 Wiki quest entry.
When should you decide?
Make your choice before leaving the house and speaking to Rudolf. Prima Games says the decision is irreversible at that point and influences a conversation during the main quest Bald Mountain. If you care about which branch you get, settle the encounter before exiting.

How do you drive Sarah out?
If you choose expulsion, threaten Sarah. Burdock is needed to complete the eviction through the oven if you do not already have it. The Gamer Guides walkthrough and Prima Games walkthrough say you can obtain it from a nearby herbalist or herb merchant when necessary. This is an in-game quest item.

Which option should you choose?
- Let her stay for the Haunted House quest and the outcome where Sarah and Corinne live together.
- Make her leave to see Sarah’s later friendship with Johnny and appearance during Bald Mountain.
- Choose according to your preferred role-play: negotiation and a shared home, or a firm eviction.
There is no objectively best ending established for this choice. It is a preference between optional narrative outcomes.
