UK guidance encourages clinicians to consider an inhaler’s environmental impact when choosing among devices that are already clinically suitable. It does not tell everyone with asthma to switch: asthma control, correct technique, and the person’s preference come first. Talk with a GP, respiratory nurse, or pharmacist before changing or stopping a prescribed inhaler.
What is a lower-carbon inhaler?
Pressurised metered-dose inhalers (pMDIs) use propellants associated with greenhouse-gas emissions. Dry powder inhalers (DPIs) and soft mist inhalers (SMIs) do not use those propellants and are generally described as lower-carbon options. This is a comparison by device type, not a universal footprint for every medicine, inhaler, or patient. The 2024 BTS/NICE/SIGN guideline and Asthma + Lung UK’s patient guidance explain the options.
The joint patient decision aid says switching to a DPI offers the greatest climate benefit among the options it compares. Keeping the same inhaler type while reducing its environmental impact offers some benefit, but less. These environmental comparisons do not establish that a particular device is right for you.

Who should consider changing, and how is the choice made?
The 2024 BTS/NICE/SIGN guideline says inhaler choice should take account of four things: correct technique, the person’s preference, the lowest environmental impact among suitable devices, and an integral dose counter. “Among suitable devices” is important: environmental impact is one consideration after clinical suitability, not a reason to select a device that cannot be used properly.

- Asthma control and the prescribed medicine: A different device must still be appropriate for the person’s treatment.
- Ability to use it: The person should be able to inhale correctly and consistently with the device.
- Preference: The person’s views are part of the guideline’s choice criteria.
- Dose counter: The guideline includes an integral counter among the criteria to consider.
- Environmental impact: Compare the impact of devices that are otherwise suitable.
If a device is changed, the guideline says to observe the person’s technique and find an alternative if they cannot use it properly. Do not stop using or switch prescribed inhalers without first speaking to a healthcare professional; the patient decision aid states, “Do not stop using the inhalers you already have without talking to your healthcare professional.”
Why might a dry powder inhaler not suit everyone?
A DPI is breath-actuated: the user must inhale strongly enough to draw the medicine in. Some children, older people, and people with more severe lung conditions may find this difficult, particularly when symptoms are bad. A lower-carbon label alone cannot show whether someone can use a specific device effectively. If a clinician recommends a change, ask them or a pharmacist to demonstrate the technique and check that you can manage it.

What if you use a metered-dose inhaler or spacer?
The guideline says a spacer should usually be prescribed with a metered-dose inhaler, particularly for children. Ask a clinician or pharmacist whether one is appropriate for your prescription and how to use it. A spacer is an accessory, not an alternative to prescribed treatment, and it does not change the inhaler’s carbon impact.
Where should used or expired inhalers go?
NICE recommends pharmacy disposal of used or expired inhalers, and NHS England guidance encourages returning them to a pharmacy. Check with your local pharmacy about its arrangements. NHS England’s greener NHS information provides broader context on the health service’s environmental work.

What does the NHS carbon-footprint figure mean?
NHS England estimates that inhalers account for approximately 3% of the NHS carbon footprint; the source page does not state a year for that figure. It is an organizational estimate, not a measure of an individual patient’s emissions or the footprint of a particular inhaler.
