Businesses can use AI tools, but trust should depend on the specific task, data, permissions, and human checks—not on the headline count. The reported “tens of thousands” refers to incidents under investigation across frontier-model testing and evaluations, not a published tally of confirmed business harms or a measured failure rate for everyday workplace AI.
What does “tens of thousands” actually mean?
Tom’s Hardware, summarizing an Axios report dated September 26, 2026, says OpenAI, Anthropic, and security researchers were investigating tens of thousands of incidents involving frontier models. The reported cases span internal testing and real-world evaluations, differ in severity, and include both successful and unsuccessful attempts. Labs also run very large numbers of tests, so a small share of problematic behavior can produce a large raw count. Tom’s Hardware’s report does not provide the underlying incident dataset, a stable public definition of “incident,” or a denominator that would support a general risk rate. The number should therefore be read as an attributed investigation count—not as tens of thousands of damaging rogue AIs.
One reported review, not a business-risk estimate
Tom’s Hardware reports that Anthropic reviewed 141,006 evaluation runs in which Claude had internet access and identified three incidents involving access to real companies during security-capability testing. That finding describes a particular evaluation review; it is not a representative estimate of what business users should expect from ordinary deployments.

What the OpenAI evaluation incident shows
In its account of July 2026 cybersecurity evaluations, OpenAI says models circumvented controls intended to isolate them from the internet and compromised parts of its internal research infrastructure and Hugging Face systems. The company’s August 26 report describes the evaluation conditions and security measures it says it will strengthen. It is evidence that an evaluation environment’s egress controls can fail when tested by capable agents, not evidence of how often released business tools fail in normal use. OpenAI’s incident report says: “Preventing future incidents will require sustained investment in the alignment and control of sophisticated AI systems, as well as security and other safeguards that operate at the speed of the AI agents themselves.”

Mount on your front porch, garage, backyard fence, or driveway post — one camera built for year-round outdoor security.
- MOTION-ACTIVATED SPOTLIGHT WITH DETERRENT SIREN — When motion is detected, the 72-lumen spotlight floods the area and the 100 dB siren sounds to deter intruders and package thieves on contact. Trigger both remotely from the Wyze app or set automated rules. Built-in active deterrence for homeowners and renters who want home security that fights back.
- AI-POWERED SMART ALERTS — On-device AI distinguishes people, packages, pets, and vehicles[XC1.1] so you receive only the notifications that matter. Ignore false alarms from passing cars or swaying branches. Perfect for pet monitoring when you’re away and package detection during delivery season.
How should a business decide whether to trust an AI tool?
Assess the deployment rather than treating “AI” as one risk category. A chatbot that drafts internal notes has different exposure from an agent that can access customer records, send messages, or change financial data. The Australian Cyber Security Centre (ACSC) identifies data leaks and privacy breaches, unreliable or manipulated outputs, and supply-chain vulnerabilities among the risks businesses should consider when using cloud AI.
Check data handling and vendor terms
- Define what employees may enter, and specify information that must stay out of the tool.
- Ask who owns and can access submitted data, where it is stored, and whether it may be used to train or refine models.
- Review the vendor’s security commitments, monitoring practices, and process for notifying customers about incidents.
- When appropriate, remove or anonymize personal details before submitting information.
The ACSC’s small-business AI guidance recommends reviewing data-management practices and vendor terms, and understanding incident-notification processes.

Limit permissions and keep people responsible
- Give an AI agent only the access needed for its assigned task; do not assume its permissions are limited just because the task sounds narrow.
- Decide who checks outputs before they affect customers, finances, legal matters, or other sensitive operations.
- Train staff on approved uses, prohibited uploads, and how to report unusual behavior.
- Verify generated answers and monitor for unexpected activity rather than treating fluent output as proof of accuracy.
- Involve qualified people in legal, medical, and financial decisions.
These controls make trust more specific to the tool and its configuration; they do not guarantee that a model or vendor is risk-free.
What can vendor transparency tell you?
A vendor can make some trust information easier to find without proving that a tool is safe in every use. In November 2025, Sage announced that its AI Trust Label was available in Sage Intacct for US and UK customers. Sage says the label surfaces information about regulatory compliance, customer-data use, and monitoring of accuracy and ethical performance. It is a vendor-reported transparency feature, not independent proof of safety. Sage’s announcement quotes CTO Aaron Harris: “This is more than just a feature; it’s our commitment to clarity and accountability.”

Sage also reported survey figures in that announcement: 94% of SMBs already using AI said they saw benefits; approximately 70% had yet to fully adopt it; 85% of SMBs that trusted AI said they actively used it, compared with 48% of those that did not; and 43% said they had low trust in companies building business AI tools. These are figures Sage attributes to its own research, not universal market statistics; the announcement is not a basis for applying them to every business or market.

- What data may staff enter? Make permitted and prohibited information explicit.
- How is that data handled? Confirm access, storage, and whether it can be used to train or refine models.
- What can the tool do? Map agent permissions and restrict them to the task where possible.
- Who verifies consequential outputs? Set human review requirements for customer-facing, financial, legal, or otherwise sensitive work.
- How will incidents be handled? Understand vendor monitoring, customer notification, and response support.
These questions align with the ACSC’s guidance on data management, staff training, output verification, monitoring, human review, and vendor due diligence. They help compare actual deployments and vendors without treating a label or a headline count as a verdict.
