Ractigen Therapeutics reported that RAG-18 increased utrophin signal at the muscle-cell membrane in paired biopsies from three boys with Duchenne muscular dystrophy (DMD). The company presented the first-in-human Cohort 1 findings at the World Muscle Society (WMS) 2026 congress on October 3. This is an early human target-engagement result—not proof that RAG-18 improves or preserves patients’ health or function.

What is RAG-18?

RAG-18 is an investigational small activating RNA (saRNA) designed to target regulatory regions of the human UTRN gene and increase production of utrophin. Ractigen says its Lipid-Conjugated Oligonucleotide (LiCO) technology delivers the candidate. The company describes the approach as using the body’s transcription machinery rather than a viral vector or permanent DNA editing.

Utrophin is related to dystrophin, the protein that is deficient or absent in DMD. The biological rationale is that raising utrophin might help compensate for missing dystrophin. Because RAG-18 targets UTRN, not a specific DMD mutation, the approach is intended to be mutation-independent. That rationale does not show that the drug benefits people across genotypes.

What did Ractigen report at WMS 2026?

Ractigen described an ongoing, open-label, dose-escalation Phase I study (NCT07282652) in ambulatory boys aged 4–15 with genetically confirmed DMD. The reported first cohort comprised three participants who received 15 mg by monthly intravenous infusion. The company reported data through Day 169, including paired muscle biopsies, imaging, spirometry and motor assessments. The results below are sponsor-reported; the presentation announcement did not identify an independent statistical analysis.

Muscle biopsies showed increased utrophin signal

At Day 113, the company compared paired biopsies from opposite sides of the body using whole-field quantitative immunofluorescence. Ractigen reported a 3.5- to 5.3-fold increase in sarcolemmal utrophin signal density in mature muscle fibers and a 4.1- to 4.7-fold increase in regenerating fibers compared with baseline. Co-staining was reported to show the signal at the sarcolemma, the muscle-fiber membrane.

This is the central evidence behind the company’s “clinical proof-of-mechanism” description: a change in the intended biological target in human muscle. It does not establish that the change produces a meaningful clinical benefit.

Other tissue and imaging observations

At Day 113, Ractigen reported a 5% to 15% increase in mean muscle-fiber cross-sectional area and increases in mean fiber diameter ranging from 5.34 μm to 27.50 μm. It also reported a 3% to 10% decrease in muscle fat fraction and no drug-induced muscle-cell death or inflammation. These are tissue observations from a very small cohort, not confirmed clinical efficacy outcomes.

By Day 169, quantitative thigh-muscle MRI reportedly showed T2 relaxation-time reductions of up to 11.6%. The company interpreted the reductions as consistent with less active edema or inflammation; the figures do not by themselves establish why the MRI measure changed or whether patients felt or functioned better.

Safety observations

Ractigen reported no dose-limiting toxicities, serious adverse events, or treatment-emergent adverse events of Grade 3 or higher among the three participants. Reported events were described as mild (Grade 1–2), transient and resolving without medical intervention. The company also reported no dose interruptions, reductions or discontinuations. Three participants cannot establish a definitive safety profile or rule out uncommon or longer-term harms.

Pulmonary and cardiac measures

Over 24 weeks, the company reported numerical increases in percent-predicted forced vital capacity (FVC) ranging from 2.8% to 41.0% and in forced expiratory volume in one second (FEV1) ranging from 2.8% to 34.0% across the three participants. Left ventricular ejection fraction remained within the normal range, at or above 55%, in all three over that period. These are descriptive observations, not controlled estimates of a treatment effect.

Motor results were mixed

Ractigen reported six-minute walk distance changes ranging from a gain of 36.5 m in one early-ambulatory participant to a decline of 62.5 m in another; a later-ambulatory participant’s distance was nearly maintained, changing by −1.0 m. The company also said that later-ambulatory participant improved on the four-stair climb. North Star Ambulatory Assessment (NSAA) scores declined by 3 points in each of the three boys.

These results do not support a general claim that motor function improved. With three participants, no control group and differing individual results, the measures are exploratory and cannot show whether RAG-18 caused a change in function.

What does “clinical proof-of-mechanism” establish?

Here, the phrase refers to evidence that the investigational drug may have engaged its intended biological target in people: the biopsy signal for utrophin increased at the muscle-fiber membrane after treatment. It is distinct from proof of clinical efficacy, which would require evidence that treatment improves outcomes that matter to patients, such as movement, breathing or disease progression.

The study was open-label and dose-escalating, and the reported first cohort had only three participants. The announcement did not report a randomized comparator or a controlled estimate of efficacy. The biopsy and other measures therefore provide an early signal to investigate, not a basis for concluding that RAG-18 works, restores muscle function or is safe for broader use.

Is RAG-18 approved, and what is the study status?

RAG-18 is described as investigational, not as an approved treatment available to patients. Ractigen has said the candidate received U.S. Food and Drug Administration orphan drug and rare pediatric disease designations in 2024; those designations are not marketing authorization and do not demonstrate efficacy.

At the time of its October 3, 2026 announcement, Ractigen said Cohort 2, using 30 mg, was fully enrolled and safety follow-up was continuing. The WMS findings were presented as a company announcement; an independently assessed or peer-reviewed results report is not established by the material available here.
