PARO is a seal-shaped therapeutic robot designed to encourage interaction and care activities, including for people with dementia. Studies suggest it may help with some outcomes, such as agitation, anxiety, or caregiver burden in particular settings. But the evidence is limited, results vary by measure, and PARO is a tool for care—not a replacement for human support.
What is the PARO robot used for?
PARO is a physical companion robot shaped like a baby seal, developed by Japan’s National Institute of Advanced Industrial Science and Technology (AIST). It moves its flippers and neck, cannot crawl, and responds to people’s actions. Care teams use it to encourage contact and engagement, either in enjoyable activities or in more goal-directed therapy.
The seal shape was a deliberate design choice, not just a cute exterior. PARO developer Takanori Shibata wrote that designers considered several animal forms and chose a seal because it was unfamiliar to users yet readily accepted. The form gives people a gentle, approachable object to touch and interact with, without asking them to relate to a realistic humanlike machine. Shibata’s 2014 paper on PARO’s design and use also reported that it became a Class II neurological therapeutic biofeedback medical device approved by the U.S. FDA in 2009. That historical report is not confirmation of current regulatory status in every jurisdiction, nor proof that every claimed clinical outcome has been validated.

Does PARO help people with dementia?
Evidence points to possible benefits, but it does not establish one dependable effect for every person, symptom, or care setting. The clearest broad view comes from a 2023 systematic review and meta-analysis covering 12 articles and 1,461 older adults with dementia. The authors graded the overall evidence as low quality because of methodological limitations, small samples, and wide confidence intervals.

For the outcomes it pooled, the review reported a moderate effect on medication use and small effects on anxiety, agitation, and depression. It found a negligible effect on total sleep time. Its narrative synthesis also suggested reduced apathy and increased sociability, but those findings should not be treated as equivalent to the pooled estimates. Read the 2023 systematic review.
| Outcome in the 2023 review | Reported pooled effect | How to read it |
|---|---|---|
| Medication use | Moderate effect (SMD −0.63) | A pooled result across included studies; it does not establish that a particular medication can be reduced for an individual. |
| Anxiety | Small effect (SMD −0.17) | Suggests a limited average effect across the review’s studies. |
| Agitation | Small effect (SMD −0.27) | Does not mean agitation will improve for every user. |
| Depression | Small effect (SMD −0.40) | Indicates a small pooled effect, not treatment for depression. |
| Total sleep time | Negligible effect (SMD −0.12) | The review did not find a meaningful overall effect on this measure. |
What newer studies add—and what they do not show
A group intervention for mild dementia
A 2024 randomized trial recruited adults aged 65 or older with mild dementia from four day-care centers. Its abstract reports significant time-by-group interactions in measures including cognition, loneliness, depressive symptoms, and well-being. Psychological measures continued to improve at one-month follow-up, while physiological changes seen at week six had diminished by week ten. These results concern that specific group intervention; they should not be generalized to every person with dementia or to unsupervised use. See the 2024 trial record.

Self-directed sessions in Japanese group homes
A 2026 cluster-randomized study in six Japanese group homes compared self-directed PARO sessions lasting one hour, once or three times weekly for one month. Among 85 analyzed participants, the more frequent-use group had a statistically significant reduction in caregiver burden relative to the once-weekly group: least-squares mean difference −3.29 (95% CI −6.26 to −0.32; p = 0.030). The between-group difference in behavioral and psychological symptom severity was not statistically significant: adjusted least-squares mean −1.98 (95% CI −4.10 to 0.15; p = 0.068).
Because both groups received PARO and there was no no-PARO group, this study compares session frequency; it does not show that PARO is better than no intervention. Read the full 2026 group-home study.

A small trial focused on chronic pain
A 2020 pilot randomized trial in three Australian long-term-care facilities studied 43 people with dementia and chronic pain. Participants in the PARO group received individual, non-facilitated 30-minute sessions five days a week for six weeks. The study found lower observed pain behaviors and fewer as-needed medications compared with usual care. It found no significant differences in staff-rated pain, agitation, anxiety, depression, or regularly scheduled medication. The authors called for larger, longer trials, so these results are promising but preliminary. See the 2020 pain trial.
Care staff remain central
A 2015 Dutch quasi-experimental study involved 91 residents with dementia and 106 tailored interventions; 71 participants completed the study. It reported a significant effect for the combined interventions, but used a within-subject ABAB design rather than a large randomized comparison. The authors’ practical conclusion was direct: “Paro should be seen as a tool for care staff and not as a replacement of care.” Read the 2015 study.

What to consider before using PARO in care
The studies do not support treating PARO as a universal intervention. Whether it is useful depends on the person, the goal, how sessions are delivered, and what outcome is being tracked. A sensible care-team discussion can begin with these checks:

- Set a specific goal. Decide whether the aim is engagement, social interaction, agitation, pain-related behavior, or another outcome. A change in one measure does not imply improvement in others.
- Respect the person’s response. The robot is intended to invite interaction; it should not be forced on someone who dislikes it or finds it distressing.
- Plan staff involvement. Some studies used facilitated or tailored activities, while the Japanese group-home study examined self-directed sessions. Those approaches are not interchangeable.
- Record what happens. Note session duration and frequency, the person’s engagement, and the relevant outcome. Available evidence does not establish a single schedule that works for everyone.
- Keep ordinary care in place. PARO can support an activity, but the evidence does not establish it as a substitute for staff attention, clinical assessment, medication decisions, or other care.
