In a phase I study, all nine people with high-risk clear cell kidney cancer who received a personalized vaccine were reported recurrence-free at a median of 40.2 months after surgery. The finding is encouraging, but this small, uncontrolled trial cannot show that the vaccine prevented recurrence. It was an experimental treatment made for study participants, not an established or commercially available vaccine.
What the trial tested
The phase I study, registered as NCT02950766, enrolled people with high-risk stage III or IV clear cell renal cell carcinoma after their tumours had been completely removed. Researchers designed a vaccine for each participant using predicted neoantigens—features arising from mutations in that person’s tumour that could be recognized by the immune system.
A vaccine was manufactured and administered for all nine participants. Researchers synthesized a median of 15 neoantigen-containing peptides per participant. Five people also received ipilimumab near the vaccination site; the other four received the vaccine without it. This was not a randomized comparison of the two approaches.
What the researchers observed
At a median of 40.2 months after surgery, none of the nine participants had a reported recurrence. The study also found vaccine-specific immune responses in all nine participants, and measured T-cell reactivity against each participant’s own tumour in seven. Researchers reported expansion of peripheral T-cell clones and responses to vaccine antigens that included selected kidney-cancer driver mutations.
The immune findings show that the vaccines prompted measurable activity in the participants’ immune systems. They do not establish that this activity caused the recurrence-free observation or that vaccination improves recurrence or survival outcomes.
Side effects reported in the study
All participants experienced low-grade reactions at the injection site, and eight of nine had transient flu-like symptoms. No dose-limiting toxicities were observed. With only nine participants, however, the study is too small to characterize uncommon or less predictable harms or to define the treatment’s full safety profile.

What “none had a recurrence” does—and does not—mean
The result describes what happened to nine people in an early-phase study; it does not tell us what would have happened to the same people without the vaccine. There was no randomized control group, and the study’s authors said direct comparisons with randomized phase III adjuvant trials were not possible. The finding therefore cannot prove that the vaccine prevented recurrence, establish its effectiveness, or show that the same result would apply to other patients.

Is the personalized vaccine available as a treatment?
The study describes a vaccine designed and manufactured for its participants. It does not establish a retail treatment or show that this vaccine is available for routine care. The results support further study of personalized vaccines in kidney cancer, but they are not evidence that a patient should replace or delay care recommended by their oncology team.
