No established nasal insulin gel is shown by the available sources to be an approved, currently available replacement for prescribed insulin injections. Researchers have studied insulin delivered through the nose, including gel formulations, but the clinical evidence is limited and raises concerns about nasal side effects. Do not stop or change prescribed insulin without guidance from your diabetes-care clinician.
What is nasal insulin gel?
Nasal insulin gel is an experimental way of delivering insulin through the lining of the nose rather than by injection. A 2005 formulation study used carbopol and hydroxypropyl methylcellulose to make a gel and assessed insulin release in laboratory testing, as well as effects in animals and healthy volunteers. Those findings describe formulation research; they do not establish a marketed treatment or prove that nasal gel can replace routine insulin therapy. The 2005 study
The paper’s authors described intranasal gel insulin as a “pleasant and painless alternative to injectable insulin.” That statement reflects their early research, not a current clinical recommendation or regulatory finding.
What did clinical testing find?
A small crossover study published in 2001 evaluated nasal insulin gel and subcutaneous insulin in 16 adults with type 1 diabetes over six months. At month six, the study reported HbA1c of 8.3% during the nasal period and 8.6% during the injection period; the difference was not statistically significant. The small, historical trial does not establish that the treatments are equivalent or that nasal insulin is a safe substitute for people generally. Lalej-Bennis et al., 2001

Tolerability was a notable problem: four participants withdrew because of nasal burning or persistent sinusitis, and another participant had purulent sinusitis at month six. The authors said that other formulations should be investigated to improve nasal tolerance and bioavailability.
Is Nasulin a nasal insulin product you can use?
Nasulin appears in pharmacokinetic research as an intranasal recombinant human insulin spray containing an absorption enhancer. The 2010 paper discusses clinical development; it does not establish Nasulin as a currently marketed consumer alternative. The evidence cited here does not identify an available nasal insulin gel or spray that can replace prescribed insulin.

How is nasal glucagon different?
Baqsimi is nasal glucagon powder, not insulin. Its labeled purpose is treatment of severe hypoglycemia, and it is supplied in a single-use intranasal device. The FDA describes inhaled insulin as an insulin delivery route, which is distinct from experimental nasal gel. Whether any treatment route is appropriate depends on the person and the specific medicine; the FDA’s consumer information does not make nasal insulin gel an available replacement.
