A 2013 study found manganese enriched on the surfaces of analyzed Mount Etna volcanic fragments, and its authors suggested this might help explain reports of manganese in local well water. The idea that some Parkinson’s cases in eastern Sicily were actually manganism remains a hypothesis: the study did not examine patients, measure anyone’s exposure, or establish that volcanic material caused neurological disease.

What the Mount Etna study measured

Domenico Andrea Cristaldi, Cosimo G. Fortuna and Antonino Gulino examined lapilli—small volcanic fragments—from Mount Etna activity in April 2012. Their 2013 paper, “A photoelectron spectroscopy study of lava stones”, used X-ray photoelectron spectroscopy (XPS) to analyze the fragments’ near-surface chemistry. XPS examines only the outer few nanometres, not the full interior composition of a rock.

The authors reported that silicon, iron, calcium and potassium were concentrated in the bulk, while aluminium, sodium, phosphorus and manganese were concentrated at the surface. In the analyzed material, the surface manganese concentration was 266.7% of the bulk value; the manganese was associated with water-soluble manganese oxyhydroxide (MnOOH). This is a finding about those fragments and that measurement method—not a measure of manganese in drinking water or in people.

How the water measurements fit—and what they do not show

A separate line of evidence came from Etna-well water samples collected in 2010. In its 5 June 2013 account, Chemistry World reported manganese concentrations of up to 2600 µg/L and compared them with an Italian legislative limit of 50 µg/L. Both figures describe what that historical account reported; they are not current measurements or confirmation of today’s legal standard.

  • ✔ HIGHEST QUALITY: ITS, manufactures test strip products in their USA facility in an environment that allows for reproducibility, accuracy, and traceability based on Good Manufacturing Practices.

Chemistry World also reported that roughly 1.5 million people were supplied with water from Etna wells. That is a reported supply population, not a count of people who drank water with elevated manganese, experienced a particular exposure, or developed illness.

The lapilli analysis and the well-water measurements are distinct findings. Surface enrichment makes it reasonable to ask whether volcanic material could release manganese during weathering, but the cited evidence does not quantify how much entered groundwater or establish that the analyzed fragments caused the reported water concentrations.

Why manganism was raised as a possibility

Manganism is a neurological condition associated with manganese exposure, and it can share motor symptoms with Parkinson’s disease. That symptom overlap helps explain why the authors’ suggestion attracted attention. It does not mean the conditions are interchangeable, or that symptoms alone can determine an individual’s diagnosis.

The 2013 coverage presented the possible link to Parkinson’s cases in eastern Sicily as a suggestion and said further studies were needed. A 2016 comment on the study likewise described a possible local manganism risk as a hypothesis: Nicoletti and Zappia’s comment in Analytical Methods.

Was anyone near Mount Etna misdiagnosed?

The available evidence does not establish that local patients were misdiagnosed. The 2013 study analyzed volcanic-fragment surfaces; it did not test patients, review their diagnoses, measure their personal manganese exposure, or demonstrate that exposure caused neurological symptoms. The cited sources also do not establish a confirmed local manganism outbreak or a causal chain from Etna lava through groundwater and human exposure to disease.

Study author Antonino Gulino told Chemistry World: “I hope that our paper could be a seed for similar studies and that scientists do not assume bulk studies of magmatic materials are sufficient to fully investigate their health implications.” That statement captures the paper’s contribution: a reason to investigate surface chemistry, not a clinical conclusion.
