Multifactor authentication (MFA) is showing up explicitly in some cyber-insurance application materials for healthcare businesses. That makes it a control medical practices should be ready to describe accurately—not proof that every insurer requires MFA, that it alone determines eligibility, or that enabling it guarantees coverage or a lower premium.

Why MFA is becoming an underwriting question

Cyber insurers assess how an applicant manages risk, and some application materials now ask about MFA by account type. Coalition’s application asks whether MFA is enforced for email, remote access such as VPN or remote desktop, and privileged network or cloud accounts. It also asks about other security controls and risks, including encryption, backups, and prior incidents. Coalition’s application guidance describes one insurer’s process, not the whole market.

Travelers’ CyberRisk applications and forms page lists a Multi-Factor Authentication Supplement and a Healthcare & Managed Care Supplement. Their presence shows that MFA and healthcare-specific information appear in another carrier’s application materials; it does not establish that every medical applicant must submit both forms or meet one uniform MFA threshold.

Travelers describes cyber liability as an available add-on for medical offices and says data-security measures may be among the factors considered in insurance costs. That is the insurer’s product description, not a universal pricing formula. The available evidence does not establish a market-wide MFA discount or how much MFA affects any particular practice’s premium. Travelers’ medical-office coverage page provides its product context.

What MFA does—and what it does not do

MFA asks a user to prove identity with more than one factor, rather than relying on a password alone. For a medical practice, the relevant question is not simply whether MFA is “on,” but which users and systems it protects, and where exceptions remain.

HHS and CISA include MFA for remote email and remote access among practices for mitigating ransomware and other threats. Their healthcare guidance also covers backups, workforce education, endpoint protection, patch management, and incident response. MFA is one layer in a broader security program, not a standalone cure. See Health Industry Cybersecurity Practices: Threat 2, Ransomware Attack.

NIST SP 800-66 Rev. 2, published February 14, 2024, helps organizations implement the HIPAA Security Rule and manage risks to electronic protected health information (ePHI). NIST explains that regulated entities must protect ePHI against reasonably anticipated threats, hazards, and impermissible uses or disclosures. Enabling MFA by itself does not make a practice HIPAA compliant. Read NIST SP 800-66 Rev. 2.

Submit answers that match actual deployment, retain the completed application, and ask a licensed broker how the application’s representations relate to the policy’s conditions and claims process. Do not install a product solely to check an insurance box. A hardware security key can be one MFA option if supported, but compatibility depends on the practice’s identity provider, email, EHR, and remote-access tools.

How to assess a cyber policy for a medical practice

An application answer is not a substitute for the policy wording. The NAIC notes that cyber-insurance policies are highly customized, so compare the terms that matter to the practice rather than assuming a standard package. The NAIC’s cybersecurity insurance overview offers general context.

Why vendor outages belong in the discussion

A practice can face serious operational disruption even when the problem begins at an outside service provider. The 2024 Change Healthcare incident affected workflows involving claims, payments, and eligibility verification. In figures reported by the American Medical Association in 2024, 36% of surveyed respondents said claim payments were suspended, 32% said they could not submit claims, and 22% said they could not verify eligibility for benefits. These are survey responses, not estimates of all U.S. practices. The AMA’s account of the Change Healthcare cyberattack provides the figures and context.

Inventory clearinghouses, EHR and practice-management providers, cloud email, payment processors, and vendors with remote access. For each critical dependency, identify how the practice would continue work if the service were unavailable. Then ask the broker what the policy says about dependent business interruption, the applicable waiting period, and an outage without a direct breach at the practice. Coverage depends on the policy wording.

New York’s Department of Financial Services issued guidance in 2024 addressing provider difficulties with eligibility verification and timely claim submission after the Change Healthcare disruption.
