Most short-term effects of supervised ketamine treatment—such as sleepiness, nausea, dizziness, dissociation, altered perception, headache, fatigue, anxiety, or temporary blood-pressure and heart-rate changes—wear off with observation. They are possible effects, not a guarantee that a symptom is harmless. Trouble breathing, collapse, a seizure, inability to wake, severe allergic symptoms, chest pain, or rapidly worsening illness needs emergency help.
The response depends on the medicine (ketamine injection versus esketamine nasal spray), route, dose, indication, and setting. Use the instructions from the clinician who administered your treatment rather than applying advice for a different product.
First decide whether this is an emergency
Call your local emergency number immediately for severe or worsening breathing difficulty, collapse or fainting, a seizure, inability to awaken, or any rapidly deteriorating condition. Do not drive yourself. Emergency numbers and transport pathways differ by country, so use the number for your location.

Possible short-term effects after ketamine injection
Ketamine injection is given intravenously or intramuscularly by a specially trained health professional, with monitoring during administration and for a period afterward. The MedlinePlus ketamine injection information (revised June 15, 2026) lists injection-site pain, nausea or vomiting, and drowsiness as effects that can occur. An Oxford Health NHS Foundation Trust service leaflet (version 3.1, March 10, 2026) describes additional experiences in that service’s monitored treatment context.

The NHS leaflet’s descriptions apply to that service and should not be treated as universal frequencies or durations for every dose, indication, or formulation. No single percentage reliably describes side effects across all ketamine treatments.
When an injection reaction needs urgent or emergency help
Call emergency services now
- Severe difficulty breathing or swallowing, collapse, or inability to awaken.
- A seizure or uncontrolled movements with loss of awareness.
- Signs of a serious allergic reaction: hives together with breathing or swallowing difficulty, dizziness, or swelling of the lips, tongue, throat, or eyes.
- Chest symptoms or a rapidly worsening, irregular heartbeat.
These warning signs are identified in the MedlinePlus ketamine injection guidance. If the person is unconscious or having a seizure, protect them from injury, do not put anything in their mouth, and follow the emergency dispatcher’s instructions.

Contact the treating clinician urgently
MedlinePlus also tells patients to seek medical attention for confusion, agitation, hallucinations, uncontrolled muscle movements, difficulty urinating, and symptoms that could indicate liver injury. A clinician must decide whether a symptom is a transient drug effect, a complication, or an unrelated emergency. Do not take another dose or use another sedating substance while waiting for advice unless your prescriber specifically instructs you to do so.
Esketamine nasal spray is a different monitored treatment
Esketamine nasal spray is not interchangeable with ketamine injection. The MedlinePlus esketamine information describes administration in a medical facility, with monitoring before, during, and for at least two hours after each treatment. The same page highlights sedation, dissociation, breathing problems, and temporary blood-pressure increases. Follow the facility’s discharge criteria and arrange the required ride or supervision rather than assuming injection instructions apply.
Esketamine symptoms requiring immediate attention
Seek emergency medical treatment for breathing problems, extreme sleepiness, fainting, chest pain, shortness of breath, a sudden severe headache, vision changes, or a seizure. Tell facility staff immediately if any of these begin during the observation period. After discharge, call emergency services for severe or rapidly worsening symptoms; otherwise use the clinic’s urgent contact instructions.

Feeling sleepy or dissociated after monitored nasal treatment does not make extreme sleepiness, inability to wake, or breathing difficulty safe. Severity and progression determine the response.
Long-term and repeated-use risks are different from session effects
Repeated exposure can cause complications that do not resemble a brief change in perception during a supervised session. The FDA-approved ketamine injection label (2026) describes urinary and liver problems with long-term use or abuse. These risks require clinician assessment, not over-the-counter products or self-treatment.
Urinary-tract injury
The label reports cystitis, reduced bladder capacity, ureteral narrowing (stenosis), ureteral obstruction, and hydronephrosis. An obstruction can impair kidney function and may require emergency intervention. Warning symptoms can include severe or persistent pain with urination, markedly increased frequency or urgency, blood in the urine, difficulty starting or passing urine, inability to urinate, or severe lower-abdominal or flank pain.

The prescribing information gives this instruction: “If signs or symptoms of urinary obstruction or severe lower urinary tract symptoms are observed, discontinue ketamine and refer for urgent urological evaluation.” This means the prescribing clinician should stop ketamine and arrange urgent urology care; do not independently restart, taper, or substitute treatment.
Liver injury and sclerosing cholangitis
The FDA label also describes liver injury, including ketamine-induced sclerosing cholangitis, particularly as a concern with recurrent dosing. It calls for baseline liver-function tests for patients placed on recurrent-dosing treatment plans. If sclerosing cholangitis is suspected, ketamine should be stopped immediately and a specialist should evaluate the patient.

Contact your prescriber promptly for these symptoms; jaundice, severe abdominal pain, confusion, collapse, or rapid deterioration warrants emergency evaluation.

What to do after a treatment session
- Stay for the required observation. Do not leave an injection or esketamine facility until staff say your vital signs, alertness, and symptoms meet its discharge criteria.
- Use a responsible escort if the clinic requires one. Sedation and dissociation can impair judgment even when you feel ready to leave.
- Do not drive or operate machinery after ketamine injection for at least 24 hours. This is the interval specified by MedlinePlus; your clinician may impose a longer restriction or different rule for your medicine and circumstances.
- Follow the exact written plan. Keep the clinic’s after-hours number, medication list, and emergency instructions available. Do not transfer an instruction from injection treatment to esketamine nasal spray, or vice versa.
- Report persistence or change. A symptom that is severe, lasts longer than your clinician predicted, appears after you were discharged, or is getting worse deserves a call even if it is listed as a possible effect.
A practical severity check
When deciding what to do, classify the symptom along four axes: the formulation and route, when it started, how severe or progressive it is, and whether it involves breathing, consciousness, the heart, an allergic reaction, urination, or the liver.

Why the treatment context matters
Injection and esketamine nasal spray differ in route, monitoring requirements, dosing schedules, and approved information. A side-effect list from an Oxford NHS service cannot establish a universal rate for all ketamine care, and an instruction written for nasal treatment cannot automatically be applied to an injection. Tell every clinician which product you received, the date and dose if known, other medicines or substances taken, and when the symptom began.
There is no reliable single incidence figure covering every formulation, dose, indication, and setting. Safety decisions should therefore be based on the symptom and its progression, the official information for the exact product, and advice from the treating team—not on a percentage found for a different study or service.
