Apple Watch heart-rate readings are useful estimates for many everyday activities and steady workouts, but they are not guaranteed to match an ECG or a chest strap on every reading. A 2025 systematic review found little average bias across included measurements, yet its reported limits of agreement show that individual readings can differ by several beats per minute. Accuracy also depends on the activity, fit, measurement mode, and person.
What the Apple Watch heart-rate monitor measures
The watch’s everyday optical sensor uses photoplethysmography (PPG): green LEDs shine light into the wrist, and light-sensitive photodiodes detect changes associated with blood flow. Apple lists an optical heart-rate range of 30–210 beats per minute. This produces a pulse estimate; it is not an ECG recording. Apple’s heart-rate support page explains the sensor and its limitations.
On Apple Watch Series 4 and later, and Apple Watch Ultra models, electrodes in the Digital Crown and the back of the watch can take an electrical heart-rate reading when you place a finger on the crown while using the Heart Rate app. Apple says that reading is taken more frequently than an ordinary optical reading. The ECG app on supported models and in supported regions is a separate feature that records an electrocardiogram; it should not be treated as simply a more accurate version of continuous optical tracking.

Apple’s support page, published September 14, 2026, says Series 12 and Ultra 4 use green LEDs continuously during the day for readings as often as every five seconds, and infrared LEDs during a sleep schedule or Sleep Focus. Those are model-specific details, not a description of every Apple Watch generation.

How close are readings to a reference?
A 2025 living systematic review and meta-analysis in npj Digital Medicine reported a mean heart-rate bias of −0.27 bpm across included measurements (95% confidence interval −0.72 to 0.17). That small average underestimation does not mean every reading is within a fraction of a beat: the review’s limits of agreement were −7.19 to 6.64 bpm. In other words, average performance and the closeness of an individual reading are different questions. The review also identifies measurement conditions and individual physiology as factors that affect accuracy. Read the systematic review in npj Digital Medicine.
Exercise studies show why a single accuracy number cannot describe every use. In a 2017 controlled study, Gillinov and colleagues tested 50 healthy adults (mean age 38 ± 12 years; 54% female) during treadmill, stationary-bike, and elliptical exercise at several intensities. Against ECG, the tested Polar H7 chest strap had a Lin’s concordance correlation coefficient of 0.996, compared with 0.92 for the Apple Watch tested in that study across exercise conditions. The tested devices generally performed well on the treadmill; with arm levers on the elliptical, none reached acceptable agreement. The authors found performance varied by exercise type, with the highest accuracy on the treadmill and the lowest on the elliptical. Those figures describe an earlier Apple Watch generation and the specific study setup, not a benchmark for current models. See the 2017 exercise study.

A 2021 study of Apple Watch Series 5 compared wrist PPG with a Polar H10 chest strap and described the watch’s readings as acceptably accurate during treadmill exercise and free-living activity among healthy adult volunteers. That supports usefulness in those settings, not identical performance for every wearer, workout, or Apple Watch generation. Read the Series 5 study.

- Movement and exercise type: Arm motion can interfere with wrist-based optical tracking. The 2017 study found the tested devices struggled most during elliptical exercise involving arm levers.
- Fit and contact: Apple recommends a good fit and acknowledges that some people may not get a reliable reading every time. A loose or inconsistent fit can make the sensor’s job harder.
- Individual differences and conditions: The systematic review found that physiology and measurement conditions affect results, so study averages cannot guarantee accuracy for an individual.
- Measurement mode: An optical wrist estimate and the electrical reading taken with a finger on the Digital Crown use different sensing methods and sampling patterns.
- Occasional anomalies: Apple says users may sometimes see abnormally high or low measurements.
Apple puts the limitation plainly: “Even under ideal conditions, Apple Watch may not be able to get a reliable heart rate reading every time for everybody.”

Can you trust it during a workout?
For many steady activities, the Apple Watch can be useful for tracking workout trends and general effort. For workouts with substantial arm movement or changing intensity, an occasional lag, missing value, or unexpected number is more plausible. A single reading should not be treated as a definitive measure of what your heart is doing.
If precise heart-rate pacing matters to your training, an electrode chest strap is a reasonable alternative to consider. In the 2017 comparison, the tested Polar H7 agreed more closely with ECG than the tested Apple Watch across exercise conditions. That result applies to those devices and that study, not to every current strap or watch. Compare readings over the same time window and activity, and keep the reference method and device generation in mind.

What to do when a reading looks wrong
- Check the fit and contact. Make sure the watch is snug enough to maintain contact with your wrist, following Apple’s fit guidance.
- Consider what you were doing. Arm-heavy movement and changing exercise patterns can make optical readings less dependable than a steady activity.
- Use the appropriate measurement mode. For a spot electrical heart-rate reading, open the Heart Rate app and place a finger on the Digital Crown on a supported model. This is distinct from using the ECG app.
- Look for a pattern rather than relying on one number. An isolated unusual or missing reading can occur; do not use a consumer wearable reading alone to diagnose a health problem.
If an unusual reading concerns you or you have symptoms, seek appropriate clinical guidance rather than relying on the watch to assess your health.
