Inner speech and speaking aloud share language-planning machinery, but they are not the same brain event. Inner speech generates verbal content without intentional audible articulation. Speaking aloud adds motor commands to the larynx, tongue, lips and breathing muscles, then compares the resulting sound with auditory feedback. The brain also uses predictive self-monitoring to label both internally generated language and overt speech as self-produced, although the prediction is tested against real sound only when you vocalize.

What is the difference between inner speech and speaking aloud?

Inner speech is covert verbal cognition: you formulate words, phrases or conversations without deliberately producing an audible voice. It can feel like hearing a voice, reading silently, rehearsing a sentence, arguing with yourself or narrating an activity. The experience is verbal, but no intentional acoustic signal leaves your mouth.

Speaking aloud begins with many of the same language and speech-planning operations. You select words, organize syntax and prepare phonological patterns. It then continues into overt motor execution: the brain drives the larynx, tongue, lips, jaw and respiratory muscles, coordinating them quickly enough to create articulated sound. That sound travels through the air to the ears and produces measurable activity in auditory cortex.

The overlap is therefore substantial but incomplete. A person can activate speech-production regions while imagining a sentence, yet inner speech does not require the full chain of articulation and acoustic feedback. Conversely, overt speech includes internal planning but also requires precise movement, timing and online correction.

Inner speech and overt speech compared

Processing dimension Inner speech Speaking aloud
Motor output Speech planning may be engaged, but there is no intentional audible articulation. Recruitment varies with the form of inner speech. Full motor execution produces articulated sound through the larynx, tongue, lips, jaw and breathing system.
Auditory input Auditory imagery or perceptual simulation may occur without an external sound wave reaching the ears. Real acoustic feedback reaches the auditory system while the person speaks.
Predictive monitoring Corollary-discharge predictions can identify the verbal event as self-generated even when it is silent. Predictions are compared with actual vocal-auditory feedback; the response to self-generated sound is normally reduced.
Neural uniformity Heterogeneous: monologic, dialogal, elicited and spontaneous forms can rely on different combinations of networks. Task-dependent as well, but every overt utterance includes articulation and sound production.
Clinical relevance A faulty self-monitoring process is one hypothesis for why internally generated verbal activity can sometimes feel like an outside voice in auditory verbal hallucinations. Abnormal suppression of self-generated speech sounds is studied as a related self-monitoring disturbance.

How the brain predicts a voice before it hears it

The central idea is predictive self-monitoring. Before or during a movement, motor systems can send an efference copy, also called a corollary discharge, to sensory areas. This copy is a prediction of the sensory consequences expected from the movement. In speech, it can specify what the sound should be like before the ears receive the result.

A useful analogy is a phone that knows which notification it is about to play. Because the system issued the command, it does not treat the expected tone as surprising information. The speech system similarly prepares a prediction from the planned articulation. If the incoming auditory signal matches the prediction, the brain can classify the event as self-generated and reduce the response devoted to detecting an unexpected external sound.

In overt speech, the prediction can be checked against the actual acoustic signal generated by the vocal tract. In inner speech, there is no intentional vocal signal to measure, but a related prediction or simulated percept may still be produced. That is why predictive monitoring is relevant to both modes without making them identical.

Speaking-induced suppression

Ford and Mathalon’s review describes speaking-induced suppression (SIS) as the finding that sounds generated by a person’s overt speech elicit a smaller neurophysiological response in auditory cortex than comparable sounds generated externally. The reduction is not simply a failure to hear yourself. It is evidence that the brain has predicted the sensory consequence of its own action and discounted part of the expected input.

SIS is normally measured by comparing neural responses to self-produced speech sounds with responses to matched sounds delivered from outside. The important comparison is the source of the sound, not merely its loudness or linguistic content. A weaker auditory-cortex response to the self-generated version is consistent with an internal prediction suppressing expected sensory consequences.

Inner speech comes in several forms

Inner speech is not one uniform neural event. A 2025 narrative review by Dahò and Monzani surveyed 83 PubMed-indexed studies published from 1990 through 2024 and classified inner speech into four broad forms: monologic, dialogal, elicited and spontaneous.

Monologic inner speech

Monologic inner speech resembles a private narration or running commentary. It may describe what you are doing, rehearse a future action or evaluate a decision. Because it is directed toward oneself rather than toward an imagined conversational partner, it may place fewer demands on perspective-taking than dialogal inner speech.

Dialogal inner speech

Dialogal inner speech involves an exchange between distinct perspectives, such as silently answering a question, anticipating another person’s objection or replaying a conversation. Dahò and Monzani report that this form recruits theory-of-mind networks, the systems used to represent another person’s beliefs, intentions and point of view. The finding does not mean that every silent sentence activates a complete social-cognition circuit; it indicates that dialog-like phenomenology adds perspective-taking demands.

Elicited inner speech

Elicited inner speech is prompted by an instruction or task, for example silently repeating a phrase during an experiment. The 2025 review reports stronger activation of the left inferior frontal gyrus for task-elicited inner speech. Laboratory instructions make this form easier to measure, but they may not capture the spontaneous quality of everyday self-talk.

Spontaneous inner speech

Spontaneous inner speech arises without an explicit cue. It can appear as an unplanned thought, an emerging phrase or a sudden internal comment. The review associates spontaneous inner speech with Heschl’s gyrus, a region of auditory cortex. This association is consistent with perceptual simulation: the brain may represent the sound-like qualities of a voice even when no external sound is present.

Two complementary mechanisms explain silent verbal experience

Pratts, Pobric and Yao’s 2023 activation-likelihood-estimation meta-analysis supports two broad, partly complementary mechanisms.

Corollary-discharge mechanism

In this account, speech-production regions participate in inner speech and send predictive information to sensory areas. The model includes left inferior frontal regions among the production-related areas. The prediction helps the brain identify the verbal event as internally generated, even when overt articulation is withheld.

Perceptual-simulation mechanism

In the second account, inner speech relies more heavily on speech-perception regions. The person internally simulates what a spoken phrase would sound like, creating an auditory-like representation without an acoustic wave. This mechanism helps explain why inner speech can have a voice, rhythm or apparent location in subjective experience despite the absence of vocal movement.

The meta-analysis found that activation of speech-production regions was not observed in all inner-speech subtypes. That result is important: it rules against a single requirement that every silent thought must activate the same motor speech circuit. Different tasks and phenomenological forms can emphasize production predictions, perceptual simulation or both.

Which brain regions are involved?

Current evidence points to interacting networks rather than a single “inner-speech spot.” The exact pattern depends on what the person is doing, how inner speech is elicited and which aspect of the experience is measured.

  • Left inferior frontal regions: associated with speech planning and, in the 2025 review, stronger activity during task-elicited inner speech.
  • Auditory cortex: involved when speech is heard, imagined or simulated. Heschl’s gyrus, an auditory-cortex region, was linked with spontaneous inner speech in the 2025 review.
  • Superior temporal regions: participate in speech perception and are among the areas discussed in self-monitoring models.
  • Ventral premotor regions: form part of audiomotor networks that connect speech-related motor predictions with auditory representations.
  • Theory-of-mind networks: become more relevant when inner speech is dialogal and requires representing another person’s perspective.
  • Language-related white-matter pathways: provide connections among frontal, temporal and auditory-language areas; proposed abnormalities in these pathways are studied in clinical self-monitoring research.

A 2025 meta-analysis reported increased activity in motor-related regions during inner speech and described distinct, inversely coupled audiomotor networks involving ventral premotor and auditory regions. “Inversely coupled” describes a network relationship in which activity in one component tends to vary oppositely with activity in another under the studied conditions. It does not establish that one region alone creates inner speech.

Why does your inner voice sound different from your spoken voice?

Your inner voice is a constructed percept rather than the result of a completed vocal gesture. During silent thought, the brain can generate language plans and an auditory simulation without moving the larynx, tongue and lips enough to create an external sound. The resulting experience is therefore based on predicted or imagined speech qualities, not on the full acoustic feedback loop used during conversation.

When you speak aloud, the vocal tract produces a sound wave and the ears receive feedback while the brain is still planning the next sounds. That feedback supplies timing, pitch, intensity and pronunciation information that an entirely silent thought does not provide. The brain compares the incoming signal with its prediction, producing the suppression effect measured in SIS studies.

The difference is especially noticeable when you hear a recording of yourself. A recording presents your voice as an external acoustic signal, without the exact mixture of internally generated prediction and immediate self-produced feedback that accompanied the original utterance. Inner speech, ordinary self-hearing and playback are consequently three related but distinct perceptual situations.

How does the brain know a thought is self-generated?

Self-generation is inferred from coordination among intention, motor planning, prediction and sensory evidence. If a verbal event follows your own speech plan and the sensory consequences match the expected pattern, the brain has reasons to label it as self-produced. A mismatch, by contrast, can attract attention because it violates the prediction.

  1. Form a verbal plan. The language system selects the intended words and phonological sequence.
  2. Prepare an action or simulation. Motor-related regions can prepare articulation, while perceptual systems can simulate the expected sound.
  3. Send a predictive copy. An efference copy or corollary discharge informs sensory regions about the anticipated consequence.
  4. Compare prediction with evidence. In overt speech, auditory cortex receives the actual sound. In inner speech, the comparison involves internally generated perceptual representations rather than an external acoustic signal.
  5. Classify the source. A close match supports the experience that the verbal event came from oneself; an unexpected or poorly predicted event is more likely to be experienced as externally caused.

This is a model of source attribution, not a conscious checklist. The operations occur rapidly and usually outside awareness.

What auditory verbal hallucinations can—and cannot—tell us

In psychosis research, the inner-speech model proposes that failures of corollary discharge or self-monitoring can make internally generated verbal activity feel as though it came from an outside speaker. The 2021 review of this model discusses proposed involvement of auditory cortex, the left inferior frontal gyrus, superior temporal regions and language-related white-matter pathways.

These findings concern clinical groups and a proposed mechanism for a particular symptom. They do not mean that ordinary inner speech is pathological, that hearing an inner voice is itself evidence of illness, or that one faulty region explains every hallucination. The model is clinically useful because it links a subjective source-attribution problem to measurable prediction and sensory processes, but it remains a network-level account.

  • EEG: records electrical activity with millisecond timing, which is useful for comparing preparation, phonological processing and feedback-related responses.
  • fNIRS: estimates changes in blood oxygenation near the cortical surface and can be used in more naturalistic or developmental settings.
  • fMRI: provides detailed spatial maps of activity but measures blood-flow changes more slowly than neural events unfold.
  • PET: measures metabolic or neurochemical signals and can reveal activity patterns that complement other imaging methods.
  • Activation-likelihood meta-analysis: combines reported coordinates across experiments to identify locations that recur across studies; it cannot erase differences in task instructions or subjective experience.

A developmental EEG/fNIRS study comparing inner and overt speech interpreted differences in terms of motor preparation, phonological processing and auditory-feedback control. Developmental results should not automatically be generalized to every adult task, because maturation, instructions and measurement conditions can change the observed pattern.

Likewise, a stronger response in one region during one paradigm does not prove that region is necessary for all inner speech. Tasks can require silent repetition, spontaneous thought, imagined dialogue or explicit perspective-taking, and those demands are not interchangeable.

What the evidence establishes

  • Inner speech and overt speech use partially overlapping language and speech networks.
  • Overt speech uniquely adds intentional articulation and real acoustic feedback.
  • Predictive self-monitoring helps the brain identify the consequences of self-generated speech.
  • Speaking-induced suppression is a measurable reduction in auditory-cortex response to self-generated speech compared with comparable external sounds.
  • Inner speech has multiple forms, and their neural patterns differ.
  • Both production-related prediction and perception-like simulation can contribute to silent verbal experience.
  • There is no single universal inner-speech region or universal inner-versus-overt activation signature.

A practical mental model

Think of speaking as a loop and inner speech as a partly simulated version of that loop. The loop begins with a language plan, sends commands to the speech apparatus, generates sound and feeds the result back through hearing. Prediction runs ahead of the sound so the brain can recognize expected consequences and focus on errors.

Inner speech keeps the language plan and may keep the prediction or auditory simulation, but it withholds the final audible articulation. Depending on the type of thought, the brain may emphasize speech-production planning, perceptual imagery, spontaneous auditory simulation or social perspective-taking. That is why two people can both report “talking silently” while engaging somewhat different neural processes.

Bottom line

Inner speech is not simply ordinary speech with the volume turned down. It is a family of covert verbal processes that can recruit speech-production predictions, auditory simulation and, for imagined conversations, social-cognition networks. Speaking aloud includes those planning operations but adds coordinated movement and actual acoustic feedback. Predictive self-monitoring links the two modes and normally helps the brain recognize its own verbal activity, while differences in that monitoring are one proposed contributor to externally experienced voices in clinical conditions.
