Prescription vitamin D receptor (VDR) agonists such as paricalcitol are being studied for whether they can change the fibroblasts, immune cells and other features surrounding pancreatic tumors. In a small 2026 clinical run-in, tumor biopsies showed changes consistent with VDR pathway activity and altered immune-cell positioning. Those are biological signals—not evidence that the drugs improve survival or how well treatment works. The findings do not show that vitamin D supplements have the same effects.
How might VDR drugs change the environment around a pancreatic tumor?
Pancreatic tumors can be surrounded by dense stromal tissue that includes activated cancer-associated fibroblasts (CAFs), cells that help shape the local environment. Researchers are investigating whether activating VDR in fibroblasts and pancreatic stellate cells can shift some of these cells toward a less activated state and change their interactions with tumor and immune cells.
The goal is not simply to remove a protective “shield.” Fibroblast populations and stromal signals are varied, and changing one signal may have more than one effect. A change in the surrounding tissue could be biologically meaningful without necessarily making a tumor easier to treat.

What did the 2026 paricalcitol study find?
A 2026 Nature Cancer study randomized 36 people with metastatic pancreatic cancer to first-line gemcitabine plus albumin-bound paclitaxel (nab-paclitaxel) with placebo, intravenous paricalcitol or oral paricalcitol. Researchers collected tumor biopsies before and during treatment. The run-in’s primary focus was safety, alongside analyses of tissue changes; it was not designed to establish treatment efficacy.

- Fibroblast findings: In the paricalcitol arms, on-treatment biopsies showed a lower proportion of αSMA-positive fibroblasts and changes in a fibroblast VDR-activation signature.
- Immune-cell findings: Biopsies also showed greater density of CD8-positive T cells and more spatial colocalization of those cells with tumor cells.
- VDR expression: The study reported variable nuclear VDR expression among participants, including expression in tumor, immune and stromal cells. Within the paricalcitol arms, VDR expression was reported as predictive of tumor response. This was an exploratory observation, not a validated test for selecting treatment.
These biopsy results are pharmacodynamic observations: they suggest that treatment was associated with changes in the tumor tissue. They do not establish that the changes caused better tumor control, longer survival or improved response rates.
Why do laboratory findings point to both possible benefits and trade-offs?
A 2020 Scientific Reports study tested calcipotriol in human pancreatic CAF and T-cell laboratory models. Calcipotriol reduced fibroblast proliferation and migration and reduced release of several signaling factors, including prostaglandin E2, IL-6, periostin and leukemia inhibitory factor. But the same study also reported PD-L1 upregulation and reduced T-cell proliferation and production of IFN-γ, granzyme B and IL-17 in its cell models.

This mixed picture cautions against describing VDR activation as uniformly immune-enhancing. It also matters that calcipotriol in laboratory models is not paricalcitol given to patients: findings for one compound, dose and experimental setting cannot automatically be transferred to another drug or to clinical outcomes.
What do the findings mean for treatment?
The run-in provides evidence that paricalcitol was studied alongside gemcitabine and nab-paclitaxel in a closely monitored clinical setting and that biopsies showed tissue changes consistent with VDR pathway engagement. Dana-Farber’s summary emphasizes that the study was not intended to determine how well the approach treats pancreatic cancer. The run-in does not show that paricalcitol improves response rates or survival, or that it should be used outside a clinical trial.

Development context further limits efficacy claims: a 2025 review reports that the planned phase II component of the NCT03520790 program was not pursued because of futility based on NAPOLI-3. That does not negate the run-in’s safety and biopsy findings, but it means those findings should not be presented as proof of clinical benefit.
What safety signal was reported?
In the small oral-paricalcitol arm, five participants (42%) had grade 2–4 hypercalcemia and required dose reduction, according to the 2026 Nature Cancer paper. This figure describes that particular study arm, not the expected risk for everyone who might receive paricalcitol. It is a material safety finding, and it underscores that the drug was used with clinical monitoring.

Are vitamin D supplements equivalent to paricalcitol?
No. Paricalcitol and calcipotriol are pharmacologic VDR agonists with specific drug properties, doses and routes; ordinary dietary vitamin D supplements are not interchangeable with them. The pancreatic cancer findings described here do not support taking supplements, starting or changing a VDR drug, or altering cancer treatment. Patients should discuss treatment and trial questions with their oncology team.

What other paricalcitol studies are listed?
The National Cancer Institute lists research in other pancreatic cancer settings, including a phase II study combining paricalcitol and hydroxychloroquine with gemcitabine and nab-paclitaxel for advanced or metastatic disease, and a phase Ib neoadjuvant study for resectable disease. The listed objectives include response, safety, survival and exploratory study of fibroblasts, immune cells, or genetic and histologic markers. Trial listings describe protocols and objectives, not results; check ClinicalTrials.gov or the study site for current status and eligibility.
