If you suspect alpha-gal syndrome (AGS), keep a dated record of suspected exposures, meals, symptoms, timing, and treatment, then bring it to your healthcare provider. Add your medication list, tick-bite or outdoor-exposure history, and photos of visible symptoms. A diary can help your clinician assess the pattern; it cannot diagnose AGS. Do not deliberately eat a suspected trigger to test yourself.

What to record for each reaction

Use a notebook, phone note, spreadsheet, or form—whichever you can keep up with and share easily. For each suspected episode, capture the details while they are fresh:

  • Exposure: date and time, everything eaten, ingredients, approximate amount, and preparation. Note restaurant or package details and possible cross-contact when known.
  • Symptoms: what happened, when it began, how severe it felt, and when it ended. Include symptoms even if they seem unrelated to the meal.
  • Treatment: what you took or did, when you did it, and whether it helped.
  • Other context: medicines and supplements taken around the episode, plus factors such as exercise, alcohol, illness, or environment. Record these without assuming they caused the reaction.
  • Evidence: photographs of visible rash or swelling, and ingredient labels or product information when available.

Record suspected tick bites and outdoor exposure separately, including approximate dates and locations if you know them. Food-allergy appointment guidance from HealthLink BC, Cambridge University Hospitals, and Lancashire Teaching Hospitals supports recording meals, symptoms, treatment, medicines, and possible co-factors. One hospital suggests noting food and activities in the 4–6 hours before symptoms and medicines in the preceding 24 hours; this is an example, not a universal rule. Follow your own clinician’s directions.

Why timing matters in suspected AGS

Alpha-gal reactions are often delayed rather than immediate. The CDC says symptoms usually appear 2–6 hours after exposure to products containing alpha-gal, including red meat or dairy. Timing and symptoms can vary between people and from one episode to another, and a person may not react every time. Record the meal or exposure time and the symptom-onset time separately; a suspected trigger alone does not show whether the pattern fits AGS. CDC: Alpha-gal syndrome signs and symptoms

What to bring to the appointment

  • Your reaction diary or a concise timeline showing suspected food or exposure, amount when known, delay before symptoms, severity, duration, and what helped or worsened them.
  • Your tick-bite and outdoor-exposure history.
  • A current list of prescription and over-the-counter medicines, vitamins, and supplements, including doses.
  • Photos of visible symptoms and relevant ingredient or package information.
  • A written list of questions. For example: Could this pattern fit AGS or another condition? What testing is appropriate? How should test results be understood alongside my symptoms? What should I avoid while waiting for evaluation? What is my emergency plan, and should I have a prescription for epinephrine?

If helpful, ask a friend or family member to come along. Mayo Clinic recommends preparing lists of symptoms, tick bites, medicines, and questions; HealthLink BC also recommends bringing photos and a medication list. Clinics may have different procedures, so ask yours whether to hold any medicine or bring a food sample. Do not stop prescribed medicine based on general online advice. How clinicians evaluate possible alpha-gal syndrome

CDC guidance updated January 5, 2026, says evaluation combines a detailed medical history, physical examination, and a blood test for alpha-gal-specific IgE. The result is not a diagnosis on its own: the CDC states, “A positive alpha-gal specific IgE test does not mean a person has AGS.” Some people have a positive result without symptoms, particularly in areas where lone star ticks are present. Clinicians interpret the result alongside symptoms, delayed timing, and tick-bite or outdoor-exposure history; allergy skin tests may also be used. CDC: Clinical diagnosis and testing

Safety while you are waiting for evaluation

Do not retry a food you suspect is causing an allergy just to make the diary more complete. Mayo Clinic advises discussing suspected food allergy and avoidance with a healthcare provider rather than trying the food. One reaction-free exposure also does not establish that a product is safe for you.

CDC describes AGS symptoms that can include hives, gastrointestinal problems, cough, breathing difficulty, swelling, dizziness, low blood pressure, and anaphylaxis. A severe allergic reaction can be life-threatening: seek immediate emergency care for severe symptoms. If you use prescribed epinephrine, CDC says to seek emergency care afterward as well. Ask your provider how to recognize anaphylaxis and prepare for a reaction. CDC: Alpha-gal syndrome signs and symptoms

Tolerance differs, so work with your clinician on individual avoidance rather than drawing conclusions from a single episode or symptom-free exposure. CDC: Managing alpha-gal syndrome
