Call your local emergency number immediately if someone exposed to heat becomes confused, acts unusually, has slurred speech, has a seizure, collapses, or loses consciousness. These can be signs of heat stroke. Start cooling them while help is on the way; do not wait for a thermometer reading. If they are alert and symptoms look more like heat exhaustion, move them somewhere cool, cool their skin, and offer small sips of fluid. Get medical help if they worsen, vomit, or do not improve.
Heat exhaustion and heat stroke can share symptoms, so this is first-aid guidance—not a way to diagnose someone remotely. Sweating or dry skin alone cannot reliably tell you which condition is present.
Signs that mean emergency care
Heat exposure followed by a change in mental state is the clearest reason to treat the situation as possible heat stroke. Call your local emergency number if the person is confused, unusually agitated or behaving differently, has slurred speech, has a seizure, faints or collapses, or becomes unconscious. A very high temperature or other signs of heat stroke are also cause for emergency care.

- Do not wait for every symptom to appear.
- Do not delay the call to find or use a thermometer.
- Do not assume the person is safe because they are still sweating. Heat stroke can involve hot, dry skin or profuse sweating.
How the symptoms may differ

These are patterns, not a definitive test. In particular, dry skin is not required for heat stroke, and sweating does not rule it out.

What to do if heat stroke is possible
- Call your local emergency number now. Use the number for your location; 911 applies in the United States and 999 in the United Kingdom, not everywhere.
- Stay with the person. Move them to shade or a cool indoor place if you can do so safely, and remove outer or unnecessary clothing.
- Start cooling immediately. Use a cold-water bath if feasible. Otherwise wet the skin or clothing, apply cold wet cloths, and circulate air. Cold wet cloths or ice may also be placed on the head, neck, armpits, and groin.
- Do not give anything by mouth if the person is unconscious or vomiting. Follow the emergency dispatcher’s instructions.
What to do when heat exhaustion seems more likely
If the person is alert, has no emergency warning signs, and symptoms are consistent with heat exhaustion, begin cooling and keep watching for changes.
- Move them to a cool place and remove unnecessary clothing.
- Cool their skin with cool water, a shower or sponge, wet cloths, or airflow.
- Offer frequent small sips of cool water or an appropriate rehydration drink only if they can swallow safely.
- Get medical help promptly if symptoms worsen, vomiting occurs, or they do not improve. Confusion, a seizure, fainting, collapse, or another mental-status change calls for emergency services.
When to escalate if symptoms seem less severe
Guidance differs on how quickly every case of heat exhaustion should be assessed. The NHS advises emergency help if someone remains unwell after 30 minutes of cooling and fluids; CDC/NIOSH advises medical evaluation for heat exhaustion and says to call 911 if medical care is unavailable. MedlinePlus also lists worsening or non-improving symptoms among reasons for emergency help. These are source-specific recommendations, not one universal timing rule. If the person is getting worse, vomiting, or not improving, seek medical help rather than relying on a fixed waiting period.
