There is no single score, wearable reading or feeling of confidence that proves a footballer is fit to start. Readiness depends on the player’s current symptoms and injury status, recent demands and recovery, football-specific function, and mental readiness. If an injury is suspected or rehabilitation is unfinished, medical staff—not a generic self-check—should assess the player.
Start by separating injury or illness from ordinary fatigue
Being tired after a match is not the same as being injured, but a new or worsening symptom changes the decision. Pain, a suspected injury, or an unresolved rehabilitation issue should be raised with the club’s medical staff before selection is considered. Do not use a wearable score or a self-assessment to override that concern.
For a player returning after injury, elapsed time alone is not clearance. The football hamstring Delphi consensus defines return to play as “the moment a player has received criteria-based medical clearance and is mentally ready for full availability for match selection and/or full training.” That definition makes medical clearance and mental readiness part of the return—not optional extras.
What should staff consider before a start?
Symptoms and injury status
Establish whether there is a current injury, whether symptoms are changing, and whether the player has completed the rehabilitation and assessment required for that injury. A player without a known injury may still need evaluation for a new symptom; a player who feels ready is not thereby medically cleared.

Recent demands and recovery
Consider the player’s recent training and match demands alongside recovery, travel, sleep, emotional wellbeing and other stressors. Load includes more than the work recorded during training: non-sport demands can matter too. Recovery varies between players and can also vary for the same player over time, so compare the current response with that individual’s pattern rather than applying a universal cutoff.
Performance and football-specific function
For an injury return, assess whether the player can perform relevant football actions progressively and at the level required. The hamstring consensus highlights repeated sprint ability, deceleration drills, single-leg bridge testing and position-specific rehabilitation using GPS-targeted match demands. Experts also emphasized explosive movements. Completing a rehabilitation plan in general terms is not a substitute for demonstrating relevant function.

Mental readiness
Fear, reduced confidence or unwillingness to perform at full availability matters, particularly during return from injury. Mental readiness is part of the hamstring consensus definition, but confidence by itself does not establish that the player has met medical or functional criteria.

How monitoring can help—and where it stops
Monitoring is useful when it helps staff understand how a player responds to demands and recovery. The athlete load-monitoring and IOC load consensuses support interpreting information individually and in context. A single daily number cannot capture every relevant symptom, performance change or life stressor, and no cited consensus validates a wearable as a standalone fit-to-start test.

| Approach | What it can show | How to use it | What it cannot establish alone |
|---|---|---|---|
| External-load measures, such as movement or GPS-derived activity | Work performed or movement demands, depending on the measure used. | Compare with the player’s own demands and with relevant training or match requirements; use field work to reflect position-specific actions where appropriate. | Medical clearance or readiness based on symptoms, recovery and mental readiness. |
| Internal-response measures, such as perceived effort or physiological response | How the player experiences or physiologically responds to a workload. | Interpret alongside recent load, recovery, symptoms and the player’s usual response. | A diagnosis, injury clearance or universal threshold for starting. |
| Football-specific field assessment | Performance of relevant actions such as sprinting, deceleration and explosive movements. | Progressively assess the actions required for the player’s role and return plan, with medical staff involved for injury return. | Readiness for every injury or every player based on one test. |
The table describes different kinds of information, not competing clearance systems. Monitoring can inform a staff decision; it does not replace clinical assessment when injury is involved.
Why there is no universal return-to-play checklist
Evidence and expert agreement have limits. In the hamstring Delphi consensus, experts did not reach consensus on whether similar eccentric hamstring strength must be a return criterion. The Bern return-to-sport consensus also notes that strong evidence for criteria across many common injuries is lacking. The Italian football muscle-injury consensus distinguishes return to training from return to play and discusses clinical, field, laboratory and GPS-related criteria; those distinctions reinforce that a single self-reported “100%” is not a complete assessment.

These sources do not establish one validated consumer checklist for all footballers, injuries, ages, playing levels or positions. The practical approach is individualized: assess the injury and symptoms, review the player’s response to demands and recovery, test relevant function progressively when returning from injury, and involve the appropriate medical and coaching staff.
Who makes the final decision?
Medical clearance and tactical selection are different decisions. Medical staff address whether the player meets the relevant health and return criteria; coaches and team staff decide selection and availability within the team’s needs. The hamstring consensus recognizes that non-medical team decisions can affect availability, but they do not replace the medical component of clearance.
