The most helpful response is calm, practical, and led by the survivor: make sure they are safe, listen without demanding details, reduce immediate stress, support basic routines, and help them reach qualified care. Fear, intrusive memories, sleep or concentration problems, anger, numbness, avoidance, and feeling constantly on guard can occur after a mass shooting. Many people improve with time; PTSD is considered when symptoms persist and interfere with daily life. A supporter cannot diagnose PTSD, but can notice warning signs, reduce pressure, and connect the survivor with help.
If the person may hurt themselves or someone else, cannot stay safe, or has suicidal thoughts, call or text 988 in the United States. For immediate physical danger or a life-threatening medical emergency, call 911 or go to the nearest emergency department.
Start with safety, medical needs, and a quieter setting
After a mass shooting, psychological support comes after immediate physical safety. Ask a short, concrete question: “Are you physically safe right now, and do you need medical care?” Do not assume a person who looks calm is uninjured or out of danger. Help them move away from weapons, crowds, noise, flashing cameras, and ongoing threats when it is safe to do so.

- Arrange urgent treatment for bleeding, breathing trouble, head injury, severe pain, intoxication, or other medical concerns.
- Offer water, food, warmth, a place to sit, access to prescribed medication, a phone charger, transportation, and a trusted person.
- Ask whether they want company or quiet. Reduce the number of people asking questions.
- Share only verified information about safety and available services. Rumors and repeated alerts can intensify fear.
- Protect the survivor from reporters, photographers, and unwanted interviews. Ask before giving anyone their name or contact information.
SAMHSA’s mass-violence guidance emphasizes medical care, a safe setting, basic comfort, emotional stabilization, accurate threat information, recovery information, and protection from media pressure. A survivor may be disoriented, numb, angry, tearful, restless, or unable to make decisions. Keep choices simple and offer one step at a time.

What to say—and how to listen
Trauma-informed communication gives the survivor control over disclosure and pace. Listen more than you speak. Acknowledge what happened without trying to explain it away or predict recovery.
Useful phrases
- “I’m here with you.”
- “You do not have to tell me details.”
- “That sounds frightening. Your reaction makes sense after what happened.”
- “Would you like company, quiet, or help finding care?”
- “Would you like me to sit here, drive you, or help with a phone call?”
- “Can I touch your shoulder, hug you, or would you rather have space?”
Ask permission before touching, driving, entering a room, making decisions, or contacting relatives. Give the person time to answer. If they cannot choose, offer two manageable options rather than an open-ended list.
Listening without forcing a retelling
Do not demand a detailed account, repeatedly ask what happened, or treat disclosure as a test of trust. Some survivors want to talk; others need silence. Follow their lead and reflect what they say: “You felt trapped,” or “You are worried about going back there.” Avoid investigative questions unless safety or medical care requires specific information.
NIMH advises emotional support, understanding, patience, encouragement, careful listening, positive distractions, and practical help with appointments. Validation is not the same as diagnosing. Say that the reaction is understandable without declaring that the person has PTSD.

Help during panic, flashbacks, or overwhelming emotion
A flashback, panic episode, or intense reminder can make the present feel dangerous. First check whether there is an actual current threat. Speak slowly, use the person’s name if they welcome it, and keep your instructions brief. Do not shout, crowd them, block an exit, or argue about whether the danger “is real.”
A simple grounding sequence
- Orient to the present: say where you are and what day or time it is, if the person is receptive. “You are in your apartment with me. The door is locked, and we are safe right now.” Do not make a safety promise you cannot keep.
- Offer a choice: “Would you like to try a grounding exercise, sit quietly, or step outside?” Respect a no.
- Slow the body: invite, rather than command, a slower exhale, feet supported on the floor, or relaxing the jaw and shoulders.
- Name the environment: suggest noticing a few things they can see, hear, or feel. Stop if this increases distress.
- Reduce stimulation: move to a quieter room, dim lights, silence nonessential notifications, and let them choose where to sit.
- Wait: remain present without demanding conversation. Recovery from a surge can take time.
Grounding is a short-term support, not a cure and not a substitute for assessment. If the person becomes confused, loses consciousness, has trouble breathing, reports a medical emergency, or remains unable to stay safe, seek emergency help.
Make the next hours and days manageable
Trauma can disrupt eating, sleeping, concentration, work, school, and relationships. Practical help is often more useful than advice. Ask what single task would make the next few hours easier.
Support basic routines
- Offer regular meals, water, and access to prescribed medication.
- Help create a low-pressure sleep routine: a predictable bedtime, less disturbing media, and a safe sleeping arrangement.
- Break responsibilities into small steps, such as taking a shower, replying to one message, or attending one appointment.
- Offer transportation or accompaniment to medical and mental-health visits.
- Keep expectations realistic. A survivor may need reduced workload, time off, or help with childcare.
NIMH recommends supportive relationships, routines for meals, exercise or mindfulness, realistic goals, and avoiding alcohol or drugs. Invite a short walk or another familiar activity only if the person wants it. Do not turn exercise, breathing, prayer, or meditation into a demand or claim that it will resolve trauma.

Reduce reminders without taking control away
Ask which reminders are most difficult and help the survivor choose boundaries. Common triggers can include breaking-news alerts, social-media videos, interviews, anniversaries, crowds, loud noises, locations connected with the shooting, or contact with other witnesses.
Build a trigger plan
- Mute news alerts and unfollow accounts that increase distress. VA advises limiting news and social-media exposure when it worsens symptoms.
- Choose a trusted person to screen calls, messages, or media requests.
- Plan a quieter route, a companion, a seat near an exit, or a signal to leave a crowded place.
- Decide in advance whether to attend vigils, memorials, court proceedings, or anniversaries. Attendance is a choice, not an obligation.
- Keep an exit plan for places that cannot be avoided, and arrange transportation that lets the survivor leave without explanation.
Do not insist on exposure to a location, crowd, image, or conversation as an informal “test.” Trauma treatment should be planned with a qualified clinician.
When to encourage professional evaluation
Suggest an appointment when distress continues, worsens, or interferes with sleep, work, school, relationships, self-care, or ordinary activities. Also encourage evaluation when the survivor is using substances to cope, feels persistently detached or hopeless, has severe anger, cannot leave home, or is struggling to care for a child.
Recommended Free Tools
A primary-care clinician, psychologist, psychiatrist, or clinical social worker can assess symptoms, consider medical causes, discuss treatment, and make referrals. Bring a brief description of when symptoms began, what triggers them, how sleep and functioning have changed, current medications, substance use, previous trauma, and any safety concerns. The survivor does not need to tell every event detail at the first visit.

How a supporter can make care easier
- Ask what kind of help the survivor wants: finding names, making the call, arranging transport, or attending the appointment.
- Use the clinician’s exact credentials and specialty when searching, and ask about trauma experience, language, cultural fit, cost, insurance, and appointment availability.
- Write down questions and symptoms before the visit. The survivor can hand the list to the clinician if speaking is difficult.
- Respect privacy. Ask before joining an appointment or receiving updates.
- Follow the agreed safety and treatment plan rather than improvising new coping challenges.
Evidence-based treatment options to discuss with a clinician
The VA’s PTSD resources and the 2023 VA/DoD guideline describe assessment and several treatment levels. Clinical judgment is still necessary; the best option depends on symptoms, medical history, preferences, access, and safety.
| Option | What it may address | Format or intensity | Questions to ask |
|---|---|---|---|
| Trauma-focused psychotherapy, including Cognitive Processing Therapy | Trauma-related beliefs, avoidance, distress, and functioning | Usually structured individual therapy; the clinician determines pace and suitability | What does each session involve? How will safety and distress be monitored? |
| Other individual psychotherapy | PTSD symptoms, anxiety, depression, sleep, and coping | Individual sessions; approach and duration vary | What evidence supports this approach for the survivor’s symptoms? |
| Medication | Selected PTSD or related symptoms | Prescribed and monitored by an appropriate clinician | What benefits, side effects, interactions, and follow-up are expected? |
| Family therapy | Communication, caregiving, relationship strain, and shared recovery | Sessions with selected family members | Who will participate, and how will confidentiality work? |
| Group therapy | Support, skills, and connection with people facing related difficulties | Facilitated group; privacy and fit vary | Who leads it, who attends, and what happens if the group feels overwhelming? |
| Residential or inpatient care | Severe impairment, inability to remain safe, or need for intensive stabilization | Higher level of care, determined clinically | What safety problem requires this level, and how will step-down care be arranged? |
Compare options by symptom target, evidence and guideline support, individual versus family or group format, intensity and setting, trauma-focused versus general symptom management, access and cost, language and cultural fit, and the survivor’s preferences. A supporter’s role is to help the person ask questions, not to select a treatment without them.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Recognize a crisis and act directly
Ask plainly and calmly if you notice suicidal talk, a plan, access to lethal means, threats toward another person, severe confusion, inability to meet basic needs, or escalating substance use. Asking about suicide does not plant the idea; it clarifies risk and opens a path to help.

What can make things worse
- Demanding a detailed retelling, insisting on forgiveness, minimizing the shooting, or saying the survivor should be “over it.”
- Pressuring the person into interviews, public vigils, group debriefings, or visits to the scene.
- Using alcohol, recreational drugs, unprescribed medication, or forced exposure as coping strategies.
- Arguing about symptoms, diagnosing from an online checklist, or promising that a technique will cure PTSD.
- Making safety promises you cannot keep or promising confidentiality when there is imminent danger.
- Taking over every decision. Protection should not become unnecessary control.
SAMHSA materials caution that poorly timed psychological debriefing can interfere with natural recovery. Trauma treatment and structured debriefing should be handled by qualified professionals, at a time and pace appropriate to the survivor.
Children, teenagers, and families
Children and teens may show fear, sleep problems, guilt, anger, revenge thoughts, withdrawal, irritability, physical complaints, or difficulty at school. Keep explanations simple and truthful: say that a dangerous event happened, adults are working to keep people safe, and the child can ask questions. Do not provide graphic details or make promises about events you cannot control.
Support a child’s recovery
- Keep dependable caregivers, school routines, meals, bedtime, and familiar activities when possible.
- Offer choices appropriate to age and let the child express feelings through words, play, drawing, or quiet.
- Limit repeated exposure to graphic news and adult conversations.
- Tell teachers or school counselors what support the child needs, with the family’s permission.
- Seek pediatric or mental-health help when symptoms persist, intensify, or change functioning, relationships, sleep, or safety.
Family members can also develop distress, particularly when they have prior trauma, ongoing stress, limited support, or a history of substance-use problems. Do not assume the person who was not physically present is unaffected. Offer each family member a path to care while respecting the survivor’s privacy.
Protect your own capacity to help
Supporting a survivor can expose you to frightening stories and sustained stress. Set clear limits about when you can visit, drive, answer calls, or manage practical tasks. Share responsibilities with trusted people instead of becoming the only support.
- Maintain your own meals, sleep, movement, medical care, and social contact.
- Take breaks from news and social media if they increase your distress.
- Avoid using alcohol or drugs to manage secondary trauma.
- Seek counseling or a support service if you develop intrusive memories, panic, numbness, sleep problems, or hopelessness.
- Call 988 yourself if you are struggling or having thoughts of suicide.
